Maintenance disclosures
DURING SCHEDULED INSPECTION MAINTENANCE FOUND THE LEFT ENGINE EXHAUST TRANSITION PIPE IS CRACKED. THE TRANSITION PIPE WAS REPLACED WITH NO FURTHER DEFECTS NOTED.
PILOT REPORTED PITOT HEAT INOP. UPON INSPECTION THE PITOT SWITCH & SWITCH CONNECTION WERE FOUND TO BE BURNT. BOTH WERE REPLACED WITH NO ADDITIONAL DEFECTS NOTED.
PILOT REPORTED LEFT ENGINE EGT, CHT & TIT ABNORMALLY LOW DURING FINE, AND HEAVY BLACK SMOKE FROM LEFT ENGINE DURING FINAL. CREW ALSO NOTED OIL LEAK ON LEFT ENGINE AFTER LANDING. MAINTENANCE INSPECTED AND FOUND THE INTAKE VALVE ON THE LEFT ENGINE NUMBER 3 CYLINDER TO BE DAMAGED. THE CYLINDER IS BEING REPLACED.
GARMIN INTERNATIONAL INC RETURNED THIS COOLING FAN TO SANDIA AEROSPACE FOR DISASSEMBLY AND ANALYSIS. SMOKE WAS REPORTED IN THE COCKPIT.
SHORTLY AFTER PUTTING AIRPLANE IN CRUISE CONFIGURATION, BOTH CAPTAIN AND F/O SMELT BURNING PLASTIC/STYROFOAM. THE SMELL WAS CHEMICALLY AND STRONG. THE F/O NOTICED SMOKE RISING FROM THE CAPTAIN`S FEET. THE ENVIRONMENTAL SYSTEMS WERE SHUT DOWN (CABIN FAN, RT A/C FAN). NO CIRCUIT BREAKERS WERE OPEN. THE COCKPIT FIRE/SMOKE IN COCKPIT CHECKLIST WAS RAN AND AN EMERGENCY WAS DECLARED. AFTER RUNNING CHECKLIST WE COULD NO LONGER VISIBLY SEE SMOKE COMING OUT, HOWEVER THE SMELL OF BURNING PLASTIC REMAINED STRONG LONG AFTER THE EVENT ENDED. COMPANY MAINTENANCE IDENTIFIED FOUND GIA COOLING FAN INOPERABLE, IT ALSO HAD SMOKE STREAKS ON IT. CHECKED CIRCUIT BREAKERS, NONE POPPED. R & R COOLING FAN AND RETURNED AIRCRAFT TO SERVICE.
THE NLG ATTACH STRUCTURE WAS FOUND TO BE DAMAGED DURING AN INSPECTION INITIATED DUE TO SIMILAR FINDINGS ON OTHER COMPANY AIRCRAFT. THE DAMAGE IS LOCATED AT FWD MOST FUSELAGE BULKHEAD, LWR CENTER SECTION, IN THE AREA CORRESPONDING TO THE NLG MOUNTS. THE STRUCTURE AFT OF NLG BULKHEAD WAS INSPECTED WITH THE FOLLOWING DEFECTS NOTED: THE FWD SECTIONS OF FLOORBOARD SUPPORTS AS THEY ARE RIVETED TO FORWARD-MOST FUSELAGE BULKHEAD ARE CRACKED AND RIVETS WERE FOUND MISSING FROM ORIGINAL PRODUCTION. A SUPPLEMENT IS PROVIDED TO ILLUSTRATE THESE AREAS. THE OEM HAS BEEN NOTIFIED AND A FLEET WIDE INSPECTION HAS BEEN ISSUED FOR ALL COMPANY AIRCRAFT.
