WAS ABLE TO ONLY PUSH LOWER LATCH PIN OVER CENTER PHYSICALLY.INSPECTED AND FOUND PIN OUT OF RIG, ADJUST PUSHROD (9) AND AFTER ADJUSTMENT WAS NOT ABLE TO PHYSICALLY PUSH PIN OVER CENTER. PERFORMED SEVERAL OPS CHECKS WITH ALL OPS CHECKS BEING SATISFACTORY. WORK PER MM CH 52-10-04.

MAIN ENTRY DOOR , WHEN OPENING FROM THE OUTSIDE AND INSIDE , SEEMS TO STICK IN THE CLOSED POSTION. HAVE TO PUSH AND PULL, TO OPEN THE DOOR COMPLETELY. NEEDS EVALUATION. MCC REQUESTED WE PERFORM THE MED HEALTH CHECK IAW AMM 52-10-04. OPERATED MED MULTIPLE TIMES AND WE WERE ABLE TO INTERMITTENTLY DUPLICATE THE DISCREPANCY. SUBMITTED OUR FINDINGS TO MCC. MCC REQUESTED WE PERFORM THE RIGGING OF THE LATCH PINS. REMOVED MED STAIRS AND CLOSE OUT PANELS. PERFORMED RIGGING OF LATCH PINS IAW AMM CH. 52-10-04. INSTALLED MED CLOSEOUT PANELS AND MED STAIRS. OPERATIONAL CHECKS GOOD AT THIS TIME.
MAIN CABIN DOOR DOES NOT ALWAYS OPEN FROM THE INSIDE. YOU HAVE TO HOLD THE RED UNLOCK BUTTON AND PUSH THE HANDLE DOWN TOWARDS THE CLOSE AND LOCK POSITION AND WIGGLE THE HANDLE UNTIL THE YELLOW DOOR OPEN LIGHT LIGHTS UP, THEN THE HANDLE WILL TRAVEL UP TO OPEN THE DOOR CORRECTIVE ACTION: T/S TO FAULTY DOOR HANDLE PUSH BUTTON SWITCH. PLACED NEEDED PARTS ON ORDER WITH MCC 4/24. JH/DP REMOVED & REPLACED DOOR HANDLE PUSH BUTTON SWITCH IAW CE-680A MM CH'S 52-10-16, 52-10-50 & CITATION SERVICE BULLETIN SB68A-52-10. OPS CHECKS GOOD.
LAV EMERG LIGHT: KICKPLATE EMERGENCY LIGHT BELOW LAV IS INOP ON EMER BATTERY AND WITH SHIP POWER. MOVED EMER LIGHTS SWITCH TO ON, CONFIRMED DISCREPANCY. PUT SWITCH TO OFF POSITION. RMEOVED LOWER LAV KICKPLATE ACCESS PANEL, FOUND KICKPLATE EMER LIGHT CONNECTOR NOT FULLY SEATED. RESEATED CONNECTOR AS REQUIRED. PERFORMED OPERATIONAL CHECK OF THE EMERGENCY LIGHT SYSTEM, OPS CHECK GOOD. NO FURTHER DEFECTS NOTED. WORK DONE IAW CE-680A AMM CH. 33-50-00. ALL ACCESS PANEL INSTALLED BACK INTO ORIGINAL CONFIGURATION.

ATTEMPTED ONE RECHARGE PROCEDURE AS DIRECTED. LIGHTS STILL FAILED TO ILLUMINATE. WHEN EMER LIGHTS SELECTED ON, NO LIGHTS ILLUMINATED. REPLACED FUSE AND BATTERY. OPS CHECK GOOD. IAW AMM 33-50-06.
EMERGENCY LIGHTS OVER THE LAV AND BY YOUR FEET ON THE LAV AND THE EXIT SIGN BEHIND SEAT 7 FAILED PREFLIGHT TEST (ARMED AND BATTERIES OFF). PERFORMED QRH PROCEDURE (CHARGED TWICE) AND THEY STILL FAILED. IN THE "ON" POSITION, NO EMERGENCY LIGHTS ILLUMINATED. CORRECTIVE ACTION: REPLACED FUSE. OPS CHECK GOOD. IAW CE-680A AMM 33-50-06 AND TC-FLT-33-1001.

