REPAIRED THE RH UPPER IB WING SKIN BY R&R OF THE RH AFT DRAG ANGLE (WING-TO-FUSELAGE FAIRING) BLENDING CORRODED AREAS, PERFORMING NDT, REPLACING BLENDED FASTENERS, ROTO-PEENING BLENDED AREAS, TREATING AND PRIMING, AND REPLACING AN EXISTING SKIN PANEL WITH NEW PIN 6722311-18 IN ACCORDANCE WITH TEXTRON FIELD REPAIR FR-750-0179-13061 REVISION - APPROVED BY FAA FORM 8100-9 DATED 17 JAN 2024.
AFTER THE AIRCRAFT TAXIED INTO THE FBO, A FUEL LEAK UNDER THE RIGHT WING WAS NOTED. A SMALL HOLE WAS SEEN UNDER THE WING, INBOARD SECTION. MAINTENANCE WAS CONTRACTED TO INSPECT THE AIRPLANE. ONCE DEFUELED AND FUEL CELL OPENED FOR INSPECTION, CORROSION WAS FOUND RESULTING IN A LEAK AT WS 56.5, APROX 1/2 INCH FORWARD OF STRINGER 18R AND OUTBOARD OF WS 54.85. APPLIED A TEMPORARY REPAIR.
THE FLIGHT CREW REPORTED THEY FOLLOWED THE BEFORE LANDING CHECKLIST & CONFIRMED ALL INDICATIONS OF LG DOWN & LOCKED. THE MAIN GEAR TOUCHED DOWN, SPEED BRAKES WERE DEPLOYED & THE NOSE GEAR CONTACTED THE RUNWAY. THE NOSE CONTINUED LOWERING TO THE RUNWAY SURFACE AS THE GEAR HANDLE MOVED UN-COMMANDED BACK TO THE UP POSITION. THE NOSE GEAR COLLAPSED & RETRACTED, THE GEAR UNSAFE WARNINGS WERE PRESENT, RED UNSAFE LIGHT & WARNING HORN WERE ACTIVE. AN INVESTIGATION OF THIS INCIDENT IS CURRENTLY BEING CONDUCTED. A MECHANICAL FAILURE OF THE GEAR HANDLE WAS THE ROOT CAUSE FOR THE NOSE GEAR COLLAPSE AFTER TOUCHDOWN. THE FAULTY LG CONTROL HANDLE WAS REPLACED & THE FAILED UNIT RETURNED FOR EVALUATION. UNIT FAILED TEST.
DURING DESCENT, THE FLIGHT CREW & PAX SMELT SMOKE, THE STRONGEST WAS IN THE AFT CABIN AREA. AT THE TIME, THE CREW WAS UNCERTAIN OF THE SOURCE & AN EMER WAS DECLARED. ONCE ON THE GND & PAX DEPLANED, TROUBLESHOOTING WITH MXC & FIELD SERVICE WAS ACCOMPLISHED. IT WAS DETERMINED THAT THE COCKPIT ACM HAD FAILED. THE PAC WAS DEFERRED & THE ACFT CONTINUED FOR A SCHEDULED MX EVENT LATER THAT DAY.
AFTER TAKEOFF & DURING GEAR RETRACTION, THE FLIGHT CREW HEARD A THUMP FOLLOWED BY INCREASED WIND NOISE. IT WAS NOTED THE GEAR INDICATING DOWN WITH THE GEAR HANDLE IN THE UP POSITION, THREE GREEN GEAR LIGHTS WITH A RED DISAGREE LIGHT WAS INDICATED. ON THE EICAS, THE FLIGHT CREW RECEIVED AN AMBER HYD VOLUME LOW A & HYD PTU FAIL. THE QTY & PRESS OF THE A SIDE HYD BOTH INDICATED ZERO. THE HYD TEMPERATURE WAS HOLDING STEADY. THE ACFT DIVERTED TO SWF, AN UNEVENTFUL LANDING WAS ACCOMPLISHED & THE ACFT WAS TOWED OFF THE RUNWAY. A RUPTURED RT MLG ACTUATOR HOSE WAS LATER DISCOVERED.
