FINALLY FOUND PIN 11 (CONNECTION ON 429 DATA BUS WIRE) OF D4821P SHORTED TO GRND BETWEEN CONNECTOR AND RELAY BASE (SHORT TO SHIELD) OF K2066. THIS IS RED, DATA BUSS WIRE LOCATED BEHIND THE E2-SHELF. INSTALLED NEW TWISTED SHIELDED PAIR BETWEEN K2066 AND PLUG D4821P. SHORT COULD HAVE BEEN CAUSED BY RECENT MOD WORK IN THIS AREA. CHECKS OK. VERIFICATION FLIGHT TESTED. RETURNED TO ETOPS FLYING.

ENGINE REPLACED AT IAD ON 7/9/05. CURRENTLY EN ROUTE TO SFO FOR TEARDOWN.
DURING PACKING PROCEDURES, 6 BEARINGS BROKE WHEN TENSION PUT ON BAR. AIRFRAME MANUFACTURER IS AWARE OF THIS ISSUE AND IS EVALUATING CHANGE TO MORE DURABLE BEARING MATERIAL. IN THIS CASE, ALL OF THE BROKEN BEARINGS WERE REPLACED.

DURING CAT2 AUTOLAND, AT APPROX 20-30 FT, AIRCRAFT PITCHED UP AUTOMATICALLY, THEN AT 15 FT AUTOPILOT DISCONNECTED ITSELF. CAPTAIN TOOK OVER AND MANUALLY LANDED. THERE WAS A FAILURE OF THE RT AUTOPILOT RUDDER SERVO ON THIS AIRPLANE DURING A PREVIOUS FLT. AS A RESULT, THE RT RUDDER SERVO CIRCUIT BREAKER WAS PULLED AND COLLARED AND THE AC WAS IN A CAT2 STATUS WHEN THIS EVENT OCCURRED. THE MCDP REPORTED THE AUTOPILOT DISCONNECT WAS CAUSED BY A PITCH AXIS MODE ERROR. THIS EITHER CAN BE CAUSED BY THE PILOT OVERRIDING THE PITCH SERVO, OR THE ELEVATOR SURFACE POSITION DIFFERING FROM THE ELEVATOR SERVO POSITION BY MORE THAN FOUR DEGREES. THE RT RUDDER SERVO AND RT FCC WERE REPLACED AND THE AC WAS RESTORED TO CAT3.

DIVERTED MIA SUPPLEMENTAL DATA UPDATE RELAY K1043 AIR/GROUND WAS FOUND STUCK IN THE GROUND POSITION. WHEN RELAY WAS REMOVED AND REINSTALLED THE SYSTEM OPERATED NORMALLY. RELAY K1043 WAS REPLACED. RELAY K1188 AIR/GROUND IND SLAVE WAS REPLACED PRECAUTIONARY. THE AIRCRAFT DEVELOPED A DIFFERENT FAULT ONE WEEK LATER. THIS FAULT WAS CORRECTED BY REPLACING THE OVERBOARD EXHAUST VALVE.
BURNING PLASTIC SMELL AT T/O FOR 5 TO 10 MIN. CLEARED AFTER DESELECTING RECIRCS AND VENTING CABIN. ON DESCENT AND POWER REDUCTION TO IDLE AT 41000 HAD STRONG SMELL AND LIGHT BLUE HAZE THROUGHOUT CABIN AND COCKPIT. ALL LAV SMOKE ALARMS WENT OFF AND REPEATED SMOKE AND FUME CLEARING PROCEDURE WHILE DIVERTING MOST OF SMOKE CLEARED BY LANDING. FOUND NO ABNORMAL EICAS OR LIGHTS AT ANY TIME. *S/D* THE NUMBER 1 AND 2 BEARING CARBON SEAL FACES WERE FOUND WORN. THE SEALS WERE REPLACED AND THE ENGINE WAS RUN IN THE TEST CELL USING A FILTER SYSTEM ON THE BLEED AIR SUPPLY. NO OIL RESIDUE WAS DETECTED IN THE FILTER AFTER EXTENSIVE TESTING. THIS ENGINE HAS SUBSEQUENTLY OPERATED ON ANOTHER AIRCRAFT FOR APPROXIMATELY 6

ON CLIMBOUT, FORWARD CARGO WARNING LIGHT AND EICAS MESSAGE SHOWN. DIVERTED LHR. *S/D* ACCOMPLISHED DOOR SENSOR CHECKS PER MM AND FOUND SENSOR S215 OUT OF RANGE. RESHIMMED PER MM. ACCOMPLISHED PSEU BITE CHECK AND ALL CHECKS NORMAL.
WEATHER RADAR FAILED. RETURNED TO FIELD. *S/D* THE WEATHER RADAR MALFUNCTION COULD NOT BE DUPLICATED ON THE GROUND. SINCE THE RT UNIT HAD BEEN PREVIOUSLY REPLACED IT WAS DECIDED TO REPLACE THE ANTENNA DRIVE UNIT AND THE ANTENNA FLAT PLATE. A NON-REV VERIFICATION FLIGHT WAS PERFORMED AND THE SYSTEM OPERATION WAS NORMAL. AT THIS TIME THE REMOVED UNITS HAVE NOT BEEN RETURNED FOR SHOP TESTING TO DETERMINE WHICH UNIT WAS THE CAUSE OF THE MALFUNCTION.

NR 2 ENGINE SHUTDOWN IN FLIGHT FOR LOW OIL PRESSURE (79-89PSI). ENGINE WINDMILLED APPROXIMATELY 20 TO 30 MINUTES WITH POSITIVE PRESSURE. THE NUMBER THREE CHIP DETECTOR WAS FOUND MISINSTALLED. RE-INSTALLED CHIP DETECTOR AND LEAK CHECKED SYSTEM.
