DURING INSPECTION OF THE HYDRAULIC POWER PAK IT WAS DISCOVERED THAT THE CHANNEL THAT REINFORCERS THE NOSE WHEEL TUNNEL WHERE THE NOSE GEAR ACTUATOR ATTACHES HAD CRACKED ON THE FORWARD CUTOUT FOR THE HYDRAULIC POWER RELIEF VALVE THE CHANNEL WAS REMOVED AND INSPECTED WHERE IT WAS DISCOVERED THAT THE OUTBOARD BOLT HOLE FOR THE NOSE GEAR ACTUATOR BRACKET WAS ELONGATED AND CRACKED ON THE BOTTOM SIDE. THE PART WAS REMOVED AND REPLACED IT IS RECOMMENDED THAT THE NOSE GEAR ACTUATOR ATTACH BRACKET BOLTS BE CHECKED PERIODICLY FOR SECURITY DURING LANDING GEAR INSPECTIONS AS IT IS SUSPECTED THAT THE LOOSE BOLT CAUSED THE PART TO STRESS AND CRACK DURING GEAR OPERATIONS.
THIS PITCH COMPUTER WAS A FACTORY REPAIR WHICH UPON RETURN FAILED IN THE SAME MANNER. WHEN INSTALLED THE AIRCRAFT WOULD PITCH UP AND DOWN 5 DEGREES WHILE THE AUTOPILOT WAS ENGAGED FOR CRUISE. THIS AUTOPILOT SYSTEM SEEMS TO BE PARTICULARLY SENSITIVE TO ANY AFT LOADS WITHIN THE ENVELOPE AND COULD BE A FACTOR IN THE PORPOISING PROBLEM. THE UNIT WAS REMOVED AND REPLACED WITH A LOANER UNIT.
PILOT REPORTED AIRCRAFT PORPOISES 5 DEGREES UP AND DOWN IN CRUISE. AUTOPILOT PITCH COMPUTER REMOVED AND REPLACED WITH LOANER UNIT. THIS PROBLEM HAS OCCURRED IN THE PAST IT IS SUSPECTED THAT THE SERVO DRIVE SPEED IS TOO SLOW. SUGEST ADJUSTING THE SPEED AND PERHAPS MOVE THE MOUNT LOCATION CLOSER TO THE ELEVATOR BELLCRANK TO LESSEN VECHANICAL DELAY IN PITCH FROM COMPUTER TO SERVO.
DURING INSPECTION, METAL CONTAMINATION WAS OBSERVED IN THE OIL FILTER. A CHECK WAS MADE OF VALVE LIFT AND IT WAS DETERMINED THAT THE NR 1 AND NR 2 INTAKE LOBES ON THE CAMSHAFT WERE EXCESSIVE. NR 2 CYLINDER WAS REMOVED AND DAMAGE ON THE CAM LOBES AND TAPPET FOLLLOWERS WERE OBSERVED. ENGINE WAS REMOVED FROM SERVICE.
DURING INSPECTION IT WAS NOTICED THAT THE FUEL PUMP WAS LEAKING FROM THE PARTING SURFACE AND THE PUMP BODY. THIS IS THE SIDE WITH THE LADDER SEAL TO THE PUMP BODY. THE PUMP WAS REMOVED FROM SERVICE. THIS IS A (M) STYLE PUMP.
DURING INSPECTION, FUEL STAIN WAS OBSERVED FROM THE DISCHARGE FITTING ON THE PUMP BODY FUEL STAIN WAS ALSO NOTED ON THE PUMP BODY HOUSING PARTING FLANGE. PUMP WAS REMOVED FROM SERVICE. NOTE: THIS IS A (M) STYLE PUMP THAT IS RE-DESIGNED TO RELIEVE LEAKS ON THE PARTING FLANGE.
DURING INSPECTION IT WAS FOUND THAT FUEL WAS LEAKING FROM THE SET SCREW FOR THE DRIVE COUPLING LOCK NUT REFERENCE THE RG9080 SERIES COMPONENT MM FOR PARTS BREAKDOWN. PUMP WAS REMOVED FROM SERVICE.
DURING PRE-INSPECTION RUN UP, IT WAS OBSERVED THAT THE FUEL PRESSURE WAS 34 PSI AT 1500 RPM. THE PUMP WAS ADJUSTED UP .5000 TURN AND WAS STILL 34 PSI (LOW RED LINE) FROM IDLE TO 1900 RPM. PUMP WAS REMOVED FROM SERVICE.
