CORRECTIVE ACTION; SWAPPED EECS & REPLACED SUSPECT FAILED LEFT ENGINE INTERCOMPRESSOR SERVO VALVE. WORK DONE IAW PW EMM 75-30-01. OPS/LEAK CHECK GOOD
THE MAIN DOOR PRECATCH DOES NOT OPERATE WHEN TRYING TO RELEASE THE PRECATCH FROM THE OUTSIDE. VERIFIED PRE CATCH PUSH BUTTON DOES NOT RETURN ONCE PRESSED MCC RECOMMENDED REFERENCING THE PRECATCH ADJUST TASK AND VERIFYING FUNCTION AND MOVEMENT. EVERYTHING CHECKS GOOD. CALLED CITATION TEAM 560, THEY RECOMMEND CLEANING THE LUB OFF AND RE LUBRICATING THE SLIDE AND BUTTON. GAINED ACCESS TO THE SLIDE, CLEANED AND RELUBED ALL MOVING PARTS. PRECATCH STILL HOLDS IN THE DEPRESSED POSITION. CALLED TEAM 560 AGAIN, THEY RECOMMEND CHANGING THE O-RING. O-RING ON ORDER. 12-10 REMOVED O-RING PN MS28775-024 NSN
CORROSION ON RIGHT STRAKE
LEFT ENGINE ENGINE COMPRESSOR STALL: AFTER BEING IN CRUZ FOR AN HOUR AND A HALF DURING INITIAL REDUCTION OF THE THRUST LEVERS, THE AIRCRAFT SHUTTERED/THUMPED FROM THE REAR OF THE AIRPLANE LIKE THE BOV STUCK. HEARD NOISE AND FELT A LITTLE KICK IN THE RUTTER PEDALS THINK IT WOULD BE THE LEFT ENGINE. ALL OTHER ENGINE INDICATIONS WERE GOOD, EXCEPT NOTICED A LITTLE FLUTTER IN THE LEFT ENGINE. THIS HAPPENED AT 43000 FEET. REMOVED AND REPLACED TQ MOTOR. P/N OFF-ON 30J4133-01 S/N OFF TRAXY000987,, S/N ON TRAXY000616.
PASSING FL310 TO FL340, HEARD A BANG & SAW CABIN ALTITUDE RISE. STARTED EMER DESCENT AS CABIN RISE AT A RATE OF 3,200FT/MIN. ABLE TO DESCEND 10,000FT . NOTICED NO AIRFLOW INTO CABIN DURING DESCENT & AN INCREASING TEMP. AFTER INSP & OPS CHKS, OPENED FORWARD FLOOR PANEL & FOUND TRANSITION DUCT SEPERATED FROM THE SUPPLY DUCT AT FLEXIBLE COUPUNG, INSPECTED & FOUND NO DAMAGE TO EITHER DUCTS FLANGES OR ANY SURROUNDING AREAS & REINSTALLED FLEXIBLE COUPLING IAW AMM 21â¢21-00, PERFORMED APU PRESSURIZATION & FOUND OUTFLOW VALVES TO NOT BE CLOSING FULLY. PERFORMED APU LEAK CHECK AGAIN & FOUND TEE FITTING LEAKING. TIGHTENED B-NUT ON MANUAL PRESSURIZATION LINE IAW AMM 21-30-00. LEAK & OPS CHECKED IAW AMM 21-30-00.
FORWARD EMERGENCY LIGHTS DO NOT TEST WITH THE MAIN BATTERY TURNED OFF, BUT WORK NORMALLY WITH THE BATTERY TURNED ON. CHARGED THEM FOR 1.5 HOURS WITH NO CHANGE. SUSPECT THE FORWARD BATTERY HAS FAILED
IN FLT, DESCENDING THRU 32500, HAD A PITCH TRIM RUNAWAY. PERFORMED MEMORY ITEMS FOR RUNAWAY PITCH TRIM, FOLLOWED RUNAWAY PITCH TRIM QRC ITEMS, PULLED LP C16R5 CIRCUIT BREAKER. COMPLIED WITH ELEVATOR TRIM ACT GREASING TO PURGE OLD GREASE FROM ACTS. MOSTURE CONTAMINANT WAS EVIDENT, NOTED THAT WHEN GREASING ACTUATORS IT TOOK A LOT OF GREASE TO BE PUMPED INTO FITTINGS BEFORE OLD GREASE CAME OTU SUSPECT NOT ENOUGH GREASE WAS INSTALLED INTO ACTUATORS. ACTUATOR TRIM THROUGH FULL OF MOTION 10 TIMES ELECTRICALLY & MANUALLY WITH TRIM WHEEL, NO DEFECTS. TRAVEL FELT SMOOTH & SATIS. NO FURTHER DEFECTS NOTED AT THIS TIME WORK IAW MM 12-20-27.
