DURING A SCHEDULED A-CHECK VISIT, DAMAGE WAS FOUND TO THE #4 ENGINE OUTBOARD THRUST REVERSER. FURTHER INSPECTION REVEALED THE #4 OUTER FAN EXIT LINER SEGMENT HAD DETACHED FROM THE FAN CASE RING AND DEPARTED THE AIRCRAFT. SUBSEQUENT C-DUCT DAMAGE WAS REPAIRED AND THE MISSING LINER SEGMENT WAS REPLACED PER THE MANUFACTURERS SRM & AMM INSTRUCTIONS.
UPPER DECK LEFT EMERGENCY EXIT DOOR ASSIST BOTTLE BATTERY WILL NOT TEST - REMOVED AND REPLACED BATTERY PER AIRCRAFT MAINTENANCE MANUAL 52-23-14.
EMER LTS L1854, L1853 AND L5735 INOP. REPLACED BATTERY PACK ON CHARGER M5170 IAW AMM 33-5104, TEST SATIS.
FLAPS AT 1, THEN FLAPS 5 SELECTED IN TURBULENCE - AT +20 KNOTS SPEED EXCURSION WAS NOTED. BEFORE FLAPS EXTENDED TO 5, SPEED WAS TRANSIENT. NO EICAS OR AUDIBLE WARNINGS OCCURRED. FLAPS OVERSPEED PHASE 1 CONDITIONAL INSPECTION PERFORMED ON FLAP SYSTEM PER AMM 05-51-04 WITH NO DEFECTS NOTED.
REFERENCE AML 54897-3: DURING DAILY CHECK, FOUND L/H UPPER DECK DOOR INDICATOR LIGHT WOULD NOT ILLUMINATE DURING FUNCTIONAL CHECK. REPLACED EMERGENCY LIGHTS BATTERY PACK FOR L/H UPPER DECK DOOR PER B747 AMM 52-23-14.
L/H UPPER DECK DOORWAY EMERGENCY LIGHTING BATTERY PACK FOUND ON THE FLOOR AFTER LANDING. REINSTALLED BATTERY PACK AS REQUIRED - BATTERY PACK INOPERATIVE. PLACED ON DEFERRAL PER B747 MEL 52-23-02-01. ON 01/25/2023, BATTERY PACKS FOR LEFT AND RIGHT DOORS SWAPPED WITH BOTH SYSTEMS WORKING NORMALLY. DMI CLOSED.
DURING REMOVAL OF LT BODY LANDING GEAR FOUND A WASHER INSTALLED BETWEEN THE OUTBOARD TRUNNION SUPPORT AND BEARING CAP WASHER CAUSED DAMAGE ON BOTH SIDE.

