Maintenance disclosures
DURING MAINTENANCE A CRACK WAS DISCOVERED ON THE RIGHT HAND CLOSEOUT RIB (P/N: 5122115-56) AT R.W.S. 106.79. THIS CRACK ORIGINATES FROM A HOLE IN THE FORWARD ROW OF FASTENERS COMMON TO THE AFT MAIN LANDING GEAR TRUNNION SUPPORT PLATE AND TRAVELS FORWARD AN APPROXIMATE LENGTH OF .625 INCHES. THE CRACK IS CONSISTENT WITH OTHERS VISUALLY IDENTIFIED IN THIS SAME AREA. THIS RIB WILL BE REPLACED.
DURING MAINTENANCE, A SPOT OF DAMAGE WAS VISUALLY IDENTIFIED ON THE RH FWD WING ATTACH FITTING (PN: 082550-46). THIS DAMAGE IS LOCATED ON THE TOP EDGE OF THE FITTING AS SEEN THROUGH THE WING GAP PANEL 611AT INBOARD OF RWS 58.94. THIS DAMAGE MEASURES APPROXIMATELY .987 LONG AND .080 DEEP. THIS FITTING HAS BEEN REMOVED AND REPLACED USING NEW PARTS.
DURING NDT INSPECTION AS NOTED IN SID 57-10-16, A CRACK WAS DISCOVERED ON THE RIGHT HAND OUTBOARD NACELLE FITTING. (P/N: 5292029-24) THIS CRACK MEASURES APPROXIMATELY .102 INCHES LONG ON THE OUTBOARD LOWER LEG OF THE FITTING, AND IS CONSISTENT WITH OTHER CRACKS FOUND USING THE LIGHTED BORESCOPE INSPECTION TECHNIQUE (TEXTRON MEL54-02R2) TO SATISFY REQUIREMENT OF AD2017-10-09. THIS FITTING WILL BE REPLACED USING NEW PARTS.
PILOT REPORTED MAGNETO ROUGH IN FLIGHT. MAINTENANCE INSPECTED AND FOUND OIL IN THE #5 CYLINDER. UPON REMOVAL OF THE CYLINDER IT WAS FOUND TO HAVE MISSING MATERIAL AT THE VALVE INTAKE GUIDE. THE #5 CYLINDER WAS REPLACED WITH NO FURTHER DEFECTS NOTED
PILOT REPORTED A ROUGH ENGINE; "MISSING" WHEN PROPS BROUGHT BACK FOR CRUISE AND STRONG VIBRATIONS ON FINAL APPROACH. MAINTENANCE INSPECTED AND FOUND THE LEFT ENGINE NR 4 CYLINDER EXHAUST VALVE GUIDE TO HAVE PLAY, THE EXHAUST VALVE CHIPPED, AND THE EXHAUST VALVE LIFTER FAILED. THE CYLINDER WAS REPLACED WITH NO FURTHER DEFECTS NOTED.
PILOT REPORTED THE LEFT ENGINE RUNNING ROUGH IN FLIGHT AND OIL PRESSURE WENT TO ZERO PSI. PRELIMINARY INSPECTION FOUND METAL CONTAMINATION IN OIL FILTER, #4 CYLINDER INTAKE PUSH ROD BROKEN AND A LARGE GAP AT THE EXHAUST ROCKER ARM. THE ENGINE IS BEING REPLACED AND SENT TO CMI FOR FURTHER ANALYSIS. FURTHER INFORMATION ON THIS EVENT CAN BE PROVIDED BY THE OEM.
PILOT REPORTED THE ENGINE SURGED TO A POINT OF UNCONTROLLABLE AFTER TAKE-OFF. THE ENGINE WAS INTENTIONALLY SHUT DOWN AND THE AIRCRAFT LANDED WITHOUT INCIDENT. MAINTENANCE TROUBLESHOT THE ISSUE. OPEN
DURING INSPECTION, LT ENG SN 1004575, IT WAS DISCOVERED THE UPPER THROUGH BOLT NR 1 MAIN BEARING SADDLE RT NUT FRACTURED, CAUSING THE THROUGH BOLT TO MIGRATE OUT OF POSITION.
PILOT REPORTED LT BRAKES INOPERATIVE. R & R THE LT BRAKE CALIPER & BLED BRAKES. OPERATIONAL & LEAK CHECKED GOOD.
AT SCHEDULED INSPECTION, NLG DRAG BRACE, PN 5142002-5 WAS FOUND TO BE CRACKED IN THE AREA OF NLG ACTUATOR ATTACH POINT. METHOD OF INSPECTION WAS DYE PENETRANT. THIS CRACK WAS FOUND TO BE BEYOND LIMIT ALLOWABLE OF MEB91-11R1. THIS UNIT HAS BEEN SCRAPPED & A REPLACEMENT DRAG BRACE HAS BEEN INSTALLED IN ACFT.

CREW REPORTED, RT BRAKE FAILED ON TAXI WHICH LEAD TO AFT PROCEEDING INTO GRASS OFF TAXIWAY. MX INSPECTED ACFT BRAKING SYS, SERVICED RT MASTER CYLINDER. ACFT RELEASED BACK TO SERVICE.
CREW REPORTED AVIONICS MASTER SWITCH INOP. MAINTENANCE REMOVED AND REPLACED CIRCUIT BREAKER SWITCH.
ON APPROACH, RT RUDDER PEDAL WENT TO FLOOR. APPROACH WAS CONTINUED, LANDING UNEVENTFUL. MAINTENANCE FOUND RUDDER PEDAL ATTACH BRACKET HAD FAILED. REMOVED AND REPLACE TORQUE TUBE AND RELEASED AIRCRAFT BACK TO SERVICE.
SWITCH FAILED, MAINTENANCE REPLACED SWITCH AND RETURNED THE AIRCRAFT TO SERVICE.
