AFTER ENGINE START, THE CREW NOTICED A RT COOLING PACK FAIL INDICATION AND SMOKE THE THE COCKPIT. THE CREW SHUTDOWN THE ENGINE AND NO OTHER ISSUES WERE NOTED. MAINTENANCE TROUBLESHOT THE NR 2 COOLING PACK FAILURE AND FOUND A BROKEN CLAMP ON THE BLEED AIR TO HEAT EXCHANGER FEED LINE. REMOVED AND REPLACED THE CLAMP. VISUALLY INSPECTED THE REMAINDER OF THE COOLING PACK. OPERATIONAL CHECKED GOOD.

THE LEFT MAIN GEAR WOULD NOT RETRACT AFTER TAKEOFF FROM TNN. CREW SELECTED THE LANDING GEAR DOWN AND ALL THREE GEAR INDICATED DOWN AND LOCKED. THE AIRCRAFT RETURNED TO TNN AND LANDED WITHOUT INCIDENT. MAINTENANCE COULD NOT DUPLICATE THE PROBLEM WITH THE AIRCRAFT ON JACKS. THE PROBLEM ONLY OCCURRED IN FLIGHT. THE LEFT GEAR RETRACT ACTUATOR AND SYSTEM CHECKED NORMAL ON THE GROUND. FLIGHT TEST WAS CONDUCTED WITH SYSTEM WORKING NORMAL.

ON TAKEOFF FROM ATLANTIC CITY (ACY), CREW RECEIVED AN AURAL WARNING CHIME ALONG WITH WARNING LIGHT ON THE MULTIPLE ALARM PANEL FUEL AND THE RIGHT ENGINE FUEL FILTER LIGHT ILLUMINATION. THE TAKEOFF WAS ABORTED AND THE AIRCRAFT RETURNED TO THE GATE WITHOUT FURTHER INCIDENT. MAINTENANCE REMOVED AND REPLACED THE RIGHT ENGINE LOW AND HIGH PRESSURE FUEL FILTERS. IT WAS REPORTED A SMALL AMOUNT OF METAL SHAVINGS WERE FOUND IN THE LOW PRESSURE FILTER. BOTH ENGINES WERE GROUND RUN AND LEAK CHECKED WITH EVERYTHING CHECKING NORMAL. AIRCRAFT WAS RETURNED TO SERVICE.

FLIGHT NUMBER 1070 LEG 3 KAFW-KJQF. AFTER TAKEOFF WHEN FLAPS WERE SELECTED UP, THE INBOARD FLAPS REMAINED AT 15 DEGREE WHILE THE NACELLE AND OUTBOARD FLAPS RETRACTED TO 0 DEGREES. CREW RECEIVED A FLAP "DISAGREEMENT" WARNING. ATC WAS NOTIFIED AND AIRCRAFT TURNED TO RETURN TO AIRPORT. WHEN CREW SELECTED FLAPS 15 DEGREE FOR LANDING, THE NACELLE AND OUTBOARD FLAPS EXTENDED TO 15 DEGREES WHILE THE LEFT INBOARD FLAP RETRACTED TO ZERO DEGREES AND THE RIGHT INBOARD FLAP EXTENDED TO 45 DEGREES. CREW RECEIVED A FLAP "ASYMMETRY" WARNING ON THE INBOARD FLAPS. AN EMERGENCY WAS DECLARED AND THE AIRCRAFT LANDED AT KAFW WITHOUT INCIDENT. MAINTENANCE FOUND THE RIGHT INBOARD FLAP ACTUATOR HAD FAILED. THE ACTUATOR WAS REPLACED AND THE AIRCRAFT WAS RETURNED TO SERVICE.
