PROPELLER RPM INDICATED 300 RPM HIGH DURING FLIGHT. PILOT PERFORMED PRECAUTIONARY LANDING. NO OTHER PERIMETER FLUCTUATIONS. REPLACED INDICATOR WITH NEW PART. OPERATIONAL CHECK GOOD.
PROPELLER RPM GAUGE INDICATED 300 RPM OVER REDLINE DURING FLIGHT. PILOT PERFORMED PRECAUTIONARY LANDING AT FIELD OTHER THAN PLANNED. FURTHER INVESTIGATION OF ENGINE AND SYSTEM: NOR TORQUE FLUCTUATUON, NO ITT FLUCTUATION, NO N FLUCTUATION, INDICATOR FAILURE ONLY. REPLACED INDICATOR. OPS CHECKED GOOD.
LOOSE AND WORKING RIVETS IN INBOARD LEADING EDGE ATTACHMENT TO FORWARD SPAR RT AND LT WING.
PILOT REPORTED SMOKE IN COCKPIT 30 MINUTES INTO FLIGHT. CONNECTOR FROM PROPELLER HEAT CIRCUIT BREAKER (P-186/J-186). CONNECTOR, 12 GAUGE WIRE(HA-36). MAINTENANCE CREW FOUND CONNECTOR FULLY ENGAGED AND STILL ATTACHED TO WIRE BUNDLE. MAINTENANCE FOUND TERMINAL PINS INSIDE CONNECTOR LOOSE CAUSING HIGH RESISTANCE WHICH RESULTED IN HEATING AND BURNING OF PLUG. SINCE THERE IS HIGH CURRENT DRAW IN THIS CIRCUIT AND THOUGH THIS CONNECTOR IS RECOMMENDED THAT THE PINS AT THIS CONNECTOR BE CHECKED FOR PROPER TIGHTNESS AND ENGAGEMENT.
PRIMARY FLAP MOTOR DRIVE COUPLER FAILED. STAND BY SYSTEM USED TO EXTEND FLAPS. FLAP LIMIT SWITCHES NOT INCORPORATED IN STAND BY SYSTEM. OPERATIONAL SWITCH OF STAD BY SYSTEM HELD TOO LONG AND FLAP MOTOR CONTINUED TO RUN PAST LIMIT SWITCH LOCATION AND TWISTED FLAP DRIVE ASSEMBLY OUT OF MOUNTING BRACKET. RECOMMENDATION, PILOT BE MORE AWARE OF SYSTEM OPERATION OR INCORPORATE SWITCHES INTO STAND-BY SYSTEM.