APPROXIMATELY 10 TO 15 MINUTES AFTER DEPARTURE WE GOT A HYD PRESS LOW- R ANNUNCIATOR. FOLLOWED SHORTLY AFTER BY THE HYD VOLUME LOW ANNUNCIATOR THIS WAS FOLLOWED SHORTLY BY THE HYD PRESS LOW R-L ANNUNCIATOR. WE COMPLIED WITH BOTH CHECKLISTS. WE BLEW THE GEAR DOWN AND UTILIZED THE EMERGENCY BRAKE SYSTEM. FOUND HYDRAULIC RESERVOIR TO BE AT 10 CU IN AND HYDRAULIC FLUID UNDER RIGHT ENGINE. OPENED RIGHT LOWER ENGINE COWL AND PERFORMED GVI OF HYDRAULIC LINES AND FITTINGS, FINDING PRESSURE LINE ON HYDRAULIC PUMP TO BE SHEARED OFF AT THE AT THE B-NUT. REMOVED AND REPLACED THE HYD PRESSURE LINE.
MAIN DOOR: MAIN DOOR ACTUATOR ARM PIN HAS FALLEN OUT OF THE SCISSOR ARM AND DOOR IS UNABLE TO CLOSE. // SIGNOFF // PRESSED OUT UPPER BUSHING AND INSTALLED NEW BUSHING PN M81934/L-04C005. CLEANED THREADS AND LOCK RING GROVE, INSTALLED PIN, NEW WASHER PN S 1438-20 AND NEW LOCK RING PIN RETAINER. OPERATED DOOR SEVERAL TIMES. OP CHECKS GOOD. REF680A MM CH 52-15-15.
DURING POST FLIGHT, ALL EXTERNAL LIGHTS WERE SHUT DOWN WHILE WALKING AROUND THE ACFT DOING THE LIGHT CHECK. WHEN WALKING INTO THE AIRPLANE, THERE WAS A NOTICEABLE ELEC BURNING SMELL IN THE CABIN WITH THE BURNING SMELL GETTING WORSE TOWARDS THE COCKPIT. AN EMER SHUTDOWN WAS COMPLETED, SMELL DISSIPATED. NO SMOKE WAS EVER SEEN. POWERED ACFT AVIONICS, INTERIOR LIGHTING AND TURNED HEAT ON. MONITORED SYSTEMS AND INVESTIGATED FOR ODORS FOR 1.5 HRS, THEN NOTED AN ELECTRICAL ODOR IN THE AREA OF THE FWD CLOSET. UPON INVESTIGATION FOUND AIR SHOW UNIT HOT TO TOUCH AND PRODUCING A HOT ELECTRICAL SMELL. FOUND COOLING DUCT GRATE FOR AIR SHOW TO BE BLOCKED/COVERED BY A TRASH BAG AND PLASTIC BINDER IN CLOSET. REMOVED OBSTRUCTIONS FROM COOLING GRATE AND MONITORED SYSTEM FOR ANOTHER 1.5 HRS. SMELL DISSIPATED AND WAS NO LONGER PRESENT AFTER CLEARING COOLING GRATE. SYSTEM CHECKS GOOD AT THIS TIME.
DURING TAXIING IN, USING LEFT BRAKE PRESSURE TO TURN LEFT, PEDAL PRESSURE WAS FELT, BUT NO BRAKE ACTION OCCURRED. WITHOUT WARNING, FULL BRAKE ACTION OCCURRED & ACFT LEFT BRAKE WAS FULLY EFFECTIVE WITH SEVERE JERKING ACTION. LEFT BRAKE PEDAL PRESS WAS CONSTANTLY HELD THROUGHOUT THE EVENT. BRAKES WERE NOT PUMPED. REMOVED AND REPLACED THE LEFT ANTI-SKID TRANSDUCER IAW MM 32-42-07, OPS CHECKED GOOD, NO DEFECTS NOTED.
THE LT BRAKE FAILED ON TAXI OUT. BRAKE PEDAL WAS FIRM BUT BRAKE WAS STILL INOPERATIVE. BRAKE RESERVOIR IS FULL & ALL BRAKE ACCUMULATORS ARE IN LIMITS. ALSO THERE WERE NO BRAKE SYS BITE INDICATORS SHOWING FLAGGED. INSPECTED LT BRAKE ASSEMBLY & BRAKE WEAR PINS, NO DEFECTS NOTED. GAINED ACCESS TO BOTH CREWMEMBERS MASTER CYLINDERS & INSPECTED FOR LEAKS OR DAMAGE, NONE FOUND. PRIMED & BLED LT MASTER CYLINDER & ALSO BLED LT BRAKE SYS. USED SHOP BRAKE BLEEDING FIXTURE, SERVICED SYS 1 QT. OF SKYDROL 500B SINCE RESERVOIR SHOWED LOWER AMOUNT ON BRAKE RESERVOIR. INSPECTED BLEED VLV, HYD PACK ASSEMBLY SUCTION LINE & LT NOSE COMPARTMENT FOR LEAKS NO DEFECTS NOTED. ALL WORK DONE IAW AMM 32-42-00. OPERATIONS CHECKED GOOD. TAXI CHECKED GOOD.
