DURING TAKE OFF FUEL QUANTITY GUAGE LOST POWER, RECYCLED QUANTITY CIRCUIT BREAKER 3 TIMES, QUANTITY INDICATOR CAME ON AND AT THE SAME TIME RT TRANSFER PUMP CAUTION ILLUMINATED. RECYCLED RT PUMP CIRCUIT BREAKER BUT COULD NOT EXTINGUISH TRANSFER PUMP LIGHT. QUANTITY GUAGE FAILED AGAIN BEFORE LANDING. MAINTENANCE REPLACED FUEL QUANTITY INDICATOR PN 407375006101 SYSTEM CHECKED GOOD AIRCRAFT RETURNED TO SERVICE.
DISC PACK FOUND TO HAVE A CRACKED DISC. REMOVED AND REPLACED.

BEARING FAILED IN FLIGHT. MAJOR BEARING FAILURE.
UPON DISASSEMBLY OF GEARBOX DUE TO CHIP LIGHT. FOUND EXCESSIVE GALLIN ON PINION AND T/M GEARS. T/M GEAR HAD ROUGH EDGES CAUSING PINION GEAR TO GALL. ASSURE PROPER MACHINING AND MESHING OF GEAR TEETH AT FACTORY.
PILOT NOTICED THE LEFT CENTER SLAT MISSING AFTER LANDING AND SHUT-DOWN AT OFFSHORE PLATFORM MC268. MOUNTING BRACKETS FOR THE LEFT CENTER SLAT WERE CRACKED AND BROKEN ALLOWING SLAT TO DEPART THE AIRCRAFT. LEFT VERTICAL FINLET WAS DAMAGED BY THE MISSING SLAT. (X)
