ENROUTE FROM RDU TO EGE, DIVERTED TO DEN DUE TO FLAPS FAILURE ANNUNCIATION WHEN FLAPS SELECTED TO 5 '. SLATS EXTENDED, BUT FLAPS DID NOT FULLY EXTEND TO 5 '. RAN ABNORMAL CHECKLIST, FLAPS FAILED TO RESET. EMERGENCY WAS DECLARED IN ROUTE TO DEN AND AIRCRAFT LANDED SAFELY IN DEN. AIRCRAFT WAS AOG IN DENVER . FLAPS WERE INSPECTED AND OPERATIONAL CHECKS COMPLETED NUMEROUS TIMES IAW CESSNA MODEL 750 AMM 27-50-00. NO DEFECTS NOTED.
N733FL INFLIGHT: PILOT MESSAGE: WE HAD AN AMBER CAS MESSAGE OF HYDRAULIC A LOW VOLUME. WE PULLED UP THE EICAS PAGE FOR HYDRAULICS AND CONFIRMED THE DROPPING QUANTITY OF A-SIDE FLUID AND RAN THE ASSOCIATED CHECKLIST, WHICH CALLED TO UNLOAD THE A SYSTEM PUMP AND PULLING THE HYDRAULIC PTU CB. BY THE TIME WE COMPLETED THAT CHECKLIST, WE NOTED THE REMAINING QUANTITY WAS AT 10%. WE DECLARED AN EMERGENCY AND NOTIFIED ATC OF THE REQUIRED DIVERSION TO RSW AND SET UP FOR THE APPROACH/LANDING. WE HAD NOTED THE A FLUID QUANTITY WAS REPORTING 25% WHILE UNLOADED. I DECIDED TO FIRST ATTEMPT A CONVENTIONAL LANDING GEAR EXTENSION WITH THE REMAINING HYDRAULIC FLUID BEFORE ATTEMPTING AN EMERGENCY GEAR EXTENSION USING THE NITROGEN BLOW DOWN BOTTLE. WE RELOADED THE A SYSTEM AND SUCCESSFULLY PERFORMED A CONVENTIONAL LANDING GEAR EXTENSION, UNLOADING THE A SYSTEM AGAIN AFTERWARD. WE MADE AN APPROACH AND SUCCESSFUL LANDING, USING THE EMERGENCY BRAKES TO FULLY STOP THE AIRCRAFT ON THE RUNWAY. MX FINDINGS: DURING TROUBLESHOOTING, MAINTENANCE FOUND A STANDOFF BRACKET FOR A CLAMP IN THE LEFT-WING ROOT THAT HOLDS A CLUSTER OF HYD LINES FATIGUED AND BROKE. THIS ALLOWED THE PRESS A HYD LINES TO FLEX OVER AND RUB AGAINST A FLIGHT CONTROL CABLE THAT CHAFFED A SMALL HOLE IN THE PRESSURE LINE. THERE WAS NO DAMAGE TO THE FLIGHT CONTROL CABLE. BAKER AVIAITON MX CREATED A QUALITY NOTIFICATION AND MX ACTION PLAN. THE PLAN INCLUDED A ONE PAGE NOTIFIACTION, MEETING WITH ALL TECHS, SHORT TERM AND LONG TERM ACTION PLAN. SHORT TERM ACTION: BAKER MX IS INSPECTING ALL CITATION X'S IN OUR FLEET TO ENSURE THIS ISSUE DOESN'T EXIST UNDETECTED ON ANY OTHER ACFT.
