15 MINUTES FROM HOUMA, SMOKE WAS OBSERVED COMING OUT OF A/C VENTS. AIR CONDITIONER AND FAN WERE TURNED OFF AND SMOKE IMMEDIATELY STOPPED.
DURING CRUISE IN IMC NO 2 DC GENERATOR FAILED AND DID NOT RESET AFTER EMERGENCY PROCEDURE WAS PERFORMED.
SUPPLEMENTAL ISSUE FOR REVISED SDR. LEFT MAIN LANDING GEAR WOULD NOT EXTEND. REPAIRED LANDING GEAR. AIRCRAFT PLACED ON JACK, PERFORMED OPERATION CHECK OF LANDING AND THE GEAR WOULD NOT EXTEND. FOUND LEFT MAIN LANDING GEAR TIE DOWN RING IN THE UP POSITION CAUSING THE GEAR NOT TO EXTEND DOWN. REMOVED TIE DOWN CLEVIS AND BUSHINGS, CLEANED AND INSPECTED AND FOUND THE BOLT WORN. REPLACED THE BOLT AND REINSTALLED THE CLEVIS. ALSO REPLACED WORN LEFT HAND MAIN LANDING GEAR DOOR LINK SWIVEL FITTING.
UPON DESCENDING FOR A NORMAL LANDING TO ST300 AT ABOUT 3000 FT PIC AND SIC NOTICED A VERY STRONG ODOR OF FUMES (ELECTRICAL FIRE) IN THE COCKPIT. COCKPIT STARTED TO GET HAZY FROM ALL THE SMOKE. TRIED TO ISOLATE BY TURNING OFF AIR CONDITIONER TO NO AVAIL. NOTICED BOTH PILOT AND CO-PILOT AP1 AND AP2 CYCLIC LIGHT FLICKERING ALONG WITH "FD" FLAG FLASHING ON THE ADI. TEMPORARILY TURNED OFF CO-PILOT CRT SCREENS TO SEE IF SMOKE CLEARED. PULLED OUT CHECK LIST AND FOLLOWED EMERGENCY PROCEDURES. WITH SECONDARY INDICATIONS AND UNABLE TO ISOLATE, LANDED AS SOON AS POSSIBLE WHICH WAS ON SS259. MX FOUND THE GROUND WIRE FOR THE AIR CONDITIONER TO HAVE BURNED AND PULLED OUT OF THE TERMINAL END THAT CONNECTS TO THE STRUCTURE. THIS CAUSED THE AIR CONDITIONER TO SEEK GROUND THROUGH THE MOUNTING BOLTS FOR THE AIR CONDITIONER PALLET.
ENGINE OUT LIGHT AND AUDIO WITH ASSOCIATED YAW KICK ON TAKEOFF. TROUBLESHOT AND FOUND TRANSDUCER DEFECTIVE. REPLACED TRANSDUCER.
ON CLIMB OUT OFF OF MCY AT 400 FEET BOTH PILOTS SMELLED ACRID ODOR. ACFT RETURNED TO BASE. CAUSE OF PROBLEM, EVAPORATOR MOTOR, PN 85-02-250-1, SN K31045. REPLACED.
THE EGRESS LIGHT WAS INOPERATIVE AT SEAT 14D. SECURED LIGHT STRIP.
JUST AFTER TAKEOFF CREW HEARD LOUD NOISE FROM NUMBER 1 ENGINE, FOLLOWED BY EGT GOING TO 653. CREW SHUT DOWN THE ENGINE AND RETURNED TO STL. METAL WAS FOUND IN THE TAIL PIPE AND THE ENGINE WAS REPLACED. FOUND VANES FROM THE 10TH AND 11TH STAGE COMPRESSOR, QUADRANT NUMBER 4, HAD FRACTURED AT THE 10 O'CLOCK POSITION AND MOVED INTO THE GAS PATH STRIKING THE C-10 BLADES. THE DAMAGE THEN MULTIPLIED.
THE EGRESS LIGHT STRIP IN FIRST CLASS WAS INOPERATIVE. DRIED MOISTURE FROM LIGHT TRACK IN FIRST CLASS. CHECK-C 8-16-00 LAX.
WHILE PARKED AT THE GATE THE NUMBER 4 CABIN DOOR ON THE RT SIDE OF THE AIRCRAFT WAS OPENED WHILE THE ESCAPE SLIDE WAS ARMED. THE ESCAPE SLIDE DID NOT FULLY DEPLOY. REPLACED THE ESCAPE SLIDE. CHECK-C 2-20-99 LAX.
DURING CHECK-C MAINT, FOUND THE LT WING I/B FLAP TRAILING EDGE AFT FLAP BOTTOM SIDE, LOWER WEDGE PANEL WITH A .375 INCH TEAR LOCATED APPROXIMATELY 4 FT O/B BY 11 INCHES HIGH FROM I/B END. REPAIRED PER SRM 51-70-17 (REFERRED FROM SRM 57-53-01) WITH MINIMUM OF 2 PLYS PER NOTE B OF FIG 24, WITH 2 LAYERS OF ADHESIVE TOP AND BOTTOM. COOKED AT 200 DEG, SEALED AND PAINTED.
