Maintenance disclosures
WITH WINDSHIELD HEAT ON, CLIMBING OUT LT WINDSHIELD CRACKED AT FL 10,000 FT. WINDSHIELD HEAT SELECTED OFF & ACFT LANDED. PARTS WERE ORDERED & THE ACFT WAS REPAIRED WITH A WINDSHIELD CHANGE ON 06 10 15. THE ACFT RETURNED TO SERVICE.
RDU - CREW REPORTED IN CRUISE FLIGHT AT 17,000 FEET WITH WINDSHIELD HEAT ON, A BRIGHT FLASH WAS OBSERVED FROM THE RIGHT WINDSHIELD FOLLOWED BY A LOUD POPPING SOUND. IT WAS OBSERVED THAT THE RIGHT OUTER PANE OF THE WINDSHIELD CRACKED IN THE SPIDER WEB PATTERN. THE AIRCRAFT MADE UNEVENTFUL LANDING IN RDU. UPON INVESTIGATION THE BROKEN WINDSHIELD, IT WS DETERMINED THERE WERE BURN MARKS BETWEEN THE PANE OF IN THE AREA OF TEMP SENSORS. THE WINDSHIELD WAS REPLACED WITH A NEW UNIT; A FULL FUNCTIONAL TEST OF THE WINDSHIELD HEAT SYSTEM WAS ACCOMPLISHED WITH NO OTHER DEFECTS NOTED. (M)
THE LEFT HAND SPEED LEVER CABLE RUNNING FROM THE CENTER PEDESTAL TO JUST AFT OF BULKHEAD STA 130 WAS FOUND WITH MULTIPLE WIRE STRANDS BROKEN. AT THE TIME OF REMOVAL, APPROXIMATELY FOUR STRANDS OF CABLE REMAINED INTACT. NO SIGNS OF CHAFING WERE DETECTED ON INSPECTION OF THE CABLE AND AIRFRAME. THE CABLE IN QUESTION WAS REPLACED WITH A NEW UNIT. (X)
FOUND LOOSE RIVETS AT BOTH ENDS OF PILOT AND CO-PILOT RUDDER PEDAL TORQUE TUBE.