MAIN DOOR PRECATCH LEVER: PRECATCH WAS STICKING AND PRECATCH HANDLE BROKE. REMOVED MAIN ENTRY DOOR INTERIOR HANDLE STOP. FUEL INTERIOR TRIM DAMAGE INTERIOR PRECATCH HANDLE, SHAFT AND NUTS. INSTALLED NEW HANDLE ASSY, NEW SHAFT AND NEW UNITS. RE-INSTALLED MED DOOR FUEL INTERIOR TRIM AND HANDLE STOP.
EMERGENCY LIGHTING: ON POSTFLIGHT, OBSERVED CB HT043 POPPED. CONFIRMED FORWARD EMER LIGHTING INOP. "CORRECTIVE ACTION" REMOVED AND REPLACED FWD EMERGENCY BATTERY PACK WITH OVERHAULED UNIT REMOVED PART# 9913-1 S/N 417 AND OPS CHECKED IAW CE560XLS OPS CHECK GOOD REF FAA FORM 813-3 TRACKING 8FUFA6061001 DATED 06/JUN/2022.
REPAIRED CRACKS ON MAIN ENTRY DOOR AFT FRAME IN ACCORDANCE WITH AEROSPACE QUALITY RESEARCH AND DEVELOPMENT MRO ERO-971710 REV IR DATED APRIL 18, 2022 APPROVED BY FAA FORM 8110-3 (DERT-833246-CE) DATED APRIL 18, 2022.
EMERG EXIT SIGN/LIGHT: WITH THE BATTERY SWITCH ON AND EMER LIGHT SWITCH IN THE ARMED POSITION, THE REAR HALF OF THIS PLANE'S EMER LIGHTS REMAIN ILLUMINATED (THEY ARE SUPPOSED TO BE OFF). THE FRONT HALF OF THE PLANE'S EMER LIGHTS ARE OFF.///SIGN-OFF///RESET INERTIA SWITCH. CORRECTED DISCREPENCY. WORK PERFORMED PER 560XL MM 33-50-00. OPS CHECK GOOD.
CLIMBING THROUGH 7000 FT CREWS EYES STARTED BURNING FOLLOWED BY DEFINITE OIL SMOKE OR MIST. CONTAMINATION CAUSED BURNING IN THROAT, CREW WENT ON O2 AND RETURNED TO LAND. APU WAS SHUT DOWN. OIL CONTAMINATION IS COMING FROM ENGINES AT HIGHER POWER SETTING. R&R RIGHT ENGINE
COCKPIT ODOR OIL SMELL. ONE HOUR INTO THE FLIGHT AT 36,000 FT, DISTINCT OIL BURNING SMELL. COPILOT EXPERIENCED PHYSICAL DISCOMFORT. ODOR WAS STRONG IN COCKPIT, NOT SO STRONG IN CABIN. WENT ON OXYGEN REMAINDER OF FLIGHT. CLEANED BOTH OZONE CONVERTORS AND WATER SEPARATOR SOCKS.
TAIL BAGGAGE DOOR WAS UPGRADED TO SB560XL-52-13, TO PREVENT THE CRACKING OF ATTACH CHANNEL PN 6612069-11S. THIS NEW CRACK WAS FOUND IN HINGE BRACKET PN 6612072-2S, WHICH WAS ADDED IAW SB560XL-52-13 TO CORRECT THE ATTACH CHANNEL FROM CRACKING. SCR 746341 SUBMITTED TO MFG ACFT COMPANY.
DURING A SCHEDULED INSPECTION WE FOUND THE AVIONICS SHELF BRACKET, PN 9256023-16, CRACKED MORE THAN HALF ITS LENGTH. REPLACED BRACKET & SENT SCR 745616 TO MFG.
WHILE OPENING THE CATERING DRAWER THE FWD LED LIGHTING STRIP GOT CAUGHT IN THE DRAWER & PULLED THE LIGHT STRIP PARTIALLY OUT OF THE PROTECTIVE SLEEVE. IT THEN BEGAN SPARKING & SMOKING BEFORE COMPLETELY SHORTING OUT. THE CABIN MASTER SWITCH IN THE COCKPIT IS OFF TO PREVENT POWER FROM GETTING TO THE LIGHT STRIP. INSTALLED NEW LED LIGHT STRIP IAW INTERIOR WIRE DIAGRAM 25-97-02. OPS CHECKED GOOD.
