PILOT REPORTED FAULTY TORQUE INDICATION ON RT ENG.
ON TAKEOFF ROLL THE LT ENGINE FLAMED OUT, PILOT ABORTED TAKEOFF NO FURTHER INCIDENT, RETURNED TO BLOCK. INVESTIGATION BY MX FOUND THE LT FUEL PUMP SHAFT SHEARED.
LT ENGINE ITT SPIKED TO 1150, THEY REDUCED POWER TO FLIGHT IDLE AND ITT REMAINED AT 830 DEG CENT. SHUTDOWN ENGINE AND CONTINUED TO DESTINATION, LANDED WITHOUT INCIDENT. INVESTIGATION, SPLIT ENG AND DID A HOT SECTION INSPECTION AND FOUND NO DEFECTS. TESTED THE T-5 PROBES AND FOUND 7 EA BAD. REPLACED ALL 10 PROBES AS PRECAUTION AND REPLACED THE FUEL NOZZLES AS A PRECAUTION. OPS CHECKS OK ON ENGINE RUN.

MAINTENANCE INSPECTED LANDING GEAR SYSTEM, AND PINNED GEAR. DISCREPANCY DEFERRED TO MSL 092197-2C. AIRCRAFT RELEASED FOR FLIGHT, AIRCRAFT RESTRICTED TO MSL PAGE 32-1/R13/07-15-96, PARA 32, INDEX 3. REPAIRED BROKEN WIRE IN RT STUB WING. RETRACTION CHECKED 10 TIMES. GROUND OPS CHECKED GOOD. AIRCRAFT RELEASED FOR FLIGHT. FLIGHT RESTRICTION REMOVED. (M)

CLT - FLT 5093 - AFTER DEPARTING CLT, THE NR 1 ENGINE OIL PRESSURE GAUGE STARTED FLUCTUATING. AIRCRAFT RETURNED BACK TO CLT, FOR AN UNEVENTFUL LANDING. MAINTENANCE WAS DISPATCHED TO THE AIRCRAFT. NR 1 ENGINE WAS TROUBLE SHOT AND FOUND ENGINE OIL LEVEL LOW. SERVICED ENGINE WITH OIL. GROUND RUN AND OPS CHECKED GOOD. AIRCRAFT RETURNED TO SERVICE. (M)
CLT/PGV - FLT 5248 - FLIGHT WAS APPROXIMATELY 46 MILES OUT OF CLT WHEN THE LEFT FUEL FILTER CAUTION LIGHT ILLUMINATED DIMLY. THE CAPTAIN CALLED MAINTENANCE CONTROL WITH THE PROBLEM AND HE WAS TOLD TO RETURN TO CLT. UPON THE AIRCRAFT'S RETURN TO CLT, MAINTENANCE PERSONNEL WERE DISPATCHED TO TROUBLESHOOT PROBLEM. LEFT ENGINE FUEL FILTER ELEMENT WAS REMOVED AND INSPECTED FOR CONTAMINATION AND THE FILTER ELEMENT WAS THEN REPLACED AS A PRECAUTIONARY MEASURE. THIS DID NOT FIX THE INDICATION PROBLEM AND THE INDICATION SYSTEM WAS THEN DEFERRED TO THE MSL PER MEL 28-3 UNDER CONTROL NR 0322-10C. THE AIRCRAFT WAS THEN RETURNED TO SERVICE. (X)