FOUND DURING CLEANING AND INSPECTION ON L/H BLG INBOARD TRUNNION SUPPORT FITTING ALL 4 BOLT HOLES WITH SURFACE DAMAGE.
DURING REMOVAL OF NOSE LANDING GEAR FOUND ON LT FITTING DISCOVERED PARTS OF THE BUSHINGS BETWEEN THE BEARING CAP AND FITTING.
DURING LDG REPLACEMENT, REMOVAL, MULTIPLE DAMAGED ON THE NLG AND BLG WHICH PROBABLY CAUSED BY BAD MAINTENANCE DURING THE INSTALLATION FROM A PREVIOUS MRO. ITEM IS ALSO REPORTED TO EASA UNDER REPORT ID OR-0000000000093739. DURING REMOVAL OF THE L/H BODY LANDING GEAR FOUND ON OUTBOARD BEARING CAP A WASHER THAT WAS CLAMP BETWEEN THE BEARING CAP AND THE OUTBOARD TRUNNION SUPPORT. WASH WAS THERE IN SINCE THE LAST GEAR CHANGE. MINOR DAMAGE ON BOTH SIDES DUE TO THE PRESSURE BETWEEN BOTH PART´S. THE SHAPE OF THE WASHER WAS STILL VISIBLE AFTER REMOVING THE WASHER. NATIONAL AIRLINES MADE SERVICE REQUEST TO BOEING. SEE BOEING MSG: MUA-MUA-22-0090-04B. BOTH BEARINGS (INBOARD & OUTBOARD) WERE NOT SEALED ACC TO AMM 32-13-01-424-083. FOUND DURING CLEANING AND INSPECTION OF L/H BODY LANDING GEAR INBOARD TRUNNION SUPPORT ON BOLT HOLES A THREAD CUTTED INTO THE MATERIAL AND ON ONE HOLE A DEEP CUT (OUTBOARD AFT POSITION). CAUSED BY WRONG INSTALLATION OF THE BOLT DURING THE LAST GEAR INSTALLATION. BOEING CONTACTED, WAITING FOR RESPOND. AFTER REMOVAL OF THE L/H WING BODY GEAR, FOUND DAMAGE ON THE RETRACT ACTUATOR INBOARD SIDE OF THE BEARING ON THE PISTON END OF THE RETRACT ACTUATOR. ACTUATOR WILL BE REPLACED. FOUND DURING CLEANING AND INSPECTION OF R/H BODY LANDING GEAR INBOARD TRUNNION SUPPORT ON BOTH AFT BOLT HOLES A THREAD CUTTED INTO THE MATERIAL. CAUSED BY WRONG INSTALLATION OF THE BOLT DURING THE LAST GEAR INSTALLATION. BOEING CONTACTED, WAITING FOR RESPOND. DURING REMOVAL OF THE LEFT WING GEAR FOUND A DAMAGED HYD. LINE CAUSED BY THE INSTALLATION OF THE TRUNNION FORK DURING THE LAST GEAR CHANGE. DURING REMOVAL OF THE NOSE GEAR FOUND AT THE L/H TRUNNION FITTING PIECES OF THE BUSHING HALFâS BETWEEN THE TRUNNION BUSHING CAP AND THE BODY SUPPORT FITTING ON TWO POSITIONS WITCH CAUSED DEEP DAMAGES. FOUND ALSO THAT THE BEARING HALF ON BOTH SIDES WERE NOT SEALED ON THE EDGES AND THERE WAS NO SEALER BETWEEN THE TRUNNION BUSHING FITTING. REF AMM SUBTASK 32-21-01-434-024. APPLY A 0.02 IN.THICK LAYER OF THE SEALANT TO THE INNER DIAMETER SURFACES OF THE SUPPORT FITTING. APPLY A 0.02 IN. THICK LAYER OF THE SEALANT TO THE INNER DIAMETER SURFACES OF THE CAP. APPLY A 0.02 IN. LAYER OF THE SEALANT TO THE OUTER DIAMETER SURFACES OF THE BUSHING ASSEMBLY. APPLY MORE SEALANT TO THE EDGES OF THE SUPPORT FITTING, THE CAP, AND THE BUSHING ASSEMBLY. APPLY A CONTINUOUS FILLET SEAL OF SEALANT BETWEEN THE INBOARD AND OUTBOARD BUSHING FLANGE AND THE CAP SURFACE. APPLY A CONTINUOUS FILLET SEAL OF SEALANT BETWEEN THE INBOARD AND OUTBOARD BUSHING FLANGE AND THE SUPPORT FITTING SURFACE. ON BOTH LOWER BRAS BUSHING HALFâS WERE BIG PIECES OF THE EDGES BROKEN OFF AND A CRACK WAS ALSO FOUND AS ALSO THE INSIDE PART OF THE GREASE POINT WAS DAMAGED.

