ACFT ENROUTE AND AIR RETURNED BACK TO DEPARTURE DUE TO F/O WINDSHIELD CRACKED AND ARCING. PILOT STATED F/O VISION IMPAIRED. R & R RT SIDE WINDSHIELD IAW MM 56-10-2 AND R & R RT WINDOW HEAT CONTROLLER IAW MM 30-41-0. OPS CHECKED NORMAL.
F/A EMERGENCY FLASHLIGHTS FOUND INOPERATIVE. R & R THE RT EMERGENCY FLASHLIGHT AND BATTERIES IAW MM 16.02. OPS CHECK GOOD.
DURING TAKEOFF ROLL, CREW EXPERIENCED A COMPRESSOR STALL ON THE NR 3 ENGINE. TAKEOFF WAS REJECTED AT 85 KIAS AND 1.96 EPR. RETURNED BACK TO GATE. MX PERSONNEL PERFORMED A VISUAL INSPECTION OF NR 3 ENGINE, NO DEFECTS OBSERVED. ACCOMPLISHED ENGINE RUN UP TO TAKEOFF POWER AND ALL PARAMETERS WERE NORMAL.
PASSENGER COMPARTMENT FORWARD BULKHEAD EMERGENCY EXIT IDENTIFIER DOES NOT ILLUMINATE. REPAIRED GROUND WIRE TO IDENTIFIER IAW MM 20-50-1 AND RELAMPED SIGN. OPS CHECKS NORMAL IAW MM 33.01.1.
LEFT HAND PASSENGER DOOR ESCAPE SLIDE PRESSURE LOW AND IN RED RANGE (2600 PSI AT 22 DEGREES CELCIUS) - DISCOVERED DURING POSTFLIGHT INSPECTION. MX R & R EMERGENCY ESCAPE SLIDE IAW MM 25-60-1.
AFTER TAKEOFF, WHEN GEARS WERE BEING RETRACTED THE CREW A HEARD LOUD NOISE IN NOSE GEAR AREA. THEY CYCLED NOSE GEAR AND NOISE CONTINUED. THEY RETURNED TO WITHOUT FURTHER INCIDENT. ON POST FLIGHT INSPECTION BOTH DOORS WERE CLOSED AND LATCHED. MAINTENANCE INSPECTED BOTH LEFT AND RIGHT NOSE GEAR DOORS FOR MECHANICAL DEFECTS, NONE NOTED IAW DACO 32-20-0. AIRCRAFT WAS RETURNED TO SERVICE.
DURING CRUISE, THE LT RECIRCULATION FAN HAS A SMELL OF SMOKE. THE CREW TURNED OFF LT RECIRCULATION FAN AND SMOKE, SMELL WENT AWAY. THEY LANDED WITHOUT FURTHER INCIDENT. MX PERFORMED OPS TEST ON BOTH LT AND RT RECIRCULATION FAN, NO SMOKE ODOR NOTED IAW MM 21-21-1. ACFT WAS RETURNED TO SERVICE.
AIR TURN BACK, DUE TO AUTO PILOT WILL NOT STAY ENGAGED WITH ELEVATOR SERVO ON. THE ACFT PITCHES UP AND DOWN AND DISENGAGED IN VERTICAL SPEED OR ALT HOLD MODES. MX TROUBLESHOT AND FOUND WEAK SHIELD GROUND AT GROUND STUD 1385, REPAIRED GROUND SHIELD AT STUD 1385 IAW SWPM 21-21-1. REPLACED AUTO PILOT CONTROLLER, OPS CHECK GOOD IAW MM 22-10-2. ACFT WAS RETURNED TO SERVICE.
DURING CRUISE, THE OVERHEAD COCKPIT CLIMATE CONTROL UNIT EMITS A BURNING ODOR WHEN TURNED ON. THE CREW LANDED WITHOUT FURTHER INCIDENT. MX INSPECTED AND CLEANED THE OVERHEAD CLIMATE CONTROL UNIT FAN. OPS CHECK GOOD IAW MM 21-02. ACFT WAS RETURNED TO SERVICE.

