DURING CRUISE FLIGHT PILOTS NOTICED #1 T-HANDLE FLICKERING, VERIFIED IT WAS A FALSE INDICATION AND DID NOT NEED TO UTILIZE ANY EMERGENCY PROCEDURES. FIRE LIGHT THEN EXTINGUISHED WITHIN 30 SECONDS. RAR AFT LOWER OUTBOARD FLAME DETECTOR. AIRCRAFT RETURNED TO SERVICE. DURING EVENING SHUTDOWN PRIOR TO SCHEDULED REMOVAL OF AFT XMSN(DUE FOR QUILL SHAFT INSPECTION) PILOT NOTICED THE NR DID NOT DROP LIKE NORMAL. GROUND MAINTENANCE VISUALLY SAW THE NR 2 ENG/MIX BOX WAS NOT DISCONNECTING IN A NORMAL MANNER. R&R AFT XMSN PER ORIGINALLY SCHEDULED MAINTENANCE ACTIVITY. SUPPLEMENTAL INFORMATION; THE CLUTCHES ASSOCIATED TO AFT TRANSMISSION, S/N TA-9-6 WERE SCRAPPED FROM SWO-036307-18 FOR FAILING THE KINETIC DRAG INSPECTION (REF. C07-5, CHAPTER 62-10-04, REV.4)
AFT ROTOR HUB WAS REMOVED. ON DISASSEMBLY IT WAS REVEALED THAT THE YELLOW LAG BEARING WAS GALLED.
WHILE PERFORMING A B3 INSPECTION, MAINTENANCE DISCOVERED A CRACK IN THE LT PYLON CAP ANGLE BS 410, WL -6,-10, AND -12. REPAIRED.
FIRE DETECTION WARNING LIGHT ILLUMINATED DURING FLIGHT (FLIGHT ILLUMINATION). THERE HAVE BEEN SEVERAL PREVIOUS OCCURRENCES (SEE SUBMITTED SDR'S). THE NR 2 AFT, INBOARD, FIRE DETECTOR WAS REPLACED AND DOCUMENTED ON LC-000973-75-16, DISC-000619-75-16. THERE HAVE BEEN NO FURTHER OCCURRENCES. SUPPLEMENTAL INFORMATION; THE DETECTORS IN THIS A/C HAVE BEEN REPLACED WITH P/N 477223 FLAME DETECTORS IAW COLUMBIA HELICOPTERS, INC. SERVICE BULLETIN NUMBER 107-26-S0004.
