DURING MAINTENANCE CORROSION WAS DISCOVERED ON THE LEFT HAND UPPER AFT SPAR CAP (P/N: 5122041-21). THIS CORROSION IS LOCATED BETWEEN LWS 229.71 AND 224.71, AND MEASURES .032 INCHES DEEP. THIS MEASUREMENT EXCEEDS THE LIMITS FOUND IN MEB95-11R1. THIS CAP WILL BE REPLACE WITH A NEW PART.
DURING MAINTENANCE CORROSION WAS DISCOVERED ON THE LEFT HAND LOWER AFT SPAR CAP P/N 5122041-23, AT APPROXIMATE L.W.S. 215.20. THIS CORROSION IS MEASURED TO A DEPTH OF .019 INCHES AND IS BEYOND LIMITS FOUND IN MEB95-11R1. THIS CAP WILL BE REPLACED.

PILOT REPORTED FUEL SMELL IN CABIN. INSPECTED LT & RT WING FUEL LINES, CABIN FUEL LINE, HEATER FUEL LINES & ENG LT & RT FUEL LINES, NO LEAKS NOTED.
PILOT REPORTED FUEL ODOR IN CABIN. FOUND THE FUEL SUMP ON LEFT WING LEAKING. R & R O-RING ON SUMP. OPERATIONAL & LEAK CHECK GOOD.
THE BRAKES VERY WEAK, LT BRAKE IS NON-EXISTENT. R & R THE LT MLG CALIPER ASSEMBLY IAW MM CH 32-40-00. RT & LT BRAKES SERVICED IAW MM 12-10-05. OPERATIONS CHECKED GOOD.
VERY POOR BRAKING ALMOST NOTHING. THIS WAS NOTED AFTER DEPARTURE WHICH INDICATES THE PROBLEM WAS FOUND ON LANDING. THE BRAKE SYS WAS THEN SERVICED IAW MM CH 12-10-03 & 32-40-00. OPERATIONS CHECKED TO ADDRESS THE ISSUE.
AT SCHEDULED INSPECTION, NLG DRAG BRACE, PN 5142002-5, WAS FOUND TO BE CRACKED IN THE AREA OF NLG ACTUATOR ATTACH POINT. METHOD OF INSPECTION WAS DYE PENETRANT. THIS CRACK WAS FOUND TO BE WITHIN ALLOWABLE LIMIT OF MEB91-11R1. THIS UNIT HAS BEEN REPAIRED IAW PROCEDURE OUTLINED IN MEB941-11R1 & RETURNED TO SERVICE.
ON TAKEOFF ROLL, CREW EXPERIENCED A YAW WHICH CAUSED THE TAKEOFF TO BE DISCONTINUED AT LOW SPEED. CREW TAXIED BACK TO THE RAMP. MX FOUND RT MAIN GEAR AXLE ASSY BROKEN AT LOWER HALF. MX R & R AXLE ASSY, AND RETURNED THE ACFT BACK TO SERVICE.
CREW REPORTED, ON CLIMB OUT, CABIN BECAME FILLED WITH SULFURIC / ELECTRICAL BURNING ODOR. CREW ELECTED TO REMAIN IN PATTERN AND RETURNED TO BASE, LANDING WITHOUT INCIDENT. PILOT REPORTED THAT ODOR MAY HAVE EMINATED FROM REAR OF ACFT. MX INSPECTED ACFT. COULD NOT DUPLICATE ODOR IN CABIN. INSPECTED AUTO PILOT SYS. CLEANED ALL CONNECTION AT TRIM AND PITCH SERVOS. NO DEFECTS FOUND. AS A PRECAUTIONARY MEASURE, MX INSPECTED CABIN VENT SYS FAN, NO DEFECTS NOTED ALSO. ACFT RELEASED BACK TO SERVICE.
