OXYGEN PRESSURE INDICATION: REPEAT WRITE UP. RETURNED TO AC THIS MORNING AFTER AOG OF 02 INDICATING SYSTEM LAST NIGHT. INITIAL INDICATION WAS 1700 PSI. INDICATION IN NOSE COMPARTMENT WAS IN THE GREEN ARC. AFTER AN 1.5 OF FLIGHT INDICATION IN THE COCKPIT EIS WAS 1400 PSI. CONTACTED MX AND FODM AND WE AGREED TO RETURN TO DEPARTURE AIRPORT FOR FURRHER EVALUATION OF 02 SYSTEM. EVALUATE SYSTEM FOR A LEAK. CORRECTIVE ACTION: REMOVED UNSERVICEABLE TEE - OXYGEN (PN: S4376-I) AND LINE ASSEMBLY 02 COVERED (PN:02C11T13-012). INSTALLED NEW TEE- OXYGEN (PN: S4376-1) WITH NEW PACKING (PN: MS9385-04) AND NEW LINE ASSEMBLY 02 COVERED (PN: 02C11T13-012). PERFORMED OXYGEN SYSTEM LEAK CHECK. LEAK. CHECK TEST GOOD. POWERED AIRCRAFT AND PERFQRRNED EMERGENCY LIGHTING OPERATIONAL CHECK IAW CE680A MM CH.33-50-00 WITH NO DEFECTS NOTED. ALLOWED EMERGENCY BATTERIES TO CHARGE FOR 2 HOURS AS REQUIRED. POWERED DOWN AIRCRCIFT AND CLOSED ALL ACCESS AS REQUIRED.

EMERG EXIT SIGN/LIGHT: EMERGENCY EXIT SIGN ABOVE SEAT 7 DOES NOT ILLUMINATE. LAV EXIT SIGN/LIGHT: EMERGENCY EXIT SIGN LIGHT BELOW THE LAV SEAT DOES NOT ILLUMINATE WITH THE EMER LIGHTS SWITCH IN THE ARMED POSITION. TECH HAS PERFORMED SEVERAL OPS CHECKS FOR THE EMERGENCY BATTERY SYSTEM. I PERFORMED AN OPS CHECK WITH THE BATTERIES CONNECTED AND NO POWER ON AND TOGGLE SWITCH SET TO THE ARM AND ALL LIGHTS TURNED ON. I ALSO COMPLETED AN OPS CHECKS WITH ALL POWER ON AND APU ONLINE AND TOGGLE SWITCH SET TO ON AND ALL LIGHTS TURNED ON AS WELL. REF. AMM 33-50-00
GEA FAIL 2 AMBER CAS: DURING CRUISE AT FL400 AMBER CAS MESSAGES ENG CTRL FAULT L AND PRESSURIZATION FAULT ILLUMINATED WITH LOSS OF INDICATION FOR L EBGINE OIL PRESSURE, L OIL TEMP, AILERON TRIM, RUDDER TRIM, L SPEED BRAKE PANELS. R SPEED BRAKE PANELS INDICATED EXTENDED AND CONFIRMED TO BE STOWED VISUALLY. THERE WAS NO CAS MESSAGE FOR GEA2. DESCENDING 7000 TO 6000 ALL IRREGULAR INDICATIONS, MESSAGES, AND DISPLAYS RETURNED TO NORMAL. FUEL IMBALANCE: BEFORE TAKEOFF, FUEL WAS BALANCED AT 4500 A SIDE. DURING TAKEOFF, RIGHT FUEL INDICATED AN INCREASE OF OVER 1000 POUNDS AND THE FUEL IMBALANCE CAS MESSAGE POSTED. RIGHT FUEL SLOWLY RETURNED TO A CORRECT INDICATION OVER THE NEXT 15 MINUTES.
CABIN EMERG LIGHT: THE EXIT LIGHTS AND SEAT 10 TOE KICK LIGHTS WILL NOT EXTINGUISH WITH EMER LIGHTS SWITCH IN OFF POSITION. SUSPECT TRIPPED G SWITCH. // SIGNOFF// VERIFIED DISCREPANCY. RESET FWD INERTIA SWITCH & CHARGED EMERGENCY LIGHTING BATTERIES. EMERGENCY LIGHTING BATTERIES OPS/CAP CHECKED GOOD. CHARGED EMERGENCY LIGHTING BATTERIES. ALL MX COMPLETED IAW CE-680A AMM CHAPTER 33-50-00.
MAIN DOOR: MAIN DOOR HAS BEEN GETTING PROGRESSIVELY MORE DIFFICULT TO OPEN. IT IS STILL FUNCTIONAL BUT NEEDS EVALUATION FOR LUBRICATION AND/OR POSSIBLE ADJUSTMENT. // SIGNOFF// PERFORMED SERVICING AND LUBRICATION OF THE MAIN CABIN ENTRY DOOR IN ACCORDANCE WITH CESSNA 680A AMM 12-20-40. PERFORMED SEVERAL SATISFACTORY OPERATIONAL CHECKS OF THE MAIN CABIN DOOR. NO FURTHER DEFECTS NOTED.

