AT FLIGHT LEVEL 350 CABIN ALTITUDE WARNING LIGHT ILLUMINATED, CABIN BEGAN CLIMBING AT 2000 FPM. CABIN ALTITUDE WENT ABOVE 12000 FEET. R&R CONDITIONED AIR CHECK VALVE SENSE LINE IAW B737 AMM 21-61-25. R&R COMPRESSOR OUTLET PNEUMATIC DUCT IAW B737 AMM 36-19-15. PERFORMED THE STANDBY AND AUTOMATIC RATE CHECK IAW B737 AMM 21-31-00, ALL CHECKS GOOD. THIS CLEARS DMI 73200161, PLACARDS REMOVED.
MULTIPLE BIRD STRIKES DEPARTING RWY 27 IN KCVG AT 200 FOOT ALT. SMELL OF BIRDS THROUGH THE PACKS ON RETURN TO KCVG. PERFORMED CONDITIONAL INSPECTION IAW B737-400 AMM 05-51-37. OXYGEN MASKS USED BY ALL CREW MEMBERS ON DESCENT. AROUND 11500 FT CAPTAIN AND FOS CARGO DOOR LIGHT WENT OUT. TRANSFERRED TO DMI #73200062 IAW MEL 52-3-1 CAT C DUE 3/15/2019 PLACARDS INSTALLED, C/W SPECIAL PROCEDURES VERIFIED ALL LATCHES LOCKED. CORRECTIVE ACTION CLEANED CARGO DOOR LATCH HOOK SWITCHES IAW SWPM 20-60-01, OPS CHECK GOOD IAW MM 52-35-00, NO DEFECTS NOTED AT THIS TIME.

DURING THE FIRST TAKEOFF ATTEMPT, HAD A FWD ENTRY DOOR LIGHT ILLUMINATE ALMOST IMMEDIATELY AFTER BRAKE RELEASE. REJECTED THE TAKEOFF. A PROXIMITY SWITCH WAS NEEDED TO BE ADJUSTED CLOSER TO THE DOOR ACTUATOR TO PREVENT THE LIGHT FROM COMING ON DURING TAKEOFF.
TAKEOFF FROM BDL RUNWAY HEADING TO 4000 FEET. ON CLIMB OUT BECAME AWARE OF LOUD NOISE FROM BEHIND FLIGHT DECK. WAS INFORMED BY MECHANIC THAT L1 WAS LEAKING AIR. AT THIS TIME NOTICE CABIN WAS NOT PRESSURISING. ASKED AND WAS CLEARED BY DEPARTURE TO MAINTAIN 4000 FEET. ALL DOOR LIGHTS WERE EXTINGUISHED HOWEVER IT WAS CLEAR WE HAD A DOOR LEAK AT L1. RAN THE ENTRY DOOR QRH CHECKLIST, AND ELECTED TO RETURN TO BDL FOR A PRECAUTIONARY LANDING. GIVEN CLEARANCE FOR ILS 24 APPROACH AND LANDING. UPON POST FLIGHT IT WAS DISCOVERED THAT THE LOWER DOOR FLAP WAS NOT IN THE SEATED POSITION EVEN THOUGH THE INTERIOR DOOR LEVER WAS IN THE CLOSED POSITION AND THE CORSPONDING DOOR LIGHT WAS EXTINGUISHED. IT WAS FURTHER DISCOVERED THAT A BENT PORTION OF THE ULD IN THE NUMBER ONE POSITION HAD HIT THE SMOKE BARRIER DOOR AND CAUSED IT TO HIT THE L1 DOOR KNOCKING TO OUT OF ALIGNMENT. NONE OF THIS COULD SEEN BEFORE TAKEOFF DUE TO THE INTERIOR L1 DOOR PANEL COVERING THE AFFECTED AREA. MX AND DISPATCH WERE NOTIFIED. DISCREPANCY WAS NOTED IN THE A/C LOG. THE ULD WAS MOVED TO P2, THE MECHANIC RE ADJUSTED L1 DOOR, NEW WEIGHT AND BALANCE AND RELEASE WERE ISSUED. FLIGHT RESUMED 1:30 LATE.
TAKEOFF ROLL WAS UNEVENTFUL. DURING THE ROTATION, FELT A SHAKING IN THE LEFT REAR OF THE AIRCRAFT IMMEDIATELY BEFORE LIFTOFF. THE SHAKING STOPPED ONCE AIRBORNE. FELT LIKE THE AIRCRAFT WAS SKIPPING TO THE SIDE DUE TO A LITTLE SIDE LOAD FROM THE CROSSWIND. THE FLIGHT WAS UNEVENTFUL UNTIL LOWERING THE GEAR DURING APPROACH. WITH THE GEAR HANDLE DOWN, SAW THE LEFT MAIN WAS SHOWING A RED UNSAFE LIGHT AND THE GEAR HORN WAS SOUNDING. CYCLED THE GEAR AND THE SAME THING HAPPENED. PERFORMED A GO AROUND TO RUN THE CHECKLIST. ACCOMPLISHED THE GEAR DISAGREE, AND ONE OR BOTH MAIN LANDING GEAR POSITION INDICATORS SHOW UNSAFE CHECKLISTS. WITH THE GEAR HANDLE DOWN, THE MAIN GEAR INDICATORS SHOWED THE RED LEFT MAIN LIGHT, BUT THE SECONDARY MAIN GEAR INDICATOR SHOWED GREEN. RETRACTED THE GEAR AND THE RED LIGHT TURNED OFF, BUT THE SECONDARY GREEN LIGHT STAYED ILLUMINATED. LOWERED THE GEAR AGAIN. PERFORMED THE FLY-BY WITHOUT INCIDENT. THE TOWER SAID THE GEAR APPEARED TO BE DOWN AND LOCKED. DECLARED AN EMERGENCY. TOUCHDOWN WAS SMOOTH, BUT THE STRONG VIBRATION IN THE BACK LEFT HAPPENED ALMOST IMMEDIATELY. NO BRAKING APPLIED. THE TOWER REPORTED SEEING ONE OF OUR LEFT MAIN TIRES BLOWN OUT. AT 70 KNOTS THE VIBRATION DISAPPEARED. SHUT DOWN THE AIRCRAFT ON THE RUNWAY.
