APPLIED TEMP REPAIR IAW EMB AMM 56-10-01 PERMENANT REPAIR REQUIRED WITHIN 50FC TDMI # 68756042C ITEM ENTERED ONTO MIC SHEET. THE AIRCRAFT WAS RETURNED TO SERVICE.
ON FLIGHT 4155 THE CREW REPORTED THE RUDDER SYSTEM 1 AND 2 FAILED IN CLIMB. THE CREW DECLARED AN EMERGENCY AND DIVERTED TO CMH. THE AIRCRAFT LANDED WITHOUT INCIDENT. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED THE RUDDER SYSTEM 2 PRESSURE SWITCH PER AMM 27-23-04. THE OPERATIONAL CHECK WAS GOOD IAW AMM 27-23-00. NO DEFECTS NOTED. MAINTENANCE AND CREW SUSPECT "SMOKE" WAS CAUSED BY DEICING FLUID RESIDUE EVAPORATING WHEN NORMAL LANDING PROCESS HEAT WAS APPLIED. THE AIRCRAFT WAS RETURNED TO SERVICE.

THE AIRCRAFT WAS RETURNED TO SERVICE.

PERFORMED ROLLS-ROYCE FIM TASK 72-00-00-810-812, FOUND OIL POOLING IN THE LPT, WHICH REQUIRES ENGINE CHANGE. REMOVED AND REPLACED THE NR 1 ENGINE IAW W/C 71-000-00. THE OPERATIONAL CHECK WAS GOOD. THE AIRCRAFT WAS RETURNED TO SERVICE.
ON FLIGHT 4165 THE CREW REPORTED THE MAIN PASSENGER DOOR EMERGENCY EXIT SIGN WAS INOPERABLE. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED THE EXIT MARKER IAW AMM 33-50-14. THE OPERATIONAL CHECK WAS GOOD. THE AIRCRAFT WAS RETURNED TO SERVICE.

THE AIRCRAFT WAS RETURNED TO SERVICE.
ON FLIGHT 2888 DURING CRUISE THE CREW REPORTED HYDRAULIC SYSTEM NR 2 SLOWLY LOST FLUID UNTIL FAILING. THE CREW FOLLOWED THE AIRCRAFT OPERATING MANUAL AND DECLARED AN EMERGENCY. THE AIRCRAFT LANDED WITHOUT INCIDENT. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED THE NR 2 HYDRAULIC PUMP IAW EMB 145 AMM 29-AMM 29-10-03. THE OPERATIONAL CHECK WAS GOOD. NO LEAKS NOTED AT THIS TIME. THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING MAINTENANCE BASE VISIT FOUND THE RIGHT OVERWING EMERGENCY LIGHT TO BE INOPERABLE. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED THE RIGHT OVERWING EMERGENCY LAMP. THE AIRCRAFT WAS RETURNED TO SERVICE.
ON FLIGHT 4088 DURING TAKEOFF THE CREW REPORTED THE WEATHER RADAR FAILED. THE CAPTAIN ELECTED TO RETURN TO ORD. OVERWEIGHT LANDING WEIGHT AT TOUCHDOWN WAS 45,500 LBS. IT WAS A NORMAL LANDING VERTICAL SPEED AT TOUCHDOWN AND WAS LESS THAN 100 FEET PER MINUTE. THE AIRCRAFT WAS REMOVED FROM SERVICE. FOUND TO BE WITHIN ALLOWABLE LIMITS, AND DID NOT EXCEED SINK RATE PER MTX WC 05-200-01. THE AIRCRAFT DID NOT REQUIRE AN OVERWEIGHT LANDING INSPECTION. REMOVED AND REPLACED THE WEATHER RADAR TRANSMITTER IAW EMB-145 AMM 34-42-01. THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING MAINTENANCE BASE VISIT FOUND THE RIGHT WING EMERGENCY EXIT LIGHT TO BE INOPERABLE. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED THE LAMP (BULB). THE OPERATIONAL CHECK WAS IN ACCORDANCE WITH EMBRAER AMM 33-50-07. THE AIRCRAFT WAS RETURNED TO SERVICE.
FOR FLIGHT 3679 PRIOR TO FLIGHT THE CREW DETERMINED THAT THE MAIN CABIN DOOR WAS NOT CLOSING PROPERLY. THE AIRCRAFT WAS REMOVED FROM SERVICE. LUBRICATED THE MAIN DOOR. THE OPERATIONAL CHECKS WERE GOOD IAW AMM 52-10-00. THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING MAINTENANCE BASE VISIT FOUND THE REAR FUSELAGE DENTED SKIN BETWEEN FRAMES 63 AND 64 AND BETWEEN STRINGER 20 LEFT AND 20 RIGHT. THE AIRCRAFT WAS REMOVED FROM SERVICE. REPAIRED REAR FUSELAGE DENTED SKIN BETWEEN FRAME 63 AND 64 AND BETWEEN STRINGER 20 LEFT AND 20 RIGHT IAW EO EM-53MR-1497. THE AIRCRAFT WAS RETURNED TO SERVICE.

ON FLIGHT 4468, THE CREW REPORTED AN OIL SMELL IN THE COCKPIT MOST OF THE FLIGHT. THE FLIGHT ATTENDANT DID NOT NOTICE ANY SMELLS IN THE CABIN. AIRCRAFT REMOVED FROM SERVICE. REMOVED AND REPLACED LEFT ENGINE NR 1 IAW 71-000-00. OPERATIONAL CHECK GOOD.
WHILE INVESTIGATING SCRATCHES ON THE NUMBER ONE AILERON MAINTENANCE DISCOVERED THAT THE NUMBER ONE AILERON DAMPENER TO AILERON ATTACHMENT FITTING BOLT WAS NOT ATTACHED TO THE AILERON. IT WAS LATER DISCOVERED THAT THE NUMBER ONE AILERON DAMPENER BOLT WAS SHEARED AND THE REMAINING BOLT SHANK WAS FOUND STILL STUCK IN THE DAMPENER BOLT HOLE. IT WAS ALSO FOUND THAT THE AILERON DAMPENER ATTACH FITTING WAS GOUGED AND DAMAGED ALONG WITH THE AILERON SHROUD. AIRCRAFT REMOVED FROM SERVICE. REMOVED AND REPLACED THE NUMBER ONE DAMPENER BOLT AND AILERON SHROUD. THE AILERON DAMPENER ATTACH FITTING WAS PASSED. THE REPAIR WAS DIFFERED UNTIL THE NEXT "C" CHECK. AIRCRAFT RETURNED TO SERVICE.