TAXIED-AIRCRAFT AND APPLIED BRAKING UNTIL HOT, LEFT INBOARD WAS NOTABLY HOTTER THAN THE REST AND WAS SMOKING. DRIP ON OUTER PRESSURE PLATE FROM MELTING GREASE. REMOVELD WHEEL ASSY AND INSPECTED BEARINGS. BEARINGS NOT DISCOLORED OR OVERHEATED. EXGESSIVE GREASS FROM.AXLE DRIPPING ONTO BRAKE AND CAUSED SMOKE. INSTALLED OVERHAULED BRAKE ASSY IN #2 POSTION IAW CE-680A AMM 32-42-05. INSTALLED MLG WHEEL ASSY IAW CE-680A AMM 32-40-05. ACCOMPLISHED BLEEDING""AND SATISFACTORY MLG BRAKE CONTROLL SYSTEM AND FUNCTIONAL CHECK REF CE-680A AMM-32-42-00.-LEAK-CHECK-GOOD.-SERVICED HYDRAULICS IAW CE-680A AMM 12-10-30.
CONDITIONED AIR HAS A STRONG MILDEW ODOR ESPECIALLY WHEN DEMANDING HEAT. CONDITIONED AIR THROUGH DUCT WORK DOES NOT HAVE A STRONG MILDEW ODOR. REMOVED WATER SEPARATOR, REPLACED CONDENSER BAG AND CLEANED ALL PARTS REASSEMBLED. INSTALLED AND OPERATED AND LEAK CHECKED. NO LEAKS OR STRONG MILDEW ODOR COMONG OUT OF THE VENTS IAW AMM 21-51-00, AND 12-19-07.
DURING A SCHEDULED INSPECTION WE FOUND THE AVIONICS SHELF BRACKET, PN 9256023-16, CRACKED MORE THAN HALF ITS LENGTH. REPLACED BRACKET & SENT SCR 745615 TO MFG.
DURING SCHEDULED INSPECTION, FOUND THE RT FWD AVIONICS SHELF ATTACHED BRACKET CRACKED. REPLACED WITH NEW.
DURING PREFLIGHT, WITH APU BLEED RUNNING, CABIN FILLED WITH BLUE SMOKE. FIRST OFFICER INDICATED CABIN WAS VERY HOT EVEN THOUGH TEMP SETTING WAS VERY LOW. REMOVED DEFECTIVE APU & REPLACED WITH REPAIRED APU. CLEANED APU CONTAINMENT BOX & FLUSHED DRAIN LINES. CHANGED CONDENSER SOCK & PERFORMED DUCT BURNOUT. NO FURTHER SMOKE OR ODORS IN CABIN OR COCKPIT. SERVICED, OPERATIONAL & LEAK CHECKS WERE FOUND TO BE GOOD AT THIS TIME. ALL WORK COMPLETED IAW MM 49-00-01. CONTAINMENT BOX ACCESS & 49-10-00, FOR APU R & R, BUILDUP, & OPERATIONAL CHECKS.
DURING CLIMB OUT, HAD A CHEMICAL ODOR IN THE COCKPIT. RETURNED TO DEPARTURE. APU WAS OFF AFTER ENGINE START & NOT USED ON THE GROUND FOR BLEED AIR. THERE WAS NO VISIBLE SMOKE. INSPECTED AND FOUND OIL RESIDUE IN THE APU INLET. REMOVED AND REPLACED THE APU IAW MM 49-10-00. OPS AND LEAK CHECKED GOOD. NO ODOR DETECTED AT THIS TIME.
DURING A SCHEDULED INSPECTION FOUND ELEVATOR TRIM CABLE FRAYED AT LEAST HALF WAY THROUGH. THIS CABLE IS A PART OF A LARGER CABLE ASSY, PN 6660003-1. THIS CABLE HAS BEEN A SUBJET OF SL, BUT THESE CABLES HAVE IN THE PAST AND CONTINUE TO FRAY PREMATURELY. A SERVICE CONDITION REPORT HAS BEEN FILED WITH MFG UNDER NR 585949.
