NUMBER ONE ENGINE BARRIER FILTER MAINTENANCE CAUTION CAME ON IN FLIGHT RETURNED TO HOUMA. ALL OTHER ENGINE INDICATIONS NORMAL.
DURING 3Q CLIMB OUT, FLUCTUATIONS IN TORQUE AND N1 OBSERVED WITH YAW TO THE LEFT. THIS WAS FOLLOWED BY DECU MINOR FAULT. PROCEEDED TO RTB.
DURING CRUISE FLIGHT THE MGB PRESSURE INDICATION BEGAN TO FLUCTUATE BETWEEN 65PSI TO 0 PSI. AIRCRAFT RETURNED TO BASE.
DURING CRUISE FLIGHT, WASHERS AND RETENTION RING WERE SEEN ON THE COPILOT FLOOR. DISCOVERED THE RIGHT TAIL ROTOR PEDAL WAS SLIPPING OFF. RETURNED TO BASE WITH NO FURTHER INCIDENT.
DURING TRANSITION FROM HOVER TO FORWARD FLIGHT, THE NR 1 ENGINE FIRE WARNING LIGHT ILLUMINATED WITH A STEADY WARNING TONE. LIGHT/TONE EXTINGUISHED AFTER 2-3 SECONDS. LANDED AND RETURNED TO THE RAMP. MX CLEANED MOISTURE FROM CONNECTOR.
HIGH IRON CONTENT IN SOAP SAMPLE - EXCESSIVE WEAR IN MO5 MUFF COUPLING AND DRIVE SPLINES. REPLACED MO5 AND MUFF COUPLING.

WHILE PERFORMING 500 HOUR TAIL ROTOR SPAR INSPECTION FOUND RED TAIL ROTOR BLADE SPAR CRACKED ALONG FORWARD EDGE. MAINTENANCE REPLACED THE TAIL ROTOR BLADE ASSY. NOTE: PREVIOUSLY SUBMITTED ON 7/24/09.
ON GEAR RETRACTION, RED CAUTION LIGHT CAME ON WITH 3 GREEN. NR 2 SERVO CAUTION LIGHT CAME ON. RESET CAUTION LIGHT AND LANDED BACK HBP. MAINTENANCE REPLACED THE NOSE WHEEL CENTERING WIRING HARNESS. NOTE: PREVIOUSLY SUBMITTED ON 7/15/09.
