R/H EMERGENCY EXIT & PAX EMERGENCY LIGHTS L996,L321,L121 & L776 INOP.
CORROSION DAMAGE HAS BEEN FOUND ON THE REAR SPAR LOWER CHORD OF THE INBOARD TRAILING EDGE FLAP ON THE RH WING. THE DAMAGE HAS BEEN CUT OUT TO REMOVE THE DAMAGED SECTION OF THE 2024-T3511 EXTRUDED TEE AND REPLACED WITH AN EQUIVALENT 2024-T3 EXTRUSION. THE REPAIR MEMBER IS SPLICED TO THE EXISTING LOWER CHORD USING 2024-T3 EXTRUDED ANGLES AT EACH END IN ACCORDANCE WITH SEMAN PERU SAC REPAIR DRAWING ED-2025-0005 (REF. 7).

FLIGHT FROM AFW TO YYZ, AFTER LANDING MECHANIC DURING POST FLIGHT INSPECTION DISCOVERED THAT THE S51MT (ATA 57 - INBOARD WING TRAINING EDGE) PANEL HAD PARTIALLY DEPARTED THE AIRCRAFT. APPEARS THE PANEL FASTENERS WERE NOT PROPERLY INSTALLED.
MID GALLEY FIRE SMOKE DETECTOR SMOKE ALARMS GO OFF DURING DESCENT SAYS "FAULT"
LEFT ENGINE FIRE (WARNING EICAS) INDICATION ON CLIMB OUT. PILOT COMMANDED ENGINE SHUTDOWN FOLLOWING QRH. NO ENGINE FIRE BOTTLE UTILIZED. FIRE WARNING LIGHTS EXTINGUISHED AFTER THE FIRE T- HANDEL WAS ACTIVATED. UNEVENTFUL LANDING - NO EMERGENCY EGRESS CREW ONLY NO PASSENGERS. RB211-524HT-36 ESN 13264.

DURING WALK AROUND FOUND 1 BLOCKER DOOR DAMAGE LEFT ENGINE THRUST REVERSERS. REV INSPECTION AND ENGINE CONFIRMATION OF CONTINUED SERVICE. SUPPLIMENTAL . REMOVED AND REPLACED L/H ENGINE CORONA AND #3 BLOCKER DOOR IAW AMM 78-31-06 AND ESM 78-32-00-440-043/044. OPS CHECK GOOD.COMPONENT REPLACEMENT BLOCKER DOORS (P/N J314179 POSITION #3 S/N TVL BW4913) AND CORONA (P/N LJ37755 AND S/N AA46).
DURING WALK-AROUND FOUND 1 BLOCKER DOOR DAMAGE LEFT ENGINE TRUST REVERSERS. REV INSPECTION AND ENGINEERING CONFIRMATION OF CONTINUED SERVICE. CONFIGURATION DEVIATION LISTING APPLIED AND NEW PARTS AND HARDWARE ORDERED.
FLIGHT DECK EMERGENCY LIGHT INOP. FLIGHT DECK EMERGENCY LIGHT BATTERY REPLACED.
THE R2 AND R4 EMERGENCY LIGHT WILL NOT ILLUMINATE. MAINTENANCE MECHANIC REMOVED AND RPLACED THE POWER SUPPLY BATTERY PACK M733 PER AMM 33-51-21. ALSO RELIGHTED THE R4 EMERGENCY LIGHT LAMP SOCKET. OPS CHECK GOOD.
THE R4 EMERGENCY LIGHT WILL NOT ILLUMINATE. MAINTENANCE MECHANIC REMOVED AND RPLACED R4 SIDE EMERGENCY LIGHT THAT WAS FOUND LOOSE IN ADDITION TO M733 POWER BATTERY PACK PER AMM 33-51-21. OPS CHECK GOOD.
DURING MAINTENANCE CHECKS THE MECHANIC FOUND L1 AND R1 EXIT EMERGENCY INTERIOR LIGHTS INOP. MAINTENANCE REPLACED BATTERY PACK ON L1 & R1 ON INTERIOR LIGHTS. OPS CHECK FOUND TO BE IAW AMM 33-51-07.
