Maintenance disclosures
ON POST FLIGHT, CREW NOTICED DAMAGE TO THE RT THRUST REVERSER (TR). MAINTENANCE FOUND THE IDLER ARM ROD END BROKEN. THE BREAK BISECTED THE ROD END PERPENDICULAR TO THE ARM. THE BUSHING REMAINED ATTACHED TO THE DOOR ASSEMBLY. THE BROKEN ARM INDUCED ADDITIONAL DAMAGE INTO THE ADJACENT STRUCTURE. THE (TR) WAS REMOVED AND REPLACED.
DURING A SCHEDULED MAINTENANCE VISIT, WHILE PERFORMING A HORIZONTAL STABILIZER ROLLER INSPECTION OF THE HORIZONTAL STABILIZER IB RIB AND BRACKET ASSEMBLIES, THE RT IB ROLLER BRACKET WAS FOUND CRACKED. DETAILED INSPECTION INSTRUCTIONS OF THIS AREA ARE OUTLINED IN MFG COMMUNIQUÃ' NR 70. THE RIB WAS REPLACED WITH THE NEW UPGRADED RIB.
INVESTIGATED PILOT REPORT OF LOSS OF NR 1 AND NR 2 VHF COMMUNICATION RADIOS WHEN FLYING IN PRECIPITATION. IAW P-STATIC TESTING FOUND POOR NR 1 AND NR 2 VHF ANTENNAS BONDS TO FUSELAGE. CLEANED, REBONDED AND REINSTALLED ANTENNAS. ALSO INSTALLED LT FUEL VENT LINE BONDING JUMPER. P-STATIC TESTS AND OPERATIONAL TESTS SATISFACTORY AFTER REPAIRS. SUGGEST VHF ANTENNAS BE REMOVED AND CLOSELY INSPECTED AT SCHEDULED INTERVALS, SUCH AS AN INITIAL 5000 HR OR 10 YEAR INSPECTION, FOLLOWED BY RECURRING 1200 HR OR 2 YEAR INTERVALS (IN CONUNCTION WITH C-CHECK INSPECTIONS).
THE CREW REPORTED THAT WHILE IN CRUISE FLIGHT AT FL 250 THERE WAS EXTREMELY HOT AIR BLOWING FROM THE REAR VENT IN THE LAVATORY. THE AFT CABIN WALL WAS FOUND TO BE EXTREMELY HOT AND WAS SMELLING LIKE BURNING RUBBER. A REQUEST WAS MADE TO DIVERT THE AIRCRAFT, AND AN UNEVENTFUL LANDING WAS MADE. FOLLOWING LANDING, THE CREW ALSO NOTED THAT THE AIRCRAFT WAS PRESSURIZING WHILE ON THE GROUND. MAINTENANCE REPLACED THE CABIN TEMPERATURE CONTROL VALVE AND PERFORMED A CABIN TEMPERATURE SYSTEM TEST. OPERATIONAL AND LEAK CHECKS WERE GOOD. THE AIRCRAFT WAS RETURNED TO SERVICE.
THE CREW REPORTED THAT THE NOSE WHEEL DID NOT INDICATE DOWN WHEN THE GEAR WAS EXTENDED FOR LANDING IN KDAL. THE LIGHT DID ILLUMINATE WHEN TESTED. WHEN THE FLAPS WERE EXTENDED TO 30 DEGREES, AND THE THROTTLES REDUCED TO IDLE THE GEAR WARNING HORN DID NOT SOUND OFF. THE TOWER INFORMED THE CREW THAT THE GEAR APPEARED TO BE DOWN WHEN A GO-AROUND WAS PERFORMED. THE AIRCRAFT WAS LANDED WITHOUT FURTHER INCIDENT. MAINTENANCE ADJUSTED THE NOSE GEAR DOWN AND LOCK SWITCH. THE AIRCRAFT WAS RETURNED TO SERVICE.
AIRCRAFT IN CRUISE AT 410 FT LOST OIL PRESSURE ON NR 1 ENGINE AND ENGINE SHUTDOWN. AN EMERGENCY WAS DECLARED AND AIRCRAFT DESCENDED AND LANDED AT SRQ. PRELIMINARY REPORT INDICATES BEARING FAILURE.
