CORROSION FORMING AT STA3300 RIGHT SIDE CABIN OVERHEAD AT BULKHEAD FITTING PENETRATION AREA. CORROSION WAS REMOVED AND FITTING REPLACED.
SEAL LEAKED ON THE TRANSMISSION, WASHED GREASE OUT OF BEARING.
WEAK M/R DAMPER CAUSED ENGINE TORQUE MISCOMPARE. CAUSED BY VIBRATION.
TAIL ROTOR BLADE LIGHTNING BONDING STRIP BROKE IN FLIGHT AND CONTACTED MAIN ROTOR BLADE. REPLACED LIGHTNING BONDING STRIP, TAIL ROTOR BLADE AND MAIN ROTOR BLADE.
LEFT SPONSON AFT PLATE CORRODED. WHEN THE CORROSION WAS REMOVED, THE DEPTH EXCEEDED THE ALLOWABLE LIMIT. THE CORROSION APPEARS TO HAVE STARTED FROM PREVIOUSLY DRILLED HOLES WHICH WERE UNTREATED WITH A CORROSION PREVENTATIVE COMPOUND. THIS SHOULD HAVE BEEN APPLIED DURING MFG.
SEAT, PN: 3G2520V00131 HAS CORROSION WHERE SEAT BACK CUSHION, PN: 3G2520L00251 CONTACTS THE METAL WEBBING ATTACHED TO THE FRAME. AFTER FURTHER INVESTIGATION IT HAS BEEN FOUND THAT OTHER SEAT BACK CUSHIONS ARE MFG WITH A DIFFERENT MATERIAL THAT CONTACTS THE WEBBING HAS NOT HAD THIS ISSUE. 10 OF THE 12 SEAT FRAMES ARE CORRODED ON THE WEBBING.
ON DAILY POST FLIGHT INSPECTION, IT WAS DISCOVERED THAT ONE OF THE ROTATING SCISSORS WAS NOT CONNECTED TO THE SWASHPLATE. BEARING HAD COME OUT OF THE RACE.
NEW STYLE EXHAUST SYS FAILURE AT ATTACHMENT POINTS WHERE RUBBER GROMMETS INSULATOR CONNECT TAIL PIPE. AFTER GROMMETS FAIL, THE MOVEMENT AND EXHAUST VIBRATION LEAD TO EXCESSIVE FIREWALL CRACKING.