DURING TROUBLESHOOTING OF A STEERING KNOCKING SOUND, FOUND THE BOLT THAT ATTACHES THE CABLE TO THE STEERING BELLCRANK WAS ROCKING IN THE BELLCRANK. AFTER REMOVAL OF THE CABLES, IT WAS NOTED THAT THE UPPER HOLE IN THE BELLCRANK WAS ELONGATED & THE BACK SIDE OF THE BELLCRANK WHERE THE WASHER UNDER THE BLOT HEAD SITS WAS WORN INTO THE MATERIAL OF THE BELLCRANK. THE BELLCRANK HAS NO BUSHING OR BEARING INSTALLED FOR THE BOLT TO WEAR INTO. IT IS JUST A PLAIN ALUMINUM BELLCRANK WITH A STEEL BOLT INSTALLED THROUGH IT. THE CABLE END IS A LOLLIPOP STYLE & NOT A FORK STYLE, THUS WHEN THE CABLE IS TENSIONED, THE BOLT IS ALWAYS SIDE-LOADED & WEARS INTO THE BELLCRANK. BELLCRANK ASSEMBLY REPLACED WITH NEW HARDWARE.
NORMAL BRAKING DURING TAXI OUT AND LANDING ROLL INCLUDING LT TURNS. WHEN MAKING LEFT TURNS AT TAXI SPEED, PILOT EXPERIENCED HARD PEDALS AND HAD NO BRAKES FOR 5-7 SECONDS. THEN THE BRAKES WOULD VIOLENTLY GRAB. PERFORMED BRAKE BLEEDING AND ANTI-SKID SYSTEM FUNCTIONAL CHECK IAW MM 32-42-00. PERFORMED TAXI CHECK WITH NO DEFECTS NOTED IAW MM 9-20-00.
PERFORMED BORESCOPE INSPECTION OF COMBUSTION CHAMBER LINER COOLING RING SEGMENT SELF LOCKING NUTS. FOUND TWO NUTS MISSING AT THIS TIME AT POSITION NR 1 & NR 18. COMPLIED WITH A HOT SECTION REPAIR SI 36-2013.
CREW ENCOUNTERED STRONG COCKPIT ODOR ALL PHASES OF FLIGHT, SMELLS LIKE BURNING RUBBER AND BURNED TOAST IN THE COCKPIT. COULD NOT IDENTIFY THE PROBLEM. REMOVED & CLEANED OZONE CONVERTER. OPS AND LEAK CHECKS GOOD. BORESCOPED INSIDE DUCT, FOUND B1/2 SEALANT ON JOINTS C3 DIFFERENT PLATES AND SOME SEALANT PEELING OFF FROM INSIDE DUCT. REMOVED DUCT AND AND CLEANED AND REINSTALLED. RAN AICRAFT AND OPERATED BOTH COCKPIT AND CABIN TEMPS IN AUTO AND MANUAL HOT WITH APU ON AND OFF. NO BURNING ODOR NOTED. RAN BOTH ENGINES AT TAKEOFF AND CRUISE POWER SETTINGS. BLEED AIR SOURCES WERE ISOLATED TO THE ENGINE AT HIGH POWER, NO ODORS NOTED. WORK DONE IAW MM 21-22-00 AND MM 21-71-00.
CLIMBING THROUGH FL 210, CREW ENCOUNTERED AN STRONG ODOR. ODOR SEEMED TO CYCLE -A 2-3 MINUTE CYCLE -IN INTENSITY. SEEMED TO BE ISOLATED TO COCKPIT. ODOR LESSENED DESCENDING THRU FL180, BUT DID REMAIN NOTICEABLE THROUGHTOUT THE LANDING AND SHUTDOWN. R & R COCKPIT RECIRCULATING FAN ASSY. OP'S CK. GOOD INSTALLED ACCESS PANELS IAW MM 21-22-00.
