Maintenance disclosures
THE FLIGHT CREW REPORTED THAT THE LEFT THRUST REVERSER LOCKED OUT OF REVERSING. THE MAINTENANCE VENDOR TROUBLESHOT AND DETERMINED THE T/R LOCKOUT SOLENOID WAS FAULTY. THE DEFECTIVE SOLENOID WAS REMOVED AND A NEW UNIT WAS INSTALLED. OPERATIONAL CHECKS SATISFACTORY. AIRCRAFT WAS RETURN TO SERVICE.
THE FLIGHT CREW REPORTED THAT DURING PREFLIGHT CHECKS THE NO1 PRIMARY FLIGHT DISPLAY (PDF) CIRCUIT BREAKER TRIPPED. THE FDR FAIL ANNUNCIATOR ILLUMINATED, ELECTRICAL SMOKE AND SMELL IN THE DEVELOPED IN THE COCKPIT. THIS CAUSED THE SCHEDULED FLIGHT TO BE CANCELLED. THE MAINTENANCE TEAM DETERMINED THE NO1 PDF WAS DEFECTIVE AND WAS REMOVED. A TESTED/INSPECTED PDF WAS INSTALLED. OPS CHECKS WERE GOOD AND THE AIRCRAFT WAS RETURNED TO SERVICE.
RT MAIN TIRE SIDEWALL AND TREAD DEPARTED AIRCRAFT ON LANDING ROLLOUT. DETERMINED BROKEN WHEEL THROUGH BOLT DISLODGED INSIDE WHEEL HALF CAUSED BREAKAGE OF FUSIBLE PLUG AND EVENTUAL DEFLATION OF TIRE DURING FLIGHT. AIRCRAFT LANDED ON DEFLATED TIRE. (CE09200404725)
(LG NR 000173) AIRCRAFT DEPARTED, CLIMBING THROUGH FL370 AT M.58, THE YAW DAMPER BEGAN A SERIES OF LT TO RT INPUTS INCREASING TO FULL SCALE DEFLECTION AT A RATE OF APPROXIMATELY ONE PER SECOND. THE AUTOPILOT WAS TURNED OFF, AND THE MALFUNCTION CONTINUED. TURNED YAW DAMP OFF, AND THE MALFUNCTION CEASED. CREW NOTIFIED ATC AND RETURNED TO AIRPORT. PHYSICAL LOCATION OF FAILED PART IS IN THE AFT TAIL CONE COMPARTMENT. (CE09200404353)
NOSE GEAR EXTENDED DURING DESCENT ON ITS OWN. EXTENDED GEAR, VERIFIED GEAR DOWN AND LOCKED, CONTINUED DESCENT TO ORIGINAL DESTINATION, LANDED WITHOUT EVENT. FOUND NLG UPLOCK MECHANISM OUT OF RIG. RE-RIG, LUBRICATE AND OP TEST SATISFACTORY.
DURING THE T/O ROLL AT APPROXIMATELY 30 KTS, A FUELING PAD (PAD USED BY THE FUELER TO PROTECT THE PAINT DURING FUELING) WAS INGESTED INTO THE NR 2 ENGINE. THE T/O WAS ABORTED AND ENGINE SHUT DOWN AFTER RETURNING TO FBO.LA - TEST
