AFTER TAKEOFF, NR 2 ENGINE OIL PRESSURE LIGHT ILLUMINATED WITH CORRESPONDING LOW PRESSURE ON GAUGE. NR 2 CHIP LIGHT ILLUMINATED ON FINAL APPROACH BACK TO PATTERSON BASE. NR 2 ENGINE BECAME VERY ERRATIC, SURGING WITH TORQUE FLUCTUATIONS, AND T5 FLUCTUATIONS. MX PERSONNEL REPLACED NR 2 ENGINE.

MAINTENANCE FOUND WIRE BROKEN AT PIN R (CONNECTOR PLUG P105) REPAIRED BROKEN WIRE AND COMPLIED WITH LANDING GEAR RETRACTION CHECK, NO DEFECTS NOTED. (K)

AT A HOVER READY FOR TAKEOFF, NUMBER ONE ENGINE CHIP LIGHT ILLUMATED AT A HOVER. HAD LANDED AT GALLIANO TO REFUEL. MAINTENANCE FOUND ONE SLIVER ON AFT CHIP PLUG. CLEANED PLUG AND COMPLIED WITH 30 MINUTE RUN AND LEAK CHECK. OPS CHECK GOOD.

DURING FLIGHT, PILOT ENCOUNTERED MODERATE VIBRATION OF MAIN ROTOR FOR 3 SECONDS IN CRUISE FLIGHT THEN IT CEASED. THE AIRCRAFT WAS DIVERTED TO LAFOURCHE AIRPORT IS LANDED WITH NO FURTHER PROBLEM, THE MECHANIC FOUND BLUE MAIN ROTOR BLADE EROSION TAPE TORN HE REMOVED THE TAPE AND THE FOLLOWING DAY THE MAIN ROTOR BLADE WAS CHANGED.

TURBINE CAME IN FOR SHOP VISIT FOR LOW POWER. BEGAN DISASSEMBLY OF TURBINE, REMOVED OUTER COMBUSTION CASE AND FOUND SULFIDATION ON 1ST STAGE WHEEL. HAD ONE BLADE MISSING AND SEVERAL BROKEN BLADES. THIS TURBINE HAD A ROLLS ROYCE LINER. HAVE WHEELS TESTED FOR PROPER COATING. (M)

DISCOVERED DURING DAILY INSPECTION, SUPPORT PN 76070-30019-101 (ALUMINUM) SECURING NR 1 EXTERNAL OIL FILTER HOUSING TO OIL COOLER COWLING FAILED FROM FATIGUE CRACK INDUCED BY NORMAL AIRCRAFT VIBRATIONS. REPLACE ALUMINUM SUPPORT WITH ONE MADE OF STAINLESS STEEL. NOTE: THIS IS AN ACTIVE IN-HOUSE MODIFICATION CURRENTLY C/W ON 14 OF 17 GULF COAST S-76'S.
