THE REPAIR WO PAPERWORK THAT ACCOMPANIED THE EMB SAID THAT THE ORIGINAL PROBLEM IT WAS SENT IN FOR WAS A "GENERATOR DISCONNECT LIGHT". THE BOX HAD SERVICEABLE PAPERWORK BUT WAS NEVER FIXED FROM THE ORIGINAL PROBLEM.

THE PILOT REPORTED THAT WHILE ENROUTE, 4 MINUTES INTO A 15 MINUTE FLIGHT, WITH A TRAUMA PATIENT ON BOARD, LEVEL AT 1200 FEET, THE PILOT AND MEDICAL CREW ON BOARD ALL HEARD A LOUD "THUD" COMING FROM THE AREA UNDER THE PILOTS FLOORING. THE PILOT AND CREW ALL CONFIRMED THAT NOTHING HAD BEEN DROPPED. THERE WERE NO FLUCTUATIONS ON THE GAUGES. THE PILOT IMMEDIATELY DIVERTED TO THE NEAREST AIRPORT AND PERFORMED A PRECAUTIONARY LANDING. THE LANDING WAS UNEVENTFUL. THIS IS THE 9TH OCCURANCE OF THIS "THUD" SINCE OCTOBER 2011. THIS ACFT IS SCHEDULED TO ARRIVE AT OUR REPAIR STATION NEXT WEEK, SO IT MAY UNDERGO INDEPTH TROUBLESHOOTING.

THE PILOT AND MEDICAL CREW REPORTED HEARING A LOUD BANG COMING FROM INSIDE THE ACFT. THE NOISE OCURRED AS THE PILOT WAS PULLING PITCH TO LIFT FROM THE DOLLY. HE IMMEDIATELY ABORTED THE TAKEOFF AND RETURNED TO THE DOLLY. THE ACFT WAS SHUTDOWN, THE DISCREPANCY WROTE UP IN THE LOGBOOK, AND MX WAS CALLED.

IMMEDIATELY RETURNED TO DEPARTURE AND LANDED WITHOUT INCIDENT. ACFT GROUNDED. MX SUSPECTS THAT THE SOURCE OF THIS PROBLEM IS A FLEXING BELLY ACCESS PANEL (NON-STRUCTURAL).

CREW REPORTED SMELLING FUEL THROUGHOUT THE CABIN APPROX 15 MINUTES INTO FLIGHT. THE ODOR LASTED FOR ABOUT A MINUTE, AND THEN DISAPPEARED. LANDED AT DESTINATION WITHOUT INCIDENT. OPEN BOTH ENGINE COWLINGS AFTER SHUTDOWN TO INSPECT FOR ANY SIGN OF A FUEL LEAK AND FOUND NONE. INSPECTED ACFT FOR SIGNS OF A FUEL LEAK. NO DEFECTS NOTED. REFUELED ACFT WITH NO ISSUE AND RETURNED TO BASE. ON FLIGHT BACK, AGAIN FUEL ODOR DETECTED IN CABIN. MECHANIC INSPECTED ACFT. NO DEFECTS NOTED. LATER FLIGHT CHECK REVEALED NO DEFECTS AND ACFT RETURNED TO SERVICE.

WHILE PERFORMING AN 800 HOUR INSP OF THE MAIN ROTOR BLADES, FOUND 2 BLADES THAT THE BEARING PIN, EXPANSION BOLT, AND NUT WERE NOT INSTALLED PROPERLY BY THE MFG. BOTH EXPANSION BOLTS WERE BROKEN DURING REMOVAL FOR THE FIRST 800 INSPECTION INTERVAL ON THESE BLADES. THE BEARING PIN HAD SLOTS IN IT AND THE NUT HAS TABS ON AND THEY LOCK TOGETHER. WHEN THEY ARE PROPERLY LOCKED 1 TO 2MM OF THREAD MUST PROTRUDE THROUGH THE NUT. WE HAVE ISSUED A QAN TO INSPECT THE BOLTS FOR PROPER SEATING AND HAVE CONTACTED THE MFG.