THE CATEGORY B REPAIR WAS ACCOMPLISHED IN ACCORDANCE WITH HAECO XIAMEN'S ENGINEERING NOTES EB-53-2861 REV IR, WITH REFERENCE TO BOEING SR 3-5365651623 & 3-5373949927, WITH DEVIATIONS TO SB AND AD 2013-19-15 APPROVED BY FAA FORM 8100-9 DATED ON AUGUST 8, 2022. DETAILS OF WORK WAS DOCUMENTED IN HAECO XIAMEN'S NRC WKL4563.
DURING HEAVY MAINTENANCE VISIT, AIRCRAFT INSPECTION REVEALED WATER/OIL INTRUSION IN THE LEFT / AFT CORNER OF ACCESS PANEL #193R (P/N 65B13883-1) FOR THE AFT MAIN LANDING GEAR DOOR SUPPORT STRUCTURE, LOCATED AT APPROXIMATE STATION 1405.3 TO 1437.09, CAUSING DELAMINATION DAMAGE. THE PERMANENT REPAIR WAS ACCOMPLISHED IN ACCORDANCE WITH FLIGHT VEHICLES CONSULTING, LNC'S ENGINEERING ORDER 22125-3 REV A, WHICH WAS APPROVED BY FAA FORM 8110-3 DATED ON AUGUST 11, 2022. DETAILS OF WORK WAS DOCUMENTED IN HAECO XIAMEN'S NRC WKL8689.
DURING HEAVY MAINTENANCE VISIT, CRACKS WERE FOUND ON THE RIGHT HAND WING LEADING EDGE NO.15 FLAP LOWER BEAM (P/N: I 65B15502-48) AND NO.16 FLAP LOWER BEAM (P/N: 65B15502-46) ADJACENT TO ILES 514.56 AND ILES 543.93 RESPECTIVELY. THE PERMANENT REPAIRS WERE ACCOMPLISHED IN ACCORDANCE WITH FLIGHT VEHICLES CONSULTING, INC'S ENGINEERING ORDER 22125-2 REV 8, WHICH WAS APPROVED BY FAA FORM 8110-3 DATED ON AUGUST .12, 2022, DETAILS OF WORK WERE DOCUMENTED IN HAECO I XIAMEN'S NRC WKL7210 AND WKL7251.

THE PERMANENT REPAIR WAS ACCOMPLISHED IN ACCORDANCE WITH FLIGHT VEHICLES CONSULTING, LNC'S ENGINEERING ORDER 22125-1 REV IR, WHICH WAS APPROVED BY FAA FORM 8110-3 DATED ON AUGUST 12, 2022. DETAILS OF WORK WAS DOCUMENTED IN HAECO XIAMEN'S NRC WKL6261.

EMERGENCY EXIT LIGHT AT UPPER DECK ENTRANCE IS INOPERATIVE. - REPLACED BULB PER B747-400 AMM 33-51-01.
DURING SCHEDULED MAINTENANCE, THE R/H FLAPS, OUTBOARD AFT POSITION, SUSTAINED DAMAGE TO THE T/E FROM GSE IMPACT. APPLIED A TEMPORARY REPAIR UTILIZING POTTING COMPOUND AS NEEDED FOR SMOOTHNESS AND 3 LAYERS OF SPEED TAPE FOR SECURITY. THIS REPAIR IS LIMITED TO 25 FLIGHT CYCLES UPON WHICH PERMANENT REPAIR OR REPLACEMENT IS REQUIRED.
ELECTRICAL SMOKE SMELL NOTICED, HOWEVER, NOT VISIBLE FROM DESCENT TO BELOW FL 200 - INCREASING UNTIL RUNNING QRH (QUICK REFERENCE HANDBOOK) CHECKLIST BEFORE APPROACH. FOUND EQUIPMENT COOLING SUPPLY FAN EXCESSIVELY NOISY AND HOT. SYSTEM DEFERRED AS A PRELUDE TO REPLACEMENT OF THE FAULTY COOLING SUPPLY FAN.

LEFT UPPER DECK DOOR EMERGENCY ESCAPE SLIDE WILL NOT ARM - REMOVED DEBRIS AND LUBRICATED ARMING MECHANISM. OPERATIONAL CHECK SATISFACTORY IN ACCORDANCE WITH B747-400 AIRCRAFT MAINTENANCE MANUAL 53-23-00.
REFER TO NAL LP 44320-3: DURING FLAP OVERSPEED INSPECTION, FOUND DAMAGE TO THE R/H INBD. FORE-FLAP TRAILING EDGE. DAMAGE WAS FOUND TO BE APPROXIMATELY 8" PARALLEL TO THE TRAILING EDGE AND 3" PERPENDICULAR TO THE TRAILING EDGE: DAMAGE FOUND TO EXCEED SERVICE LIMITS.