FIVE MINUTES AFTER TAKEOFF, CLIMBING THROUGH 11000 AT 220KIAS, AUTOPILOT ENGAGED, PITCH TRIM ANNUNCIATOR APPEARED ON THE PF. DISCONNECTING THE AUTOPILOT REVEALED ABOUT 15 LBS OF NOSE UP ELEVATOR CONTROL PRESS REQUIRED TO MAINTAIN LEVEL FLIGHT. NEITHER ELEC TRIM SWITCH WOULD MOVE TRIM. MANUAL TRIM ATTEMPT ALSO INEFFECTIVE IN CHANGING TRIM. THE ELEVATOR TRIM WAS JAMMED IN BOTH DIRECTIONS. COMPLIED WITH EMERGENCY JAMMED TRIM CHECKLIST & LANDED WITH TRIM 1/2 INCH FWD OF THE TAKEOFF RANGE. ON TAXI BACK I FELT THE TRIM WHEEL SUDDENLY MOVE A QUARTER TURN & IT APPEARS TO FUNCTION NORMALLY. FLIGHT CONDITIONS WERE WELL OUTSIDE ICING CONDITION PARAMETERS. PERFORMED A VISUAL AND TENSION CHECK OF THE ELEVATOR TRIM CABLES. NO DEFECTS NOTED. RING-OUTS OF THE CAPAINS AND CO-PILOT'S ELEVATOR TRIM SWITHES CHECK GOOD. REMOVED AND REPLACED THE PC BOARD IAW MM 24-51-00. REMOVED AND REPLACED THE ELEVATOR TRIM ACTUATOR MOUNT IAW MM 27-32-04. PERFORMED TRIM WHEEL ADJUSTMENT. ALL FUNCTION AND OPS TESTS GOOD IAW MM 27-31-00.
WHILE ON BATTERY POWER AND INTERIOR LIGHTS, DETECTED AN ACRID SMOKE ODOR, CENTERED NEAR COCKPIT INTERIOR LIGHTS FLICKERED BRIEFLY. ODOR EVENTUALLY DISSAPATED, 10 MINUTES.
WITH APU ON AND BLEED AIR FOR APROX 10 MINUTES, COCKPIT FILLED WITH SMOKE, SMOKE STARTED IN THE COCKPIT AND MOVED BACK INTO THE CABIN. ALL THIS HAPPENED ON THE GROUND WITH MAIN CABIN DOOR OPEN. R & R THE APU IAW MM 49-10-00. OPS AND LEAK CHECKS GOOD.
DURING TAKEOFF, WITHIN 2 SECONDS OF SELECTING THE GEAR LEVER UP, HEARD A LOUD "THUMP". ALL THE GEAR GREEN LIGHTS EXTINGUISHED AND THE RED UNLOCK ANNUNCIATOR REMAINED ILLUMINATED. RAN EGINES INDEPENDENTLY APPLYING HYDRAULIC LOADS. DID NOT OBSERVE A "LOW HYD FLOW" FROM EITHER ENGINE DRIVEN PUMP. PERFORMED GEAR SWINGS AND WAS ABLE TO DUPLICATE PROBLEM. FOUND RT UPLOCK SWITCH AT FAULT . R & R RT MLG UPLOCK SWITCH IAW MM 32-61-00. OPS CHECKED GOOD.
ATTITUDE AND HEADING ON PFD NR 1 AND HEADING ON MFD FAILED ENROUTE. R & R NR 1 AHRU IAW AMM 34-21-01. OPS CHECKED GOOD.
AFTER TAKEOFF, WHILE CYCLING LANDING GEAR UP, A LOUD BANG AND THUD HEARD. FLAPS & ALL ENGINES OPS NORMAL. PERFORMED LANDING GEAR HYD FUNCTIONAL TEST. NO DEFECTS NOTED. OPS CHECKED NORMAL. WORK PERFORMED IAW MM 32-01-00.
DURING CRUISE FLIGHT WE OBSERVED THAT THE TRIM INDICATOR WAS CLOSE TO THE FULL FWD LIMITS ON THE WHITE TAPE. ON APPROACH WE SLOWED TO 190KTS AND WITH THE A/P ENGAGED SELECTED FLAPS 7 DEG. TRIM INDICATOR MOVED FWD BEYOND THE WHITE TAPE ALL THE WAY TO IT'S FWD LIMIT OF THE PANEL. EVEN AT THE TRIM LIMIT IT CONTINUE TO TRY TO TRIM ITSELF FWD WITH THE A/P ENGAGED. WE DISCONNECTED THE AUTOPILOT AND HAND FLEW THE REST OF THE APPROACH WITHOUT ANY PROBLEMS. FOUND POINTER FOR ELEVATOR TRIM NOT POSITIONED CORRECTLY FOR TAKEOFF. RESET POINTER TO CORRECT POSITION FOR TAKEOFF. ELEVATOR AND ELEVATOR TRIM TRAVEL CHECKS AND OPS CHECKS GOOD. ALL WORK DONE IAW MM 27-32-01. ALSO COMPLIED WITH DC-5XL-27-080101 WITH NO DEFECTS NOTED.
