ENGINE BECAME INCREASINGLY DIFFICULT TO START, CHECKED IGNITION TIMING AND FOUND LT MAGNETO OUT OF TOLERANCE. INSPECTED LT MAGNETO, POINTS FAILED TO OPEN DUE TO EXCESSIVLY WORN BREAKER POINT CAM. 87 HOURS SINCE NEW
NR 3 CYLINDER DEPARTED THE ENGINE AFTER FAILURE OF THE CYLINDER HOLD DOWN NUTS TO MAINTAIN PROPER TORQUE. (3) ADDITIONAL CYLINDERS FROM THE SAME ENGINE WERE INSPECTED REVEALING (5) MORE UNDER TORQUED CYLINDER HOLD DOWN NUTS.
CYLINDER HEAD SEPARATED FROM CYLINDER FOR REASONS UNKNOWN. (K)
DESCENDING IN FLIGHT NR 3 CYL ON RT ENG FAILED. ENG WAS SHUTDOWN AND FEATHERED. AC LANDED WITHOUT FURTHER INCIDENT. THERE WAS NO FIRE. POST INCIDENT INSP OF AREA FOUND, CYLINDER HAD CRACKED AND DISLOCATED AT INTERFACE AREA OF STEEL BARREL AND CYLINDER HEAD. INTAKE PUSHROD HAD SEPARATED AND WAS LYING ON THE CYLINDER HEAD, WITH CONSIDERABLE OIL LOSS. FURTHER INSP FOUND ENGINE BAFFLING INTACT AND CORRECTLY POSITIONED. CYLINDER PRESENTED NO OBVIOUS FLAWS AT THE MOST RECENT INSP, WITH OIL CONSUMPTION AND COMPRESSION WITHIN NORMAL LIMITS. IT IS SUSPECTED THAT A DEFECT IN CYLINDER REPAIR PROCESS (I.E., GRINDING) ALLOWED AN ABNORMAL STRESS CONCENTRATION TO CAUSE A FAILURE. CYL WAS REGROUND AND HONED TO P.010. (K)
DURING A PART 91 TRAINING FLIGHT, IT WAS NOTED THAT THERE WAS AN ASYMMETRICAL FLAP CONDITION (SLIGHT). CREW RETURNED TO BASE AND MAINTENANCE FOUND FLAP TORQUE TUBE 80 PERCENT CRACKED THROUGH AT LEFT BELLCRANK AREA.
