Maintenance disclosures
ON APPROACH TO KLAN, THE PILOT REPORTED THAT HE SELECTED THE LANDING GEAR HANDLE TO THE DOWN POSITION, BUT THE LANDING GEAR FAILED TO EXTEND. HE THEN RESET THE LANDING GEAR RELAY CIRCUIT BREAKER AND THE GEAR CAME DOWN NORMALLY. LAN MAINTENANCE REMOVED AND REPLACED THE LANDING GEAR POWER RELAY AND THE LANDING GEAR TIME DELAY RELAY. SEVERAL GEAR EXTENSION AND RETRACTION CHECKS WERE PERFORMED WITH SUCCESS AND THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING APPROACH INTO KPLN, THE FLIGHT CREW HAD TO MANUALLY EXTEND THE LANDING GEAR. LAN MAINTENANCE INSPECTED THE LANDING GEAR RELAY AND WEIGHT ON WHEELS SWITCH/ WIRING AND COULD NOT DUPLICATE THE FAILURE. MAINTENANCE SERVICED THE MANUAL EXTENSION RESERVOIR AND RETURNED THE AIRCRAFT TO SERVICE AFTER MULTIPLE SUCCESSFUL GEAR EXTENSION AND RETRACTION OPERATIONAL CHECKS.
PILOT REPORTED THAT ON LANDING, THE GEAR DOWN SELECTION FAILED WHEN ACTUATED. HE THEN EXERCISED THE EMERGENCY LANDING GEAR HANDLE AND LANDED WITHOUT FURTHER INCIDENT. MAINTENANCE REPLACED THE HYDRAULIC RESERVOIR CAP AND SEAL AND THE LANDING GEAR MOTOR. THE AIRCRAFT PERFORMED A SUCCESSFUL OPERATIONAL CHECK FLIGHT BEFORE IT WAS RETURNED TO SERVICE.
PIC REPORTED THE LANDING GEAR WOULD NOT EXTEND ON APPROACH TO KESC. EMERGENCY PROCEDURES WERE FOLLOWED, AND THE AIRCRAFT LANDED SAFELY. MAINTENANCE FOUND THE LANDING GEAR HYDRAULIC SYSTEM LOW ON FLUID. THE HYDRAULIC SYSTEM WAS SERVICED IAW C99 MM 32-31-00. NO OTHER DEFECTS OR LEAKS WERE NOTED. MAINTENANCE PERFORMED EXTENSION AND RETRACTION CHECKS WITH SUCCESS; THE AIRCRAFT WAS RETURNED TO SERVICE.
WHILE EN ROUTE TO KESC, THE FLIGHT CREW REPORTED A SMALL FLAME AND SMOKE EMANATING FROM THE FLAP POSITION INDICATOR. BACKLIGHTS WERE TURNED OFF. THE FLAME AND SMOKE DISSIPATED. THE AIRCRAFT LANDED WITHOUT INCIDENT IN KESC. MAINTENANCE TROUBLESHOT SYSTEM TO A FAULTY FLAP POSITION INDICATOR BACKLIGHT PANEL ASSEMBLY. DISCONNECTED AND STOWED CONNECTOR, DEFERRING IAW MEL 33-01. THE PANEL WILL NEED TO BE REPLACED WHEN RECEIVED, THIS SDR WILL BE UPDATED WHEN THE DISCREPANCY HAS BEEN CLEARED.
THIS IS A SUPPLEMENT TO SDR 2021FA0000432. THE DIFFICULTY DATE ON THE ORIGINAL SUBMISSION WAS IMPROPERLY REPORTED AND HAS BEEN CORRECTED ON THIS SUPPLEMENT TO REFLECT 12/27/2021. MAINTENANCE REMOVED AND REPLACED THE BACKLIGHTING SUB-PANEL ASSEMBLY AND SYSTEM OPS CHECKED GOOD.
ON FINAL APPROACH AFTER THE PILOT SELECTED THE LG TO THE DOWN POSITION AN ELEC SMELL WAS REPORTED. THEY LANDED WITHOUT INCIDENT & DURING TAXI TO PARKING THE SMELL BECAME STRONGER & SMOKE BEGAN TO FILL THE COCKPIT. THE PILOT STOPPED THE ACFT AT THE PARKING RAMP, TURNED OFF ALL ELEC POWER & EXITED THE ACFT. ONCE THE POWER WAS TURNED OFF THE SMOKE DISSIPATED. FOUND THE LG MOTOR POWER RELAY WAS SHORTED INTERNALLY ALLOWING THE MOTOR TO RUN CONTINUOUSLY, CAUSING THE SMOKE. REPLACED THE RELAY, PN MS24171-D1, AS A PRECAUTION REPLACED THE LG MOTOR & CURRENT LIMITER.
ON APPROACH, THE PILOT SELECTED THE LG HANDLE TO THE EXTEND POSITION, THE LG WOULD NOT EXTEND & THE TIME DELAY RELAY CB POPPED. THE PILOT TRIED TO RESET THE CB WITH THE SAME RESULTS. THE PILOT PERFORMED A MANUAL EXTENSION OF THE LG & RECEIVED A THREE GREEN DOWN & LOCKED INDICATION. THE PILOT DIVERTED & LANDED WITHOUT INCIDENT. TROUBLESHOOTING REVEALED A FAULTY HYD POWER PACK ELEC MOTOR. REPLACED THE HYD POWER PACK ELEC MOTOR & PERFORMED OPERATIONAL CHECKS. ALL CHECKS WERE NORMAL.
