ON TAXI PRIOR TO TAKE OFF, AIRCRAFT BRAKES OVERHEATED DUE TO EXCESSIVE USE BY FLIGHT CREW, LEADING TO A FIRE AT THE RIGHT MAIN LANDING GEAR.
DURING APPROACH ERRONEOUS SMOKE INDICATION FROM SMOKE DETECTOR IN NOSE COMPARTMENT OF AIRCRAFT. SMOKE DETECTOR IS A SINGLE DETECTOR SINGLE OPERATING SYSTEM.
WHILE IN CRUISE, LEFT ENGINE LOST POWER WITH INCREASE OF ITT TO 1200 FOLLOWED BY LOUD NOISE. FOLLOWED PROCEDURES AND SHUT DOWN ENGINE. DIVERTED TO MWA. ENGINE REPLACEMENT IN WORK.
THE LANDING WAS NORMAL AND UNDER CONTROL. SLOWING TO ABOUT 60-70 KNOTS THE LT MAIN TIRE BLEW. WHEN THE TIRE BLEW AIRCRAFT WAS MAINTAINED ON CENTERLINE AND STEERING AIRCRAFT OFF THE ACTIVE RUNWAY. UPON FURTHER EVALUATION THE LT TIRE SHOWED SIGNS THAT WAS NOT SPINNING AND BULLSEYED THE TIRE AND THE ZEUS FASTENERS PULLED THROUGH THE FAIRING. THE LT MAIN WHEEL ASSY BLEW OUT DURING LANDING IN IND WITH 1.1 HOURS AND 1 CYCLES SINCE INSTALLED ON 9/06/2006. THE IND MECHANIC REMOVED THE DAMAGED LT MAIN WHEEL ASSY AND REPLACED IT WITH A SERVICEABLE MAIN WHEEL ASSY IAW SD360 MM, CHAPTER 32. THE LEAK AND OPS CHECKS WERE GOOD. THE IND MECHANIC MAEDE A TEMPORARY REPAIR TO THE LARGE FAIRING IN ORDER FOR AC TO BE FLOWN F
HNL FLT 7670 - AFTER LANDING AT HNL, THE AIRCRAFT WAS LEANING TO THE RIGHT SIDE. THE LEFT MLG SHOCK STRUT GLAND NUT FAILED ALLOWING THE SHOCK STRUT TO OVER EXTEND, AND BIND IN THAT POSITION. THE LEFT MLG FAIRING SUSTAINED DAMAGE DUE TO THE OVER EXTENSION. SHOCK STRUT GLAND NUT FRACTURED, SHOCK STRUT DAMAGED, BRAKE LINES TORN, LT MLG FAIRING DAMAGED. A HIDDEN DAMAGE STRUCTURAL INSPECTION/REPAIR IS UNDERWAY, AND REPLACEMENT PARTS HAVE BEEN ORDERED AND ARE INTRANSIT TO THE AIRCRAFT AT THIS TIME. GLAND NUT INSPECTION WAS COMPLIED WITH 2801 CYCLES PREVIOUSLY ON 4-30-96, AT 26494 TAC. (M)
BIL - PILOT REPORTED A LOSS OF BRAKING ACTION AND A LOSS OF HYDRAULC FLUID DURING TAXI. LEAKING PRESSURE HYDRAULIC FLUID LINE TO THE NOSE STEERING ACTUATOR. REMOVED AND REPLACED HOSE ASSY. REPLENISHED HYDRAULIC CONTENTS. LEAK AND FUNCTIONAL CHECKED. AIRCRAFT RETURNED TO SERVICE. (M)
