HUNG START ON NO.2 FOLLOWED BY FUEL SMELL IN A/C. PERFORMED GVI OF ENG 2 NO OBVIOUS DEFECTS NOTED OTHER THAN POOLED FUEL. DRY MOTORED ENG TO CLEAR OUT FUEL. DURING DRY MOTOR CONSTANT FUEL SPRAY WAS NOTED AT THE 6 OCLOCK POSITION WITH A 0 FUEL FLOW INDICATION ON THE EICAS SUSPECT FUEL LEAK INSIDE THE ENGINE. UNABLE TO CONTINUE T/S AT THIS TIME. DURING B INSPECTION FOUND THE 1, 2, 3 & 4 EMERGENCY BATTERIES FAILED CHECK. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED THE EMERGENCY BATTERIES AND THE BATTERY CHARGER ASSEMBLIES. THE OPERATIONAL CHECK WAS GOOD. THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING MAINTENANCE INSPECTION NOTICED THE FLIGHT ATTENDANT EMERGENCY FLASHLIGHT WAS MISSING. THE AIRCRAFT WAS REMOVED FROM SERVICE. FOUND THE MISSING FLIGHT ATTENDANT EMERGENCY FLASHLIGHT AS REQUIRED. THE OPERATIONAL CHECK WAS GOOD. THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING B INSPECTION FOUND THE CABIN AREA EMERGENCY LIGHT STRIPS INOPERABLE. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED THE NR 2 INVERTER IAW EMB-145 AMM 33-50-11. REMOVED AND REPLACED CONNECTOR ON FIRST AND LAST FLOOR EMERGENCY LIGHT STRIPS IAW EMB-145 SWPM 20-21-00, AND REPLACED EMERGENCY LIGHTING POWER SUPPLIES #1-4I AW MNTC WC 33-700-19. THE OPERATIONAL CHECK WAS GOOD. THE AIRCRAFT WAS RETURNED TO SERVICE.
ON FLIGHT 2878 THE FLIGHT ATTENDANT REPORTED THE FORWARD FLASHLIGHT IS WEAK AND DOESN'T CHARGE DISPITE RED LIGHT OF CHARGING. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED THE FORWARD FLIGHT ATTENDANT FLASHLIGHT ASSEMBLY IAW AMM 33-50-16. THE OPERATIONAL CHECK WAS GOOD. THE AIRCRAFT WAS RETURNED TO SERVICE.
ON FLIGHT 3421 THE CREW REPORTED THAT AFTER FLAP 45 WAS SELECTED AND INDICATED " FLAP FAIL" MESSAGE APPEARED ON EICAS FLAP DID NOT RETRACT AFTER LANDING. THE AIRCRAFT WAS REMOVED FROM SERVICE. PERFORM OPERATIONAL CHECK ON FLAP SYSTEM FROM 0 TO 45 ABOUT 20 TIMES. THE OPERATIONAL CHECK WAS GOOD PER AMM 27-53-00. THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING B-INSPECTION FOUND THE RIGHT OVERWING EXTERIOR EMERGENCY LIGHT INOPERABLE. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED THE RIGHT OVER THE WING EMERGENCY LAMP IAW EMB 145 AMM 33-50-07-000-801-A AND 33-50-07-400-801-A. THE OPERATIONAL CHECK WAS GOOD AT THIS TIME. THE AIRCRAFT WAS RETURNED TO SREVICE.
STRONG ODOR SIMILAR TO BURNING RUBBER EMANATING FROM COCKPIT AREA DURING DESCENT. NO SMOKE NOTED NOTHING OBSERVED OTHERWISE. REMOVED AND REPLACED RT EXHAUST FAN PER 21-26-02. OPS CHECK GOOD. PASSED FCF.
DURING PREFLIGHT INSPECTION FOUND THE BAGGAGE EXTINGUISHER LIGHT DOES NOT ILLUMINATE DURING FIRE TEST. THE LIGHT DID NOT ILLUMINATE AND STAYED ON AFTER TEST. THE AIRCRAFT WAS REMOVED FROM SERVICE. COMPLIED WITH OPERATIONAL CHECK OF FIRE DETECTION SYSTEM IAW AMM 26-10-00. NO PARTS REMOVED OR REPLACED. THE OPERATIONAL CHECK WAS GOOD.
ON FLIGHT 3579 DURING THE CLIMB THE FLIGHT ATTENDANT REPORTED SHE SMELLED SMOKE AND SAW SMOKE COMING FROM AROUND THE MAIN PASSENGER DOOR. THE ODOR WENT AWAY AFTER A COUPLE OF MINUTES. THE CREW ELECTED TO RETURN AND LANDED WITHOUT INCIDENT. THE AIRCRAFT WAS REMOVED FROM SERVICE. PERFORMED ENGINE RUN'S NO ODOR OR SMOKE ANY WHERE IN AIRCRAFT NO DISCREPANEY NOTICE IAW AMM EMB 145 71-00-06. NO PARTS REMOVED OR REPLACED. THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING MAINTENANCE BASE VISIT FOUND STRINGER 21 RIGHT FORWARD OF FRAME 13 TO BE CORRODED. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED STRINGER 21 RIGHT WITH NEW STRINGER 21 RIGHT IAW EMB SRM 51-40-02. THE AIRCRAFT WAS RETURNED TO SERVICE.

ON FLIGHT 3654 DURING CRUISE THE CREW REPORTED A "LAV SMOKE" MASTER WARING MESSAGE. CIRCUIT BREAKER E11 & E12 WAS PULLED PER AIRCRAFT OPERATING MANUAL VOL 1 AND THE FIRST OFFICER INVESTIGATED THE LAVATORY. NO EVIDENCE OF FIRE ANYWHERE. THE LAVATORY WAS THEN LOCKED DOWN. THE AIRCRAFT LANDED WITHOUT INCIDENT. THE AIRCRAFT WAS REMOVED FROM SERVICE. PERFORMED TEST OF THE LAVATORY SMOKE DETECTION SYSTEM PER EMB 145 AMM 26-11-00. THE OPERATIONAL CHECK WAS GOOD, NO DEFECTS NOTED. NO PARTS WERE REMOVED OR REPLACED. THE AIRCRAFT WAS RETURNED TO SERVICE.
