Part: RELIEF VALVE — LEAKING (FUEL STORAGE) · airframe 10,802 hrs · 5,503 cycles

SHORTLY AFTER T/O, TOWER REPORTED FUEL SPRAYING FROM RT WING. CREW VISUALLY VERIFIED, RETURNED AND LANDED WITHOUT INCIDENT. INSP FOUND FUEL LEAKING FROM RT WING VENT. WING FUEL PRESSURE RELIEF VALVE FOUND TO BE OPEN. EXTENDED REFUELING TEST WAS PERFORMED AND VALVE WENT TO CLOSED POSITION. STUCK VALVE WAS SUSPECTED. LT AND RT WINGS WERE REFUELED TO MAX LEVEL AND A/C STOPPED LEAKING FUEL. NO FURTHER DISCREPANCIES WERE NOTED. WHEN A/C ATTEMPTED T/O, FUEL WAS NOTICED STREAMING FROM RT WING. T/O WAS ABORTED PRIOR TO REACHING V1 SPEED. REPLACED RT WING LEVEL CONTROL VALVE AND FUEL VENT VALVE. BOTH ENGINES RUN TO T/O POWER WITH NO NOTED DEFECTS. A/C RETURNED TO SERVICE.

Part: BULB — BURNED OUT (NLG INDICATOR) · airframe 10,747 hrs · 5,465 cycles

CREW REPORTED THAT UPON GEAR EXTENSION THEY DID NOT RECEIVE THE NOSE GEAR DOOR INDICATION. A FLY-BY PAST THE TOWER WAS PERFORMED TO VERIFY NOSE GEAR POSITION. DOOR INDICATION WOULD NOT TEST, AND A MANUAL GEAR EXTENSION WAS PERFORMED. GREEN NOSE GEAR DOOR LIGHT DID NOT ILLUMINATE. A REQUEST WAS MADE TO HAVE FIRE/RESCUE PERSONNEL STANDING BY. AN UNEVENTFUL LANDING WAS PERFORMED IN KSNA. MAINTENANCE REPLACED A 327 BULB FOR THE NOSE LANDING GEAR DOOR INDICATOR. OPERATIONAL CHECKS WERE GOOD. AIRCRAFT WAS RETURNED TO SERVICE.
