EMER LIGHTS OFF AMBER CAS: DURING PREFLIGHT INSPECTION, FOUND INTERIOR EMERGENCY LIGHTS AT SEAT 7 AND 5 DOES NOT ILLUMINATE. TRIED RECHARGE PROCEDURES TWICE, FAILED TO RECHARGE AND DID NOT ILLUMINATE. VERIFIED INTERIOR EMER LIGHT AT THAT CORRESPONDS WITH READING LIGHT ABOVE SEAT 7 AND OVERWING EMER EXIT LIGHT NOT ILLUMINATING. FOR TROUBLESHOOTING PURPOSES. EMER BATTERIES PACK #1 AND #3 WERE SWAPPED. EMER LIGHT IN ZONE 3 NOW ILLUMINATED BUT FAIL TO FULLY ILLUMINATE EMER LIGHT IN ZONE 1. WHICH WERE SIGNS OF A DEFECTIVE BATTERY. NOTIFIED MCC AND PLACED A BATTERY ON ORDER. CONFIGURED AIRCRAFT BACK TO NORMAL CONFIGURATION. WORK DONE IN REF WITH CE680A AMM 33-50-00.GAINED ACCESS TO BATTERY PACK 3 DISASSEMBLED SEAT #8. CABIN EMERG LIGHT: UPON SHUTTING DOWN AND POWERING OFF APU AND BATTERIES THE EMERGENCY LIGHTS OVER SEAT 3 UNDER SEAT 3 OVER MAIN CABIN DOOR AND LEFT WING INSPECTION LIGHT REMAIN ILLUMINATED //SIGNOFF// WENT OUT TO RAMP AND IMMEDIATELY POWERED UP AIRCRAFT WITH GPU AND VERIFIED EMERGENCY LIGHTS SWITCH WAS IN THE OFF POSITION. GAINED ACCESS TO THE G-SWITCH LOCATED UNDER THE FORWARD MOST LH PAX SEAT. RE-SET G·SWITCH AND ALL EMERGENCY LIGHTS THAT REMAINED ON, TURNED OFF. OPERATED THE EMERGENCY LIGHTS SWITCH NUMEROUS TIMES AND IT OPERATED NORMALLY IN ALL SWITCH POSITIONS, OPS CKS GOOD. CLOSED UP ALL ACCESSED AND RETURNED AIRCRAFT BACK TO ORIGINAL CONFIGURATION REF. MAIN EXIT SIGN/LIGHT: WITH ELECTRICAL POWER OFF AND THE EMER LIGHT SWITCH OFF, THE CABIN EXIT LIGHT REMAINS ILLUMINATED AS WELL AS THE FLOOR EXIT LIGHTS AT SEAT 3 AND THE TWO EXTERIOR EMERGENCY LIGHTS.//CORRECTIVE ACTION// RESET THE #2 EMERGENCY BATTERY PACK G SWITCH IAW CESSNA 680A AMM 33-50-00. OPS CHECK GOOD.
ENGINE FOD: WHILE DEPARTING OUT OF FTW THE LEFT ENGINE INGESTED A LARGE BALLOON. A PIECE OF IT IS STILL STUCK BETWEEN THE FAN AND THE COWL. NO VIBRATIONS NOTED OR ANY ENGINE ABNORMALITIES.//CORRECTIVE ACTION//WE ARRIVED ONSITE AND DID THE INITIAL INSPECTION. WE COULD NOT FIND ANY PIECES OF BALLOON STILL STUCK IN THE ENGINE BUT WE DID FIND A PIECE ON THE GALLEY COUNTER. COMPLETED THE INSPECTIONS CALLED OUT IN THE CE 680A AMM 05-50-00. INSPECTION OF THE COWLING, AIR INTAKE SECTION , THE FAN BLADES, AND CONE WITH NO DAMAGE OR FOD FOUND. ALSO COMPLETED THE P & W EMM 306D FOD INSPECTION CH 05-50-00 UP TO STEP TWO "INSPECT CONE, FAN BLADES, CASE, AND/OR INNER AND OUTER STATORS IF DAMAGES INDICATE THAT A FOREIGN OBJECT MIGHT HAVE ENTERED THE HP COMPRESSOR, DO A BORESCOPE INSPECTION OF THE 1ST STAGE IGV AND COMPRESSOR BLADES". SINCE THERE WAS NO DAMAGE INDICATED IN THE FIRST TWO STEPS WE STOPPED THERE. WE USED A SMALL HANDHELD BORESCOPE WE HAVE TO CHECK BEHIND THE FAN BLADES AND LOOK AT THE STATORS JUST TO BE SURE, WITH NO DAMAGE OR FOD FOUND. MCC REACHED OUT TO THE P&W TECH REP TO ENSURE NO FURTHER INSPECTIONS WERE REQUIRED. P&W TECH REP ROBERT FISHER RESPONDED "SINCE THE FOD INSPECTION WAS COMPLETED PER EMM 05-50-00 AND NO DAMAGE OR EVIDENCE OF DEBRIS WAS FOUND, NO FURTHER ACTION IS REQUIRED AT THIS TIME."