DURING MAINTENANCE CHECKS THE MECHANIC FOUND L5 AND R3 EXIT EMERGENCY INTERIOR LIGHTS INOP. AIRPLANE CHARGED EMERGENCY BATTERY L3 AND R3 EXIT INTERIOR LIGHT, OPS CHECK IAW AMM 33-51-00 TO BE NORMAL.
DURING A ROUTINE MAINTENANCE THE MECHANIC FOUND L1,L3 AND L4 EXTERIOR EMERGENCY LIGHT INOP. MAINTENANCE REMOVED AND REPLACED M738 BATTERY PACKS. OPS CHECK IS NORMAL AND IAW AMM 33-15-66.
DURING WALK AROUND IT WAS NOTED THE LEFT ENGINE OUTBOARD NACELLE STRUT DOOR WAS MISSING. THE FME DURING THE WALK AROUND NOTED THE MISSING DOOR IN THE LOG PAGE (LEFT ENGINE NACELLE DOOR MISSING). THIS WAS PLACED ON CDL ISSUED AND ISSUED AN MRN. MAINTENANCE MECHANIC RE-SECURED THE SENSOR TARGET BRAKE WITH NEW RIVETS. A/C WAS RETURN TO SERVICE.
DURING INSPECTION N700KW WAS IN HEAVY CHECK. C-CHECK MAINTENANCE IT WAS DISCOVERED THAT THE EMERGENCY EXIT LIGHT TEST CONTROL SWITCH WAS FOUND DEFECTIVE. THE CORRECTIVE ACTION TAKEN WAS TO REMOVE AND REPLACED THE CONTROL SWITCH ON P40 EMERGENCY EXIT LIGHT TEST, IAW 0767 SWPM 20-83-11 PAGE 13. PERFORMED OPERATIONS CHECK RESULTS WERE GOOD, IAW B767 AMM 33-51-06 PAGE 202.
EMERGENCY LIGHTS AT THE L4, R4, AND L1 DOORS. REMOVED AND REPLACED POWER SUPPLY (M736)IAW B767 AMM 33-51-01 OPS CHECK FOUND OK AND REMOVED AND REPLACED AFT R AND L DOOR SLIDE EMERGENCY 1T LAMP IAW B767 AMM 33-51-06 OPS CHECK OK
#2 ENGINE FIRE INDICATION ON THE GROUND, AT IDLE, PRIOR TO TAKE OFF. QRH WAS FOLLOWED AND FIRE BOTTLE #1 DISCHARGED. INDICATION REMAINED SO FIRE BOTTLE #2 WAS DISCHARGED AND EMERGENCY DECLARED. AIRPORT FIRE RESCUE RESPONDED AND FOUND NO INDICATION OF FIRE. AIRCRAFT RETURNED BACK TO THE GATE. MAINTENANCE FOUND NO INDICATION OF FIRE. REMOVED AND REPLACED ZONE 2 FIRE DETECTOR ASSY (FIRELOOP) I/A/W B767 AMM 26-11-02. CONDUCTED AN OPERATION CHECK I/A/W B767 AMM 26-11-00. OPS CHECK GOOD. ALSO REMOVED AND REPLACED #2 ENGINE FIRE BOTTLE AND CARTRIDGE #1 AND FIRE BOTTLE AND CARTRIDGE #2 I/A/W B767 AMM 26-21-03. OPS CHECK GOOD.
DURING 28 DAY CHECK FOUND R/H EXIT SIGN LIGHT INOP. REMOVED AND REPLACED R/H AFT BATTERY PACK IAW B767 AMM 33-51-07. OPS CHECK NORMAL. NOTE: AIRCRAFT WAS IN MAINTENANCE CHECK AT THE TIME THE ISSUE WAS FOUND. FAA PMI HAS BEEN NOTIFIED.
LOST OIL PRESSURE AND QUANTITY TO THE POINT WE HAD TO SHUTDOWN IN FLIGHT AND DIVERTED TO ALTERNATE DESTINATION. FOUND BOLTS FOR DEDICATED GENERATOR BACKED OUT AND HELICOILS ALSO BACKED OUT. REPLACED HELICOILS AS REQUIRED AND REINSTALLED GENERATOR
LEFT ENGINE HOT START 510 DEGREES WITH ONLY 17% N2 FUEL FLOW NORMAL N3 MAXIMUM AT 23%. TROUBLESHOT IAW FIM 71-05 F//T 808 AND AMM 71-00-00.