MAY 18TH 2019 AIRCRAFT DIVERTED FROM KIAD TO TJSJ,-C/WO 109380: FOUND CRACKED STAINLESS STEEL A SIDE HYDRAULIC LINE LT PYLON TO TAILCONE. REMOVED AND REPLACED LINE P/N 6717770-19 WITH SERVICEABLE PART SAME P/N ON MAY 23, 2019. AIRCRAFT DIVERTED FROM TJSJ TO KMIA FOR AN A SIDE HYDRAULIC FLUID LOSS RESULTING IN EMERGENCY LANDING GEAR EXTENSION/BRAKING UPON LANDING IN KMIA, TECHNICIANS DETERMINED THERE WAS NO ADDITIONAL LEAK AND THAT THE LOSS OF VOLUME WAS DUE TO ADDITIONAL AIR STILL PRESENT IN THE LDG HYD LINES WHICH WOULD REQUIRE FURTHER GEAR SWINGS ON A HYDRAULIC MULE TO BLEED OUT. SINCE NO MX HANGAR FACILITIES OR JACKS COULD BE SECURED FOR USE AT KMIA, THE A HYDRAULIC SYSTEM WAS RESERVICED TO THE CORRECT LEVELS VIA HAND PUMP AND CHECKED DURING AN ENGINE RUN, DURING WHICH NO DECREASE IN A SYS VOLUME WAS NOTED AFTER CYCLING ALL POSSIBLE HYDRAULIC SYSTEMS (EXCEPT FOR THE LANDING GEAR). THE AIRCRAFT WAS FERRIED TODAY 5/25 WITH THE LANDING GEAR DOWN AND LOCKED TO KMCO, WHERE THE REQUIRED EQUIPMENT IS AVAILABLE TO JACK AND CYCLE THE LANDING GEAR ON A MULE IN ORDER TO REBLEED THE A HYDRAULIC SYSTEM. THE FERRY FLIGHT WAS CARRIED OUT WITHOUT INCIDENT. SERVICE CENTER KMCO ACCOMPLISHED HYDRAULIC A SIDE SYSTEM BLEED PROCEDURES AND SERVICING. AIRCRAFT HAS FLOWN SEVERAL FLIGHTS WITHOUT REOCCURANCE.
CREW REPORTED AT FL450, LOST GUIDANCE COMPUTERS A & B WHICH CAUSED LOSS OF NUMEROUS FLIGHT INSTRUMENTS. DESCENDED TO FL300. DECLARED AN EMERGENCY AND DIVERTED. ACFT LANDED WITHOUT INCIDENT. MX COULD NOT DUPLICATE THE DISCREPANCY.
WHILE ON SHORT FINAL, CREW REPORTED THE NR 1 FMS DISPLAY WENT BLANK FOLLOWED BY AN ODOR OF SMOKE. MX VERIFIED THE NR 1 FMS WAS INOPERATIVE AND REPLACED THE UNIT.
CREW REPORTED OF A NOTICEABLE "SLACK" IN THE YOKE CAUSING A DELAY OF INPUT FOR A TURN. NOTED AT ALTITUDE WITH THE AUTOPILOT ON THE WINGS WOULD SLOWLY ROLL AND THEN A MECHANICAL "BUMP" COULD BE FELT CORRECTING, THIS WOULD CYCLE ABOUT EVERY 10-15 SECONDS. MX PERFORMED AN INSPECTION OF THE AILERON SYS AND FOUND THAT THE AIRFRAME ATTACHMENT FOR THE AILERON PCUWAS CRACKED.
DURING CRUISE FLIGHT, CREW REPORTED A WHISTLING SOUND FROM THE MAIN ENTRY DOOR IN THE CABIN, FOLLOWED BY A RUSH OF AIR AND PRESSURIZATION LOSS AT A 2000 FPM RATE. EMERGENCY WAS DECLARED, THE PASSENGER O2 MASKS WERE DEPLOYED AND AN EMERGENCY DESCENT WAS INITIATED. THE ACFT LANDED SAFELY. MX FOUND PRIMARY DOOR SEAL DAMAGED AND REPLACED SEAL.
WHILE OPERATING ON THE GROUND WITH THE COCKPIT PAC IN MAX COOL, CREW REPORTED A LOUD BANG, FOLLOWED BY A SMOKE ODOR. NO FIRE OR VISIBLE SMOKE WAS NOTED. THE COCKPIT ACM WAS FOUND FAILED AND REPLACED.
DURING DESCENT, CREW REPORTED A BAGGAGE COMPARTMENT SMOKE WARNING. ACFT LANDED SAFELY. MX INSPECTED THE SYS BUT WERE UNABLE TO DUPLICATE THE DISCREPANCY.
DURING TROUBLESHOOTING OF DISCREPANCY FOR NOSE WHEEL STEERING INOPERATIVE, MX FOUND THE NOSE WHEEL STEERING CIRCUIT BOARD SEVERELY BURNED. NO SMOKE OR BURNING ODOR WAS NOTED BY THE CREW OR PASSENGERS. MX REPLACED THE FAULTY BOARD.
DURING DESCENT, THE FLIGHT CREW REPORTED "BAGGAGE SMOKE" CAS MESSAGE ILLUMINATED. NO SMOKE WAS NOTED AND THE ACFT LANDED SAFELY. MX PERSONNEL CLEANED AND RESEATED THE BAGGAGE SMOKE DETECTOR ELECTRICAL CONNECTOR AND TESTED THE SYSTEM. SYSTEM TEST WAS SATISFACTORY.