DURING TRAINING FLIGHT, AFTER TOUCH AND GO, DURING GEAR RETRACTION, CREW REPORTED RIGHT MAIN LANDING GEAR DOWNLOCK INDICATION REMAINED ILLUMINATED. PERFORMED "LANDING GEAR WILL NOT EXTEND HYDRAULICALLY" CHECKLIST WHICH RESULTED IN THREE GREEN LIGHTS. ON LANDING ROLL OUT AIRCRAFT VEERED TO THE RIGHT. AIRCRAFT WAS BROUGHT TO A SAFE STOP ON TAXI WAY. MAINTENANCE FOUND RIGHT MAIN LANDING GEAR LOWER TORQUE LINK BROKEN AT OUTBOARD PIVOT POINT. MAINTENANCE REMOVED AND REPLACED RIGHT MAIN GEAR LOWER BARREL ASSEMBLY TO INCLUDED BOTH UPPER AND LOWER TORQUE LINKS. AIRCRAFT RELEASED BACK TO SERVICE.
CREW REPORTED NO NOSE GEAR DOWN LOCK INDICATION ON FINAL APPROACH. LIGHT TESTED MORMAL. CREW DECLARED EMERGENCY AND PERFORMED EMERGENCY GEAR EXTENTION. STILL NO NOSE GEAR INDICATION. LANDING WAS UNEVENTFUL. MX REMOVED AND REPLACED DEFECTIVE DOWN LOCK SWITCH. ACFT RELEASED BACK TO SERVICE.
CREW REPORTED AVIONICS MASTER INOP. MX CHANGED CIRCUIT BREAKER SWITCH.
CREW REPORTED GEAR UNSAFE LIGHT REMAINED ON WITH GEAR HANDLE IN UP POSITION, MAINTENANCE ATTEMPTED TO REMOVE AND REPLACE THE RT UPLOCK MICROSWITCH, 2 SWITCHES FAILED (BAD FROM STOCK) BEFORE INSTALLING A SERVICEABLE UNIT, GEAR SWING CHECKS, OK. AIRCRAFT RELEASED FOR SERVICE. (K)
CREW REPORTED GEAR UNSAFE LIGHT REMAINED ON WITH GEAR HANDLE IN UP POSITION, MAINTENANCE REMOVED AND REPLACED THE RT UPLOCK SWITCH, GEAR SWING CHECKS OK. AIRCRAFT RELEASED FOR SERVICE. (K)
CREW REPORTED RT MAIN GEAR DID NOT FULLY RETRACT WITH GEAR HANDLE IN UP POSITION, MAINTENANCE REMOVED AND REPLACED THE RT UPLOCK MICROSWITCH, GEAR SWING CHECKS OK. AIRCRAFT RELEASED FOR SERVICE. (K)
DURING TAXI, RIGHT MAIN LANDING GEAR STRUT ASSEMBLY BROKE AT ACTUATOR ATTACH COLLAR. UPON INSPECTION, FOUND STRUT HAD LOST NITROGEN CHARGE AND HAD BEEN TAXIED IN THE FULLY DEPRESSED POSITION, POSSIBLY DAMAGING THE STRUT INTERNALLY. THE LANDING GEAR DID NOT RETRACT SUCCESSFULLY AND THEN THE AIRCRAFT LANDED WITH THE GEAR JAMMED IN THE FULLY EXTENDED POSITION CAUSING THE UPPER BARREL TO FRACTURE AND FINALLY FAILED COMPLETELY WHILE TAXING TO THE GATE.
CREW REPORTED FUEL/FUEL PUDDLE ON COCKPIT FLOOR ON THE FIRST OFFICER'S SIDE. DIVERTED TO HYA EN ROUTE FROM BOS TO ACK. MAINTENANCE COULD NOT CONFIRM PUDDLING OF FUEL. FOUND FUEL STAIN (BLUE DYE) ON FUEL FLOW BLEEDER LINE ASSY. TIGHTENED AN 929-2 CAP. C/W FULL POWER GROUND RUN. NO LEAKS NOTED.