MAIN DOOR: DURING THE CLOSING OF THE MAIN CABIN DOOR AT THE HALF WAY POINT OF THE CYCLE UPWARDS THE DOOR PRODUCES A BINDING / SCREECHING SOUND. WE ALSO NOTED A CABIN DOOR MONITOR PCB ON THE FAULT HISTORY PAGE. // SIGNOFF// INSPECTED MED DOOR FOR ANY OBVIOUS DISCREPANCIES BEFORE PERFROMING OPS CHECK AND FOUND NONE. OPENED AND CLOSED DOOR TO VERIFIY WHERE THE BINDING WAS OCCURING. INSPECTED AFT TELESCOPING ARMS AND FOUND DEBRIS JAMMED IN TO THE UPPER TELESCOPING ARM OF THE MED DOOR. FLUSHED ALL DEBRIS OUT USING AEROSOL CONTACT CLEANER AND APPLIED LUBRICATION TO BOTH THE UPPER AND LOWER TELESCOPING ARMS. OPERATED MED DOOR SEVERAL TIMES WITH NO DEFECTS NOTED. ALL WORK IN REF TO CE-680A MM 52-10-00
RETRIM NOSE LEFT AMBER CAS: AT ROTATION FELT STIFFNESS IN L RUDDER PEDAL, AIRCRAFT TRACKED STRAIGHT. AT FLAP RETRACTION CAS ILLUMINATED, & REMAINED ON FOR DURATION OF FLT. FOLLOWED CHECKLIST . NOTED HEAVINESS IN RUDDER PEDALS, TRAVEL WAS AVAILABLE. FULL LEFT RUDDER TRIM WAS REQUIRED. THIS ISSUE HAPPENED COINCIDENT WITH ELEVATOR ISSUE. LT ELEVATOR CABLE, AT ROTATION, AIRCRAFT POPPED UP (OVER ROTATED) EVEN THOUGH PITCH TRIM SETTING WAS CONFIRMED PROPER. ABLE TO CORRECT THE PITCH ATTITUDE & CONTINUE NORMAL CLIMBOUT. AIRCRAFT PITCH CONTROL FELT NORMAL THEREAFTER. DURING POSTFLIGHT, THE LT ELEVATOR HANGING DOWN, THE RT NEUTRAL. AFTER INVESTAGATION FOUND DAMAGE WAS DETERMINED TO BE CAUSED BY OTHER AIRCRAFT.

CABIN ODOR BURNING SMELL - THROUGH 28000 FEET DURING CLIMB BOTTH THE CREW AND PASSENGERS NOTICED AN ELECTRICAL BURNING SMELL. THERE WAS NO SMOKE. WE TURNED OFF THE INTERIOR MASTER SWITCH AND THE SMELL WENT AWAY. WE CONTINUED TO DESTINATION AS IT WAS ONLY 15 MINUTES AWAY. THE SMELL NEVER CAME BACK. *** INSPECTED CABIN ELECTRICAL SYSTEM AND FOUND R/H FORWARD MOST UP WASH LIGHT BURNT. REPLACED LIGHT ASSY WITH NEW ITEM. RAN ENGINES AT VARYING POWER ALONG WITH ALL INTERIOR ELECTRICAL COMPONENTS AND COULD NOT DUPLICATE SMELL.

COCKPIT ODOR OIL SMELL - DURING FLIGHT, SELECT A WARM TEMPERATURE, THERE'S A STRONG OIL SMELL IN THE COCKPIT. BORESCOPED APU INLET FOR ERODED OIL SEALS WITH NO ABNORMALITIES FOUND. RAN THE APU WITH BLEED AIR SELECTOR IN VARIOUS POSITIONS AND RAN BOTH ENGINES WITH ANTI ICE ON INDIVIDUALLY AND EXCLUSIVE FROM THE APU TO DETECT ANY ABNORMAL SMELLS, NONE FOUND. REPLACED WATER SEPARATOR CONDENSER BAG.