AT FL390 IN CRUISE. RUBBER ODOR LIKE A BURNING TENNIS SHOE. AFTER 5 MINUTES EYES BEGAN TO BURN. NO SMOKE BUT THE BURNING ODOR WAS MODERATE TO STRONG. ADJUSTED COCKPIT TEMPERATURE CONTROLLER WITH NO NOTICEABLE CHANGE IN ODOR. THE ODOR DIDN'T APPEAR TO COME FROM THE ENVIROMENTAL CONTROL SYS. ODOR SEEMED BE STRONGER AROUND THE CIRCUIT BREAKER PANEL AREA ON THE LT SIDE OF THE FLIGHT DECK. BORESCOPE INSPECTION FOUND EVIDENCE OF BIRD INGESTION IN LT ENGINE GAS CHAMBER. R & R LT ENGINE WITH A OVERHAULED ASSY AND PERFORMED ALL OPS AND LEAK CHECKED GOOD LAW MM 71-00-04.
BRAKES CHECKED NORMAL ON TAXI OUT. WHILE TAXIING & STOPPING AT HOLD SHORT LINE, LT BRAKE FAILED. FURTHER TAXIING REVEALED THAT WITH LIGHT PRESSURE APPLIED TO BRAKES, FIRST APPEAR NORMAL, THEN LEFT SIDE SEEMS TO RELEASE PRESSURE AS IF ANTI-SKID KICKS IN RELEASING PRESSURE ON LEFT SIDE. RESULTS & INDICATIONS ON BOTH PILOT AND COPILOT SIDE. PERFORMED VISUAL INSP OF BRAKE SYS, RESERVOIR, ACCUMULATOR, BRAKE PACKS AND PLUMBING , NO DEFECTS NOTED. PERFORMED BRAKE BLEEDING PROCEDURES AND SERVICE BRAKE SYS. TAXIED ACFT TO RUN UP AREA HELD BRAKES AT IDLE AND WITH 75 PERCENT POWER, BOTH BRAKES HELD. SET PARKING BRAKE AND TRIED AGAIN AT IDLE AND 75 PERCENT, BOTH BRAKES HELD, NO DEFECTS NOTED. ALL WORK IAW MM 32-42-00.
COPILOTS RT RUDDER PEDAL IS CHAFING ON LT HYDRAULIC BRAKE LINE AT FORWARD BULKHEAD.
THE FWD AND AFT HORIZONTAL STAB ATTACH POINTS ARE WORN, ALLOWING HORIZONTAL STAB MOVEMENT BOTH RADIALLY AND UP AND DOWN. THIS MOVEMENT HAS PLAY IN THE HORIZONTAL, SEEMS TO HAVE STARTED TO LOOSEN UP ANOTHER AREA. THE LT SIDE OF THE AFT VERTICAL SPAR HAS AN AREA THAT IS GAPPING BETWEEN THE SPAR AND THAT FRAME STATION. THE GAP IS AROUND 0.002" WHEN PRESSURE IS BEING PLACED ON THE UPPER OTBD SIDE OF THE HORIZONTAL. THE GAP IS CLOSED IN THE STATIC POSITION OR WHEN PRESSURE IS APPLIED ON THE LOWER SIDE OF THE STAB.
DURING A ROUTINE PHASE A/B INSPECTION, TECHS DISCOVERED THAT THE AUTOPILOT RUDDER CABLE BRACKET LOCATED AFT OF THE CANTED BULKHEAD HAS A 1" CRACK AT THE BASE OF THE BRACKET PASSING THROUGH THE TOP MOUNT HOLE.
CLIMBING THROUGH FL280, THE RIGHT ENGINE THRUST REVERSER UNLOCK LIGHT ILLUMINATED. THE EMERGENCY STOW SWITCH WAS SELECTED AND THE UNLOCK LIGHT EXTINGUISHED. A MINUTE LATER THE UNLOCK LIGHT REILLUMINATED AND REMAINED ON THROUGHOUT DESCENT AND LANDING. IT WAS STILL ILLUMINATED AFTER SHUTDOWN. ABMORMAL CHECKLIST PROCEDURED ACCOMPLISHED.
