RECEIVED A LT ENG VIBRATION CAS FOLLOWED BY A LT ENG FLAMEOUT CASS. VIBRATION DECREASED BUT NEVER WENT AWAY. REMOVED UNSERVICEABLE LT ENGINE, IAW AMM 71-00-01-000-801. INSTALL CUSTOMER PROVIDED REPAIRED ENGINE INSTALLED ON REPLACEMENT LT ENGINE. ALL POST OPS CHECK COMPLETED OPS CHKD GOOD.
CABIN ODOR IN CABIN. "SOCKS OR WET DOG" SMELL. SEEMS MORE PRONOUNCED AT SEA LEVEL, DENSE AIR. TRANSFERRING TO APU SEEMS TO INCREASE THE SMELL BUT IT MAY JUST BE THE HIGHER FLOW OF AIR. REMOVED AND REPLACED ACM DUE TO TORQUE CHECKS FAIL . OPS/LEAK CHECKS REQUIRED. ACM OPS/LEAK CHECKED GOOD. ALL MX COMPLETED IAW AMM CHAPTER 21-52-13.
RIGHT AND LEFT ENGINES - ENGINE OIL PRESSURE: DURING CRUISE AT FL370 THE RIGHT ENGINE OIL PRESS LOW CAS MESSAGE ILLUMINATED ALOMG WITH RED LOW OIL PRESSURE.//CORRECTIVE ACTION//FOUND OIL CAP NOT SECURED, SECURED CAP. PERFORMRMED ENGINE RUNS WITH NO FURTHER LEAKS OR DEFECTS NOTED
AT FLIGHT LEVEL 210 AND AT 345 KNOTS, THE ACFT HAD A BUZZING SOUND & AIRFRAME VIBRATION FELT IN CONTROLS. SHORTLY AFTER HAD A VIOLENT UNCOMMANDED RUDDER LEFT WITH NO PILOT INPUT. THE ACFT JERKED HARD FOR A FEW SECONDS. PERFORMED A NORMAL LANDING. PERFORMED THOROUGH BORESCOPE INSPECTION OF NR 1 & 2 ENGINES WITH NO SIGNIFICANT FINDINGS. PERFORMED FIVE POINT ENGINE CALIBRATION RUNS OF BOTH ENGINES WITH ALL PARAMETERS TO BE WITHIN LIMITS. PERFORMED OPS CHECK OF RUDDER BIAS SYSTEM WITH NO DEFECTS NOTED. INSTALLED P2.8 MOISTURE TRAP AND P2.8 TRANSDUCER LINE WITH NO DEFECTS NOTED. SWAPPED P3 TRANSDUCERS FOR FURTHER OBSERVATION. ALL WORK PERFORMED IAW EMM 75-30-03, 753009, 72-00-00 & AMM 27-20-35.
ELEVATOR FEEL FAIL ILLUMINATED TWO TIMES DURING FLIGHT. REMAINING ON FOR 30-60 SECONDS EACH TIME. R & R Q FEEL SERVO ACTUATOR IAW CMP 273014. PERFORMED STALL PROTECTION AND Q FEEL ACTUATOR ADJUSTMENT TEST IAW CMP 273030. NO DEFECTS NOTED.
HEAVY SULFUR ODOR IN COCKPIT WITH APU OPERATING WITH AVIONICS ON. ODOR IS NOT ASSOCIATED WITH ENVIRONMENTAL SYS. NO VISIBILE SMOKE WAS NOTICED. FOUND "PROBE HEAT" SWITCH IN OVERRIDE POSITION. THIS SETTING DELIVERS FULL POWER TO ALL PROBES AND TAKES THE AOA'S FROM LOW TO FULL POWER. OVERHEATING OF THE PROBES WAS CAUSING THE ODOR. INSPECTED PROBES AND FOUND NO DEFECTS. PLACED SWITCH IN AUTO POSITION, ODOR DID NOT RETURN. WORK DONE IAW AMM CH - 30-30-00.
