WHILE EN ROUTE, LEFT ENGINE STARTED TO RUN ROUGH. PILOT FEATHERED PROPELLER AND SECURED ENGINE. AN UNEVENTFUL LANDING WAS MADE. MAINTENANCE FOUND NR 1 CYLINDER CRACKED. CYLINDER REPLACED WITH AN OVERHAULED UNIT. ENGINE GROUND RUN AND RETURNED TO SERVICE. (X)
WHILE IN-FLIGHT, THE CREW NOTICED OIL PRESSURE STARTING TO GRADUALLY DROP. SHUT DOWN ENGINE AND FEATHERED PROPELLER. MAINTENANCE FOUND NR 8 CYLINDER CROSS-CONNECT TUBE HAD SPLIT. REPLACED HOSE, RAN ENGINE, NO LEAKS NOTED. RETURNED TO SERVICE. (X)
IN CRUISE FLIGHT, ENGINE STARTED TO RUN ROUGH. ENGINE SHUT DOWN AND PROP FEATHERED. MAINTENANCE FOUND NO COMPRESSION ON REAR CYLINDERS. SUBMITTER SUSPECTED REAR CAM FAILURE. ENGINE TO BE REMOVED AND REPLACED.
AFTER LEVELING OFF, ENGINE STARTED TO RUN ROUGH AND BACKFIRE. SECURED ENGINE AND FEATHERED PROPELLER. MAINTENANCE FOUND CYLINDER NR 1 CRACKED. REPLACED CYLINDER WITH AN OVERHAULED UNIT. AIRCRAFT GROUND RUN AND CHECKED SATISFACTORY.
APPROXIMATELY 45 MINUTES AFTER TAKEOFF, PILOT NOTICED OIL PRESSURE DROPPING. ENGINE WAS SECURED AND PROPELLER FEATHERED WHEN MINIMUM PRESSURE WAS OBTAINED. MAINTENANCE FOUND THE ENGINE OIL OUT TO OIL COOLER IN LINE HAD BROKEN. LINE WAS REPLACED AND OIL TANK REFILLED. ENGINE GROUND RAN, NO LEAKS NOTED. AIRCRAFT RETURNED TO SERVICE.
APPROXIMATELY 30 MINUTES AFTER DEPARATURE, FLIGHT CREW NOTICED OIL LEAK IN LEFT ENGINE. CREW DIVERTED BACK TO AWM. MAINTENANCE FOUND CYLINDER NR 13 HEAD SEPARATING FROM BARREL.
WHILE IN CRUISE FLIGHT, THE FLIGHT CREW NOTICED OIL COMING FROM THE ENGINE VENT. THE ENGINE WAS SECURED AND THE PROPELLER FEATHERED. AN UNEVENTFUL LANDING WAS MADE. INSPECTION FOUND CYLINDER NR 5 WITH NO COMPRESSION. CYLINDER TO BE REMOVED AND FURTHER TROUBLESHOOTING DONE IF THE CYLINDER IS NOT THE PROBLEM.
WHILE EN ROUTE, ENGINE STARTED TO RUN ROUGH. ENGINE WAS SECURED. FOUND NR 11 CYLINDER HEAD CRACKED. REPLACED NR 11 CYLINDER WITH A YELLOW TAGGED UNIT. ENGINE OPS CHECKED SATISFACTORY.