DURING PREFLT, HEARD A GRINDING NOISE WHEN DEPRESSING RUDDER PEDALS. ATTEMPTED 1 DEFLECTION OF RUDDER PEDALS IN EACH DIRECTION TO INVESTIGATE. UPON DEPRESSING RT RUDDER PEDAL, A SUDDEN RELEASE OF PRESSURE FELT ALONG WITH A LOUD "BANG". THERE IS NOW ZERO RESISTANCE FELT DEPRESSING RT RUDDER PEDAL AND NORMAL RESISTANCE WHEN DEPRESSING THE LT PEDAL. IN LT NOSE COMPARTMENT, LOWER STEERING CABLE IS NOTICEABLY SLACK. FOUND LOWER CABLE IN 2 PIECES - WORN COMPLETELY IN HALF & UPPER CABLE IS WORN WITH BROKEN STRANDS. BOTH CABLES ROUTED AROUND PULLEY GUIDE PINS, INSTEAD OF PROPERLY APPROVED PULLEY. R & R BOTH STEERING CABLES & BOTH CABLE SEALS AT BULKHEAD. TAXI CHECK & OPS CHECKED GOOD . WORK DONE IAW MM 32-50-00.
NO BRAKES - POWER BRAKE MOTOR NOT WORKING. LOW BREAK PRESSURE ANNUNCIATOR STAYS ILLUMINATED. TROUBLESHOT & FOUND POWER COMING INTO BRAKE CONTROL MOTOR RELAY KNOB BUT NOT GOING OUT. RUNG OUT RELAY, NO POWER AT X1 OF THE RELAY. JUMPERED A1 & X1 AND RELAY CLOSED AND MOTOR CAME ON. DIODE BETWEEN A1 & X1 NOT WORKING (31711) REMOVED DIODE PN S3106-4. INSTALLED NEW DIODE PN S3106-4. SPLICED IN NEW DIODE WITH NEW WIRES. OPS CK GOOD. ALL IAW MM 20-21-03.
LT AND RT SIDE BRAKE PEDALS ARE NOT BRAKING NORMAL. WITH VERY LITTLE PRESSURE THE BRAKES WANT TO LOCK. INSPECTED BRAKES WITH NO FAULTS FOUND. PERFORMED COMPLETE BRAKE BLEEDING PROCEDURE IAW MM 32-42-00. BRAKES OPS/FEEL CHECK GOOD AT THIS TIME.
ON CLIMBOUT OF PBI LEFT FUEL LOW PRESSURE LIGHT CAME ON INTERMITTENTLY FOLLOW BY A STEADY LT FUEL BOOST PUMP LIGHT ON. THEN A MASTER CAUTION CAME ON FLASHING. FOLLOWED ALL ABNORMAL CHECKLISTS TO THE END. MASTER CAUTION CONTINUE TO FLASH ON WITH A INTERMITTENTLY LT LOW FUEL PRESSURE LIGHT. CALLED THE ACP AND DISPATCH AND DIVERTED. ON DESCENT MASTER CAUTION LIGHT WHEN OUT. T.S.S. FOUND DEFECTIVE SWITCH. INSTALLED NEW SWITCH. OPS CK GOOD. IAW M/M 73-30-02

WHEN GEAR SELECTED DOWN, LT MAIN GEAR DID NOT SHOW GREEN LIGHT. WENT TO GEAR WOULD NOT EXTEND CK LIST AND BLEW GEAR DOWN. UNEVENTFUL LANDING. [07-11-07] TROUBLESHOT TO A FAULTY DOWN & LOCK SWITCH ON LT MLG ACTUATOR. REMOVED LT MLG ACTUATOR, INSTALLED A REPAIRED ACTUATOR (REF. CESSNA CNQR918C, WO 2-59802). PERFORMED MLG WHEEL CAMBER ADJUSTMENT. PERFORMED GEAR BLOWDOWN CHECK, CHECK GOOD.

AFTER TAKEOFF GEAR HANDLE WOULD NOT MOVE, THEREFORE COULD NOT RETRACT THE GEAR. ITEM IS UNDER INVESTIGATION.
PILOTS LEFT BRAKE PEDAL WENT TO FLOOR ON LANDING ROLL. CO-PILOTS BRAKES WORKED FINE. ITEM IS UNDER INVESTIGATION.
LEFT BRAKE PEDAL ON LEFT SIDE GOES ALL THE WAY DOWN WHEN BRAKES APPLIED. TROUBLESHOT LEFT BRAKE TO MASTER CYLINDER. REMOVED AND REPLACED PILOT'S LT WHEEL BRAKE MASTER CYLINDER WITH A SERVICEABLE INSPECTED BRAKE MASTER CYLINDER IAW ESSNA 560XL MM CHAPTER 32-42-02. BLED BRAKE SYSTEM IAW CESSNA 560XL MAINTENANCE MANUAL CHAPTER 32-42-00. LEAK AND OPERATIONAL CHECKS SATISFACTORY.