BLEED PRESSURE AND TEMP HIGH WARNING MESSAGE ILLUMINATED ON TAKEOFF ROLL. PERFORMED ENGINE RUNS FOR OPS CHECK, WITH ECS SYS ON APU, RAN LT ENGINE & RT ENGINE NOTING NO MESSAGES, INSPECTED PRECOOLERS FOR DAMAGE AND BLOCKAGE NOTING NO FAULTS, INSPECTED PRECOOLER BY PASS VALVE INDICATOR WITH NO FAULTS NOTED. INSPECTED PRECOOLER TEMP SENSOR NOTING NO FAULTS, REF MM 21.00-00.
ABOUT 45 MINUTES INTO THE FLIGHT, CLIMBING THRU FL 360, CREW HEARD A LOUD "BANG", FELT A VIBRATION, ITT BEGAN AN IMMEDIATE CLIMB, FOLLOWED BY THE "FIRE" LIGHT AND SMOKE IN THE CABIN. CREW BROUGHT THE THROTTLE TO IDLE, SHUT THE ENGINE DOWN VIA THE FUEL CUTOFF SWITCH, DECLARED AN EMERGENCY AND WAS VECTORED BY ATC TO THE NEAREST SUITABLE AIRPORT.
RT ENGINE SHUTDOWN INFLIGHT DUE TO VIBRATION DETECTOR AT MAX LEVEL WITH CORRESPONDING RISING HIGH ITT LEVEL (NOT IN RED) AND ENGINE FIRE CAS MESSAGE. THERE WAS ALSO THE SMELL OF SMOKE IN THE CABIN BUT NOT VISIBLE. ENGINE WAS SECURED BY POWER TO IDLE AND FUEL CUTOFF SWITCH. THE ENGINE DID NOT AUTO SHUTDOWN. R & R RT ENGINE WITH RENTAL UNIT. PERFORMED OPS CHECKS AND LEAK CHECKS WITH NO DEFECTS NOTED, REF MM 71-00-00.
EMERGENCY BATTERY OVERHEATED IN FLIGHT; TEMP WENT TO 170 DEGREES F, VOLTS REMAINED AT 26.7. R & R EMERGENCY BATTERY WITH O/H UNIT MM 24-30-20, CMP 243019. OPS CHECKS GOOD.
NO AIRSPEED ON TAKEOFF ROLL, ABORTED T/O. VISUALLY INSPECTED PITOT TUBES AND LINES. PERFORMED RECONNECT CHECK ON PILOT COPILOT & STANDBY PITOT/STATIC SYSTEM'S REF MM 34-11-00 PGS 602-606 SYSTEM OPS CHECK GOOD AND LEAK CHECK GOOD.
DURING LANDING AND AFTER TOUCHDOWN, THE AIRCRAFT DEPARTED THE LT SIDE OF THE RUNWAY. DAMAGE NOTED ON THE LOWER IB SECTION OF THE RT WING, DAMAGE NOTED ON THE RT WING IB LEADING EDGE, AND DAMAGE NOTED ON THE RT KRUEGER FLAP.
ACRID ELECTRICAL BURNING SMELL EMINATING FROM BEHIND LT PILOT SEAT IN COCKPIT. CHECKED ALL CB'S, NOTHING FOUND (POPPED). [4-27-07] PERFORMED ENG RUN AND APCS RUNS FOR COCKPIT SMELLS, RUNS SATISFACTORY. ACCESSED L/R RELAY PANEL FOUND SATISFACTORY. ACCESSED FWD EQUIPMENT CLOSET CB CHR105& CHR203 POPPED. RESEAT OPS CK 115AC OUTLET GOOD. .RE-RACK AIRSHOW COMPUTER. OPS CK GOOD. ACCESSED L/H PILOT WINDSHIELD HEAT T/B. AT OVERHEAD FOUND SATISFACTORY. OPS CHECK DOOR MOTOR GOOD. COULD NOT DUPLICATE BURNING ODOR FOUND AIRCRAFT SATISFACTORY FOR FLIGHT WORK DONE IAW G2000 M/M CH 5-00-00 AND CH 24-00-00.
WE WERE LEVEL AT FLIGHT LEVEL 350 WHEN THE (APU FIRE LIGHT) AND BELL CAME ON. WE FIRED THE BOTTLE AND THE FIRE LIGHT STAYED ON. REMOVED AND REPLACED RT FIRE BOTTLE IAW MM 26-20-05. FIRE LOOP FOUND BAD AND REPLACED. APU FIRE DETECTION SYSTEM WAS OPS CHECKED AND CHECKED GOOD.