COCKPIT ODOR BURNING SMELL - THERE IS A STRONG SMELL OF FUMES IN THE COCKPIT AND CABIN. THERE IS A SMELL OF EXHAUST OR BURNT FUEL THAT COMES AND GOES IN THE AIRCRAFT, STRONG ENOUGH THAT IT MAKES YOUR EYES BURN. IT IS STRONGER WHEN THE THERMOSTAT KICKS THE HEAT ON. IT IS STRONG ON THE GROUND WITH THE APU AND BLEED AIR ON OR IN THE AIR (IN-FLIGHT) WITH THE APU OFF. IN-FLIGHT IS WHEN THE EXHAUST SMELL IS THE STRONGEST. PERFORMED ECS SYSTEM BURN OUT TASK CARD & INSTALLED NEW CONDENSER BAGS. INSPECTED APU INLET & APU BLEED OUTLET WITH NO DEFECTS. OPS, LEAK, & ODOR CHKS SATISFACTORY.
CORRECTED SDR AS PER FAA REQUEST. UPON SELECTING LANDING GEAR DOWN ONLY THE RIGHT MAIN GREEN AND GEAR UNSAFE LIGHTS ILLUMINATED. COMPLIED WITH CHECKLIST. FOUND GEAR CONTROL CB POPPED. AFTER A NORMAL TAKEOFF LEVELED OFF AT 8000 FT AND THE RT ENGINE OIL PRESS LIGHT ILLUMINATED WITH A MASTER WARNING. THE RT ENGINE OIL PRESS GAUGE SHOWED ZERO. ACCOMPLISHED THE ENGINE SECURING CHECKLIST. DECLARED AN EMERGENCY AND RETURNED TO THE DEPARTURE AIRPORT. AFTER SHUTDOWN CHECKED THE OIL SIGHT GLASS ON THE RIGHT ENGINE AND IT WAS SHOWING OIL JUST BELOW THE FILL LINE. REMOVED UNSERVICEABLE RT ENGINE S/N: PCE-DD0052 DUE TO LOSS OF OIL PRESSURE. INSTALLED A SERVICEABLE REPAIRED ENGINE S/N: PCE-DD0375 AT THE RT POSITION AND ACCOMPLISHED OPERATIONAL AND LEAK CHECKS WITH NO DEFECTS NOTED. PERFORMED 5 PT. CALIBRATION RUN WITH ALL PARAMETERS WITHIN EMM LIMITS. ALL WORK ACCOMPLISHED IAW AMM 71-00-01, 71-00-01 AND PW545B EMM 71-00-00.
DURING A SCHEDULED INSPECTION WE FOUND THE FLEX PAN CRACKED ON THE CAPTAINâS SEAT ASSEMBLY. THE FLEX PAN HOLD THE CREW SEATS LOWER SEAT CUSHION. SEAT MODEL 1423 IN THE COMPONENT MM ITEM 100. SERVICE CONDITION REPORT SENT TO UNDER REPORT 707939.
ON SHORT FINAL, POWER BRAKES CIRCUIT BREAKER TRIPPED AND ANTI-SKID WARNING LIGHT ILLUMINATED. COMPLIED WITH QRH AND LANDED. R & R THE POWER BRAKE MOTOR. PERFORMED BLEED AND OPS CHECK OF BRAKE SYSTEM. OPS CHECK GOOD IAW MM 32-44-00 AND MM 32-44-20.
WHEN BRAKES ARE APPLIED DURING TAXI THE ACFT VIBRATES AND SHAKES. BLED BRAKES, NOTICED AIR IN LT BRAKE, TAXI & LEAK CHECKED GOOD. WORK DONE IAW MM 32-44-00.
AFTER TAKEOFF, GEAR HANDLE WOULD NOT COME UP. HANDLE STAYED IN DOWN LOCKED POSITION. FOUND LT SQUAT SWITCH ON EDGE OF ADJUSTMENT. ADJUSTED SQUAT SWITCH TO SPECS IAW MM 32-61-02, LANDING GEAR CONTROL OPS CHECKED GOOD IAW MM 32-61-00.
DESCENDING THROUGH 34,000 FT, BOTH PILOTS OBSERVED AN EXHAUST FUME ODOR THAT DID NOT DISSIPATE UNTIL THE ENGINES WERE SHUTDOWN. R & R ECS WATER SEPARATOR CONDENSER IAW MM 21-51-00. OPS & LEAK CHECKED GOOD.