PILOT REPORTED FRONT COMBUSTION HEATER INOPERATIVE. IT WAS DISCOVERED THAT THE HEATER CONBUSTION CAN HAD A HOLE IN THE REAR OF THE CAN, AFT OF THE EXHAUST. UNIT WAS REMOVED FROM SERVICE. IT IS SUSPECT THAT IMPROPER COOL DOWN PROCEDURES LED TO PREMATURE CAN FAILURE.
MAGNETO WAS INSTALLED ON LEFT ENGINE 11-24-97, AND WAS REMOVED 12-31-97 DUE TO CRACKED, MISSING TOOTH. PREVIOUS MAGNETO HAD SUFFERED FAILURES OF BOTH DIST GEARS. SUSPECT TORSIONAL VIBRATION IN ENGINE.
AFTER DEPARTURE AND CLIMBING THROUGH 1,200 FEET AGL, PILOT REPORTED THE LEFT ENGINE POPPED TWICE AND SHUT DOWN. UPON INSPECTION, FOUND THE MAGNETO DISTRIBUTOR DRIVE GEARS WERE STRIPPED. ALSO NOTED WAS THE BUSHING IN THE DISTRIBUTOR WAS WORN OUT.
11-24-97, MAGNETO WAS REMOVED FROM LEFT ENGINE OF N NR 59829 DUE TO FAILED DISTRIBUTOR GEAR TEETH. MAGNETO HAD 1,601.16 HOURS SINCE OVERHAUL BY ELECTROSYSTEMS, INC., HOWEVER, GEARS MAY NOT HAVE BEEN NEW AT MAGNETO OVERHAUL.
TOP END OF FUEL CONTROL ARM BROKE OFF WHERE HANDLE ATTACHES WITH ROLL PIN. HANDLE BROKE OFF DURING TANK SELECTION IN THE OFF POSITION. RIGHT ENGINE QUIT AND WAS FEATHERED. TOP OF CONTROL ARM SHAFT HAD BEEN REPAIRED AND RE-DRILLED FOR ROLL PIN. FUEL CONTROL ARM SHOULD BE REPLACED ANY TIME THERE IS PLAY BETWEEN OPERATING HANDLE AND SHAFT. STEEL ROLL PIN WHICH ATTACHES HANDLE TO SHAFT WILL ELONGATE HOLE IN SHAFT.
TURBO MOUNT BOLT LOWER INBOARD AND OUTBOARD AND GETS LOOSE USUALLY AFTER 100 HOURS FLYING TIME. SUBMITTER RECOMMENDS THE BOLT BE CHANGED FROM COURSE THREAD TO FINE THREAD WITH LOCKNUT.
UPON TAKING THE CAP OFF THE MAGNETO, NOTED THAT ONE OF THE DRIVE GEAR TEETH WAS BROKEN OFF FROM THE NYLON DISTRIBUTOR GEAR. NOTE: THE DIRECTOR OF MAINTENANCE REPORTED A TIMING TOOL WAS NOT USED.
MAGNETO CHECK PRIOR TO TAKEOFF. ENGINE WOULD NOT RUN ON RT MAGNETO. INSPECTION OF MAGNETO FOUND 4 TEETH MISSING, RT SIDE DISTRIBUTOR GEAR ASSY. ALL TEETH ON RT AND LT GEAR ASSYS BADLY WORN AND CHIPPED. SUSPECT CAUSE, AGE, HEAT, AND LACK OF LUB ON DRIVE GEAR. CLOSER INSPECTION OF MAG GEARS ON INSPECTION.
SECOND BEFORE ROTATION, A LOUD BANG AND LOSS OF POWER ON NR 1 ENGINE OCCURRED. PROPELLER WENT INTO FEATHER. INSPECTION OF NR 1 ENGINE, NO DEFECTS FOUND, BUT ENG WOULD NOT TURN MORE THAN 600 RPM. PROPELLER WOULD NOT COME OUT OF FEATHER. REPLACED PROPELLER. NR 1 ENG CHECKED OUT NORMAL. TEAR DOWN OF PROP REVEALED THE PTICH CHANGE ROD WAS SEVERED JUST AFT OF PISTON, PN B-3237, RETAINING FLANGE CROSSING PROP TO ABRUPTLY GO INTO FEATHER. PITCH CHANGE ROD BEING FORWARDED TO HARTZELL FOR FURTHER INVESTIGATION. PROP TSO, 274.00 HRS.