WHILE INCREASING THE INTENSITY OF THE ENGINE INSTRUMENT LIGHTS, THE CREW SMELLED AN ELECTRICAL ODOR COMING FROM THE ENGINE INSTRUMENT LIGHT PANEL. THEY TURNED OFF THE RHEOSTAT TO THE ENGINE INSTRUMENT LIGHTS, THE ODOR WENT AWAY. THEY LANDED WITHOUT FURTHER INCIDENT. MX INSPECTED THE ENG INSTRUMENT LIGHTS AND FOUND THE NR 1 EGT GAGE BACKGROUND LIGHT CONNECTOR SHORTED OUT. THEY DISCONNECTED AND CAPPED WIRES TO CONNECTOR. OPS CHECK GOOD IAW MM 33-13-0. ACFT WAS RETURNED TO SERVICE.
DURING CLIMBOUT, THE NR 2 CARGO SMOKE DETECTOR FLASHED ON MOMENTARLY SEVERAL TIMES WITH ASSOCIATED AUDIBLE WARNING. THE CREW INSPECTED THE CARGO COMPARTMENT FOR SMOKE OR FIRE AND NONE WAS NOTED. THE CREW RETURNED TO DEPARTURE WITHOUT FURTHER INCIDENT. MX INSPECTED THE ACFT FOR EVIDENCE OF FIRE, NONE WAS NOTED. THEY TROUBLESHOT THE SYS AND REPLACED THE NR 2 SMOKE DETECTOR. OPS CHECK GOOD IAW MM 26-.03. ACFT WAS RETURNED TO SERVICE
AT FL 320 CABIN PRESSURIZATION CONTROL WAS SET TO 340 FOR FOR A SELECTED CABIN 4,500 FT CABIN CLIMBED TO 8500 FT. CABIN PRESS CONTROL LEVER PHYSICALLY LIMITED TO 2" FROM FULL CLOSED. THE CREW RETURNED BACK TO DEPARTURE WITHOUT FURTHER INCIDENT. MX FOUND THE OUTFLOW VALVE PRESSURE ACTUATOR WAS NOT WORKING AND R & R IAW MM 21-31-0. THE ACFT WAS RETURNED BACK TO SERVICE.
DURING CRUISE THE RT RECIRCULATION FAN EMMITTING AN ELECTRICAL ODOR. THE CREW TURNED OFF FAN AND SMELL WENT AWAY. THE CREW LANDED WITHOUT FURTHER INCIDENT.

THE CREW WAS PERMITTED PARTIAL CONTROL OF CABIN PRESSURE UNTIL ACFT DESCENDED BLOW FL200. THEY RETURNED TO DEPARTURE WITHOUT FURTHER INCIDENT. MX REPLACED CABIN RATE CONTROLLER. OPS CHECK GOOD IAW MM 21-31-2 AND 21-21-0. ACFT WAS RETURNED TO SERVICE.
DURING CRUISE THE LT RECIRCULATION FAN STOPPED WORKING AFTER 40 MIN AND EMITTED A SMOKE ODOR. THE CREW PULLED CURCUIT BRAKERS AND LANDED IN PHIK WITHOUT FURTHER INCIDENT. AFTER 10 MINUTES OF RUNNING THE RT RECIRCULATION FAN , THE CREW SENSED AN ELECTRICAL/ OZONE SMELL. THE RT RECIRCULATION FAN WAS TURNED OFF, CIRCUIT BREAKER WAS PULLED AND SMELL WENT AWAY. MX REPLACED THE RT RECIRCULATION FAN. OPS CHECK GOOD IAW MM 21-21-1 AND 21-21-0. ACFT WAS RETURNED TO SERVICE.

DURING TAKEOFF ROLL AT APPROX 90 PERCENT N2 RPM THE ENG COMPRESSOR STALLED, EPR, N2 AND EGT ROLLED BACK. THE CREW ABORTED THE TAKEOFF AND RETURNED TO PARKING. GOT CONTROL OF CABIN AT ABOUT 15,000, ACFT ALTITUDE CABIN WAS AT 10,2000. TURNED BLEED OFF ON LEFT SIDE, PRESSURE WOULD NOT GO DOWN. MANIFOLD PRESS AND TEMP STAYED AT 35 PSI AND 200C. MAINTENANCE TROUBLESHOT AND FOUND A LEAKING BLEED HEAT EXCHANGER ON NR 1 ENGINE PYLON. ALSO FOUND BAD PRESS REGULATOR ON NR 2 ENGINE. MAINTENANCE DEFERRED BOTH PER ALL APPLICABLE MANUALS. AIRCRAFT WAS RETURNED TO SERVICE.
DURING CRUISE AT FL 370 LOWER CARGO COMPARTMENT FIRE WARNING ILLUMINATED. DISCHARGED BOTH FIRE EXTINGUISHERS. CREW RETURNED AND LANDED WITHOUT INCIDENT. UPON LANDING MAINTENANCE INSPECTED ALL LOWER CARGO COMPARTMENTS AND FOUND NO EVIDENCE OF FIRE. REF DMI NR 30653, CAT (C) NFD 10/31/04, IAW ALL APPLICABLE MANUALS AND AIRCRAFT WAS RETURNED TO SERVICE.
AFTER TAKEOFF LEFT PACK CONTROL VALVE WOULD NOT MAINTAIN CABIN ALTITUDE OF 10000 FEET WITH AIRCRAFT AT FL250 AND RIGHT PACK OFF. RIGHT PACK CONTROL VALVE WILL NOT MAINTAIN CABIN ALTITUDE OF 10000 FEET WITH AIRCRAFT AT FL 250 AND LEFT PACK OFF. CREW RETURNED AIRCRAFT TO PHIK AND LANDED WITHOUT INCIDENT. AN INSPECTION IS ON GOING AND INFORMATION WILL BE PROVIDED AS SOON AS IT IS AVAILABLE.

OPERATIONAL CHECK GOOD ON GROUND AND ENGINE RUN GOOD PER ALL APPLICABLE MANUALS. AIRCRAFT WAS RETURNED TO SERVICE.
