AFTER 464.6 HOURS, THIS CONTROLLER SPLIT IN 2 PIECES, RENDERING THIS PART USELESS AND CAUSING BOOST TO FALL OFF IN THE INTAKE MANIFOLD. IT APPEARS THAT DURING MANUFACTURE, THE HOUSING FOR THE SPRING WAS MACHINED TOO DEEP ALLOWING ONLY ABOUT .0312 INCH METAL HOLDING THE PART TOGETHER.
THIS CONTROLLER SPLIT IN 2 PIECES, RENDERING THIS PART USELESS AND CAUSING BOOST TO FALL OFF IN THE INTAKE MANIFOLD. DURING MFG, HOUSING FOR THE SPRING WAS MACHINED TOO DEEP ALLOWING ONLY ABOUT .0312 IN OF METAL HOLDING THE PART TOGETHER.
AUTOPILOT WILL NOT TEST DOWN, ALWAYS TESTS UP. UNIT REMOVED AND BENCH TESTED, MALFUNCTION DUPLICATED ON BENCH.
KICKS AUTOPILOT OFF IN SMOOTH AIR. WILL NOT TEST UP OR DOWN. UNIT REMOVED AND BENCH TESTED: DUPLICATED MALFUNCTION 2 ABOVE ON BENCH.
TURBOCHARGER MOUNT LOWER RIGHT HAND ATTACH BOLT LOOSENED AND VIBRATED UNTIL THE NUT AND COTTER KEY FAILED. BOLT THEN DEPARTED THE ENGINE.
LOWER RIGHT HAND ATTACH BOLT HAD LOOSENED AND VIBRATED UNTIL THE NUT AND COTTER PIN WORE OUT AND CAME OFF. BOLT THEN FELL OUT.
PRELIMINARY REPORT - PRECAUTIONARY ENGINE SHUTDOWN FOR VIBRATION, NOISE, AND LARGE QUANTITY OF OIL TRAILING FROM COWLING. SUBSEQUENT EXAMINATION INDICATES THAT THE NR 5 CYLINDER, PISTON CONNECTING ROD, P/N LW 10646 19332, FAILED BELOW THE PISTON PIN. THE RESULTING IMPACT FORCES THEN CAUSED SEPARATION OF THE NR 5 CYLINDER ASSY FROM THE CRANKCASE ASSY.
AIRCRAFT WAS CLIMBING THROUGH APPROXIMATELY 7,000 FEET, PILOT HEARD A MUFFLED SOUND FROM RIGHT ENGINE AND NOTICED FLAMES BURNING THROUGH THE TOP OF ENGINE COWLING. PILOT SHUT DOWN ENGINE. V-BAND CLAMP FROM TURBOCHARGER MALFUNCTIONED AND EXHAUST MANIFOLD BECAME DETACHED FROM TURBOCHARGER. (CLAMP MISSING.) (X)