INTERMITTENTLY AFTER TAKEOFF AND GEAR RETRACTION, A VERY LOUD BANGING SOUND AND VIBRATION COMES FROM THE FWD RT SIDE OF ACFT. SLOW CYCLED GEAR. FOUND NOSE GEAR DOORS OUT OF ADJUSTED AND BINDING AGAINST EACH OTHER WHEN GEAR GOES UP IN WELL. NO DAMAGED TO DOORS NOTED. VISUALLY INSPECTED ALL NOSE GEAR DOOR SYS COMPONENTS & VERIFIED NOTHING IS BENT OR DAMAGED. REFORMED BOTH AFT CORNERS ON LT & RT DOORS TO ORIGINAL CONTOURS. PERFORMED MULTIPLE GEAR SWINGS, LT & RT GEAR DOOR LOWER PUSH/PULL TUBE ROD END ADJUSTMENTS UNTIL DOORS HAD PROPER FIT & CLEARANCE. GEAR SLOW CYCLES SHOW NOSE DOORS TO HAVE PROPER RELATIVE POSITIONS WITH NO BINDING OR CATCHING PRESENT. SAFETIED LOWER DOOR ROD GROUNDS REF MM 31-20-00 R 30.

WHILE DESCENDING THROUGH 12000 FT AT 250 KTS, THE RT SIDE FORWARD COCKPIT WINDSHIELD SHATTERED AND THE RT W/S FAULT LIGHT ILLUMINATED. 30-60 SECONDS LATER, THE RT WINDSHIELD SHATTERED, FOLLOWED BY THE LT W/S FAULT LIGHT ILLUMINATION. REMOVED & INSTALLED NEW UNIT AND PERFORMED LEAK AND OPERATIONAL CHECK GOOD. WORK DONE IAW MM 56-11-00.
ON TAKEOFF ROLL, LEFT ENGINE ITT WENT STRAIGHT TO 780 (DEG)A/ INITIAL POWER UP. NO DELAY IN ABORTING. ABORTED LESS THAN 20 KNOTS AND TEMP WAS ONLY MOMENTARY. PERFORMED ITT INDICATOR SYSTEM CHECK. OPS CHECK GOOD. PERFORMED FIVE POINT RUN CAPPED AND UNCAPPED. NO DISCREPANCY NOTED. NO BLEED AIR LEAK DETECTED. NO FURTHER ACTION TAKEN AT THIS TIME. WORK DONE IAW 545 PW&C MM CHAPTER 71-00-00.
RIGHT MAIN LANDING GEAR INDICATOR LIGHT WOULD NOT ILLUMINATE AFTER LOWERING THE GEAR. USING THE ABNORMAL CHECKLIST MANUALLY (BLEWDOWN) THE GEAR. SEVERAL MINUTES LATER THE RIGHT GEAR LIGHT DID ILLUMINATE FOR PROPER INDICATION. RESET GEAR & BRAKE BLOWDOWN BOTTLE HANDLE, SERVICED GEAR/BRAKE EMER ACC BLED GEAR SYSTEM PER CE560XL MM 12-13-01. PERFORMED LANDING GEAR, OPS CHECK PER CE560XL MM 32-01-00. FOUND TOP R/R IND INOP. REMOVED AND REPLACED BOTH BULBS PER CE560XL MM 33-10-06. OPS CHECK GOOD. NO DEFECTS NOTED.
INFLIGHT UNLOCK LIGHT ILLUMINATED WITH 3 GREEN LIGHTS LEFT, GEAR WAS FLICKERING ON/OFF. ACCOMPLISHED CHECK LIST AND BLEW GEAR DOWN. T/S TO BAD GROUND ON LEFT MAIN GEAR ACTUATOR DOWNLOCK SWITCH GL007. REPAIRED WIRING TO GROUND, JACKED UP AIRCRAFT RESET EMERGENCY N2 GEAR BOTTLE. PERFORMED GEAR FUNCTION, ALL CHECKED GOOD. BLED HYD SYSTEM AND SERVICE HYD & N2 BOTTLE. WORKED DONE IAW 560 XL M/M 32-01-00.
PERFORMED HIGH SPEED TAXI AFTER BRAKE MAINTENANCE, TAXIED BACK USING NORMAL BRAKING PROCEDURE. UPON RETURN TO RAMP, LEFT WHEEL/BRAKE ASSEMBLY WAS ON FIRE, RIGHT WHEEL/BRAKE ASSEMBLY SMOKING. EXTINGUISHED FIRE WITH PURPLE K, DRY CHEMICAL (B-C) EXTINGUISHER. INSPECTED LEFT AND RIGHT MAIN GEAR, NO OTHER DAMAGE NOTED. REMOVED AND REPLACED BOTH LT AND RT MAIN GEAR WHEELS AND BRAKES, BOTH BRAKES HAD BEEN SENT TO OVERHAUL SHOP TO DETERMINE CAUSE OF SMOKE/FIRE.
AFTER TAKEOFF OUT OF IAD FOR BCT, WITH GEAR HANDLE SELECTED UP GOT GEAR UNLOCKED WARNING LIGHT BUT GEAR STAYED DOWN. ACCOMPLISHED AIR TURNBACK TO IAD AND LANDED SAFELY. ITEM IS UNDER INVESTIGATION.
