LIGHT CORROSION FOUND ON THE CABIN FLOOR UNDER THE PILOT AND COPILOT SEAT RAILS. DURING A 12 YEAR INSPECTION OF THE AIRCRAFT.
SERVERAL CRACKS FOUND IN FUSELAGE CANOPY ALONG THE INTERIOR LOWER FRAME FOR THE LEFT AND RIGH-HAND WINDOWS.
AFTER DEPARTURE PILOT EXPERIENCED SLIGHT NG FLUCTUATION AND SLIGHT YAW MOTION. PILOT RETURNED FOR LANDING. DURING TROUBLE SHOOTING MAINTENANCE OF THE ENGINE FUEL CONTROL UNIT, MAINTENANCE FOUND INSIDE THE ACCELERATION CONTROLLER CHAMBER THAT THE LOWER CIRCLIP WAS MISSING FROM FROM A LEVER PIN AND A SPRING FROM THE PIN WAS DISENGAGED FROM THE PIN. FUEL CONTROL WAS REMOVED FROM ENGINE TO BE SENT TO REPAIR SHOP.
DURING COAST DOWN AFTER POST INSPECTION GROUND RUNS MECHANICS HEARD A NOISE COMING FROM THE ENGINE MODULE 4 AND MODULE 5 AREA. TURBOMECA WAS CONTACTED AND HAS REQUESTED THAT THE MODULES BE SENT IN FOR ANALYSIS.
PILOT EXPERIENCED A MAIN ROTOR GEARBOX CHIP LIGHT ENROUTE TO THE AIRPORT. MAINTENANCE EXAMINED THE SYSTEM CHIP DETECTORS AND ELECTRICAL SYSTEM AND FOUND NO CHIP OR ABNOMALIES. AN OPERATIONAL TEST FLIGHT WAS PERFORMED AND FOUND SATISFACTORY. THE AIRCRAFT WAS RETURNED TO SERVICE.
PILOT REPORTED AN ENGINE CHIP LIGHT. MAINTENANCE FOUND A SMALL PIECE OF CARBON (LESS THAN.5MM) ON REAR BEARING CHIP PLUG. PERFORMED LEVEL "B" CHECKS IN REF TO ENGINE MANUAL. RETURNED AIRCRAFT TO SERVICE WITH CONDITION CHECKS.
BETWEEN FLIGHTS WHILE ON GROUND AT FLIGHT IDLE, THE PILOT RECEIVED A ENGINE CHIP LIGHT. MAINTENANCE FOUND A 1.5 MM PIECE OF CARBON ON LOWER ELETRICAL ENGINE CHIP DETECTOR. MAINTENACE PERFORMED ENGINE OIL RINSE, REPLACEMENT, FILTER CHANGE AND GROUND RUNS. CHIP DETECTOR REMAINED CLEAN. ENGINE PLACED IN A 25 HOUR MONITORING CYCLE.
DURING PREFLIGHT CHECKS, PILOT TURNED ON FUEL BOOST PUMPS. NOTICE FUEL COMING FROM THE ENGINE DECK DRAINS. MAINTENANCE DETERMINED THAT THE ENGINE FUEL CONTROL GOVERNOR DRIVE FUEL SEAL WAS LEAKING. THE FUEL CONTROL UNIT WAS REPLACED.
PILOT HAD AN ENGINE CHIP LIGHT ILLUMINATION DURING FLIGHT. HE PERFORMED AN UNEVENTFUL UNSCHEDULED LANDING. MX FOUND A 1MM WIRE (FILIMENT) ON THE LOWER ENGINE ELECTRICAL DETECTOR. MX PERFORMED A DRAIN AND RINSE OF THE OIL SYS AND FOUND ALL PLUGS CLEAN AND RELEASED ACFT BACK INTO SERVICE.
.3 HRS INTO HIS TOUR FLIGHT THE PILOT HAD AN ENGINE CHIP WARNING LIGHT. HE MADE AN UNEVENTFULL UNSCHEDULED LANDING. AT ARRIVAL MX FOUND NO CHIP LIGHT ENGINE OFF. MX FOUND (1) 1.5 MM FILAMENT LEVEL "A" ON THE UPPER ELECTRICAL PLUG ALL OTHER PLUGS WERE CLEAN. THE OIL SYS WAS DRAINED AND RINSED IAW THE MX INSTRUCTIONS AND NO FURTHER PARTICLES WERE DISCOVERED. THE ACFT WAS RETURNED TO SERVICE.
BEFORE THE FIRST FLIGHT OF THE DAY THE PILOT COULD NOT START ENGINE. INSPECTED ENGINE ASSY AND FOUND THE GAS GENERATOR LOCKED UP DUE TO COKING FROM THE FUEL INJECTION MANIFOLD. REMOVE THE ENGINE ASSY AND ASSIST TECH IN REPLACING THE FUEL INJECTION MANIFOLD. (K)

INSPECTED ENGINE ASSY AND THE GAS GENERATOR LOCKED UP DUE TO COKING FROM THE FUEL INJECTION MANIFOLD. REMOVED THE ENGINE ASSY AND ASSIST MFG SERVICE CENTER IN REPLACING THE FUEL INJECTION MANIFOLD. INSPECTED ENGINE ASSY AND THE GAS GENERATOR LOCKED UP DUE TO COKING FROM THE FUEL INJECTION MANIFLOD. REMOVE THE ENGINE ASSY AND ASSIST MFG SERVICE CENTER IN REPLACING THE FUEL INJECTION MANIFOLD. DURING A 500 HR (T-INSPECTION), COULD NOT SPIN COMPRESSOR DISK, DUE TO GAS GENERATOR INJECTION MANIFOLD COKING. MFG TECH REMOVED AND REPLACED INJECTION MANIFOLD ON MODULE NR 3. (K)
