DURING AIRCRAFT ACCEPTANCE INSPECTION MAINTENANCE FOUND ATTACH BRACKETS WORN, A 6 INCH CRACK ON BOTTOM TE NEAR INBOARD END AND A 6 INCH CRACK ON TOP TE NEAR INBOARD END. REPAIRED CRACK IN TOP SKIN IAW SRM 57-00 AND BOTTOM SKIN IAW AIR MIDWEST TO 1900-27-00-0001. REPLACED BRACKETS IAW SRM 57-00. (M)

DURING A FLEET CAMPAIGN DIRECTIVE INSPECTION, BOTH THE LEFT AND RIGHT OUTBOARD ELEVATOR ATTACH BRACKETS WERE FOUND TO HAVE INTERGRANNULAR CORROSION. BOTH BRACKETS WERE REPLACED. (M)

MTH - AIRCRAFT DEPARTED. ON CLIMB AT APPROXIMATELY 9,000 FEET, YELLOW LOOP ANN FLICKERED AND RED FIRE T-HANDLE ILLUMINATED. PILOT MADE PRECAUTIONARY RT ENGINE SHUT-DOWN AND RETURNED TO MTH. INSPECTION REVEALED RT ENG AFT FIRE LOOP BROKEN, VIBRATION CAUSED.