AFTER TAKEOFF, PASSING 10,000' (WITH NO DEMAND ON ANY HYDRAULIC SYSTEMS) THE LOW HYD FLOW R ANNUNCIATOR ILLUMINATED. CONTINUED THE CLIMB TO FL250. 15 MINUTES LATER, DURING DESCENT THRU FL230 THE LIGHT EXTINGUISHED AND DID NOT REOCCUR. ALL HYDRAULIC SYTEMS WORKED NORMALLY DURING THE REMAINDER OF APPROACH AND LANDING. R & R THE RIGHT HYD SYSTEM "LO HYD FLOOR" SWITCH IAW MM 29-11-04. INSPECTION OF THE RT HYD SYS FILTER SHOWED NO CONTAMINATION OPS AND LEAK CHECKS ARE GOOD.
DURING A SCHEDULED INSPECTION, FOUND THE RT ELEVATOR OTBD HINGE BRACKET CRACKED USING EDDY CURRENT NDT METHOD. THE CRACKED BRACKET WAS SENT TO MFG FOR EVALUATION AND A REPORT WAS SENT UNDER SCR 624231.

ON PREFLIGHT, AFTER TURNING ON APU BLEED AIR, LOUD WHINING SOUND CAME FROM THE REAR OF ACFT ACCOMPANIED BY BURNING ODOR. SHUTDOWN APU BLEED AND RAN BOTH ENGINES WITH SAME RESULT. ACM TURBINE IS SEIZED. R & R THE ACM IAW MM 21-51-00. OPS AND LEAK CHECKED GOOD.
VIBRATION OCCURRED IN FLIGHT WITH A 10 KNOT RAPID DECREASE IN AIRSPEED (AS IF THRUST REVERSER PARTIALLY DEPLOYED). NO THRUST REVERSER COCKPIT INDICATIONS ILLUMINATED. ON POSTFLIGHT NOTED LOWER RT THRUST REVERSER BUCKET STILL DEPLOYED ABOUT .2500 INCH AND NOT FULLY STOWED. PERFORMED UNSCHEDULED ACCIDENTAL IN-FLIGHT THRUST REVERSER DEPLOYMENT CHECK IAW MM 5-50-00. NO DEFECTS OR DAMAGE FOUND. REMOVED AND REPLACED RT THRUST REVERSER. ALL WORK IAW MM 78-31-00. OPS AND LEAK CHECKED GOOD.
LT BRAKE PEDAL MUSHY AND DEPRESSES TO THE STOP. WHILE TAXING IN, MAKING A LT TURN, FELT LITTLE TO NO BRAKING ACTION REMAINING. RT SIDE, LT BRAKE PEDAL NOMINAL. INSPECTED BRAKE, BRAKE COMPONENTS AND LINES FOR LEAK. NONE FOUND. CHECKED OPERATION AND FOUND AIR IN LT BRAKE SYS. COMPLETED BLEED PROCEDURES ON LT AND RT BRAKE SYS. OPS CHECKED GOOD IAW MM 32-44-00.
STANDBY ATTITUDE INDICATOR FAILED INFLIGHT, NO PREVIOUS PROBLEMS, NO TURBULANCE, SCREEN SHOWED "ATT FAIL 126 SEC".
AFTER TAKEOFF, GEAR WAS SELECTED TO THE UP POSITION. LT MAIN DOWNLOCK LIGHT (GREEN LIGHT) REMAINED ILLUMINATED.
AFTER TAKEOFF, GEAR WAS SELECTED TO THE UP POSITION. LT MAIN DOWNLOCK LIGHT (GREEN LIGHT) REMAINED ILLUMINATED. RED UNLOCKED LIGHT DID NOT REMAIN ILLUMINATED. REMOVED AND REPLACED LT MLG ACTUATOR REF AMM 32-30-04. ALL ATTACHING HARDWARE IAW IPC HAS BEEN REPLACED WITH NEW. PERFORMED MAIN GEAR CAMBER CHECK AND FUNCTIONAL CHECK, OPS/LEAK CHECK GOOD.
NR 2 AHRS FAILED IN FLIGHT. RT PFD DISPLAYED "ATT FAIL" AND "HDG FAIL" EMERGENCY CHECKLIST TAB V4 AND X1 COMPLIED WITH. REMOVED DEFECTIVE NR 2 AHRS UNIT. INSTALLED REPAIRED UNIT; OPS CHECK GOOD. WORK DONE IAW MM CHAPTER 34-21-01.
AIRCRAFT ON ACTIVE, PUSHED FOR TAKEOFF POWER, GOT FUEL GAUGE ANNUN LIGHT. TAXIED OFF RESET LIGHTS AND TOOK ACTIVE AGAIN WITHOUT PROBLEMS.
FOUND THE STATIC SYSTEM DRAIN VALVE POPPET RETAING RING COMING LOOSE FROM VALVE BASE.
