PROBLEM DESCRIPTION: TRAILING EDGE FLAP ASYMMETRY LEFT FLAP INDICATE UP & RIGHT FLAP INDICATES L1, SLATS DID NOT EXTEND WHEN FLAP HANDLE WAS MOVED TO #1 DETENT. CORRECTIVE ACTION & OTHER PERTINENT INFORMATION: FOUND #3 FLAP TRANSMISSION TORQUE TUBE COUPLING DISENGAGED. INSPECTED TORQUE TUBE IAW B737 AMM 27-51-00. RE-ENGAGED AND SECURED TORQUE TUBE COUPLING IAW B737 AMM 27-57-64, PERFORMED OPS CHECK OF TRAILING EDGE FLAP SYSTEM IAW B737 AMM 27-51-00 OPS CHECK GOOD.

DURING FLIGHT LE FLAP TRANSIT LIGHT ILLUMINATED (#5 LE LIGHT)
DURING TAXI AT KCVG THE AIRCRAFT DEPARTED THE PAVED SURFACE. DURING RECOVERY THE AIRCRAFT WAS DAMAGED BY SUPPORT EQUIPMENT. DAMAGE IS LOCATED ON THE AFT FUSELAGE JUST FORWARD OF THE AFT LOWER CARGO DOOR. CORRECTIVE ACTION FOR THE REPAIR WILL BE LOGGED AFTER THE COMPLETION OF THE REPAIRS AS A STRUCTURAL DAMAGE REPORT.

CO PILOT STATIC INSTRUMENT AS WELL AS TRANSPONDER FAILED ON TAKEOFF. CORRECTIVE ACTION & OTHER PERTINENT INFORMATION: PERFORMED OPERATIONS CHECK OF THE NR 2 EFIS SYSTEM, IRU SYSTEM, DADC AND ATC SYSTEM, IAW B737 AMM 34-22-00, 34-28-01, 34-12-01 AND 34-53-02. ALL SYSTEMS OPERATIONS CHECK GOOD, NO DEFECTS NOTED AT THIS TIME.
FLAPS FAILED TO EXTEND PAST 15 DEGREES ON LANDING APPROACH. CORRECTIVE ACTION & OTHER PERTINENT INFORMATION: R & R TRAILING EDGE FLAP POSITION INDICATOR, IAW B737 AMM 27-58-41. ALL OPERATIONS CHECKS GOOD NO DEFECTS NOTED.
DRAIN MAST MATING SKIN SURFACE HAS CORROSION @ #2 & #4 MOUNTING HOLES LOCATED @ STA 986 - 1006 BETWEEN STR 26L & 25L
DURING TAKEOFF, NR 1 ENGINE HIT 965 DEG. FOR 3 SECONDS UNTIL PULLED BACK. ENGINE WAS REMOVED AND SENT FOR A SHOP VISIT DURING SHOP VISIT, R & R THE EGT HARNESS, RERIGGED THE VSV, AND R & R THE MEC, ENGINE WAS REINSTALLED AND PAST ALL TESTS
DURING TAKEOFF NR 1 ENGINE HIT 965 DEG. FOR 3 SECONDS UNTIL PULLED BACK. PERFORMED VISUAL INSPECTION OF NR 1 ENGINE INLET AND EXHAUST, NO DEFECTS NOTED AND PERFORMED BORESCOPE INSPECTION OF THE HTP BLADES AND THE 1ST STAGE LPT BLADES IAW AMM 72-00-00, NO DEFECTS NOTED, A/C FOUND TO BE SUITABLE FOR NON-REVENUE FLIGHT TO OSC IAW AMM 72-00-00. FURTHER INSPECTION IN PROGRESS IN KOSC.

ON GEAR RETRACTION RIGHT MAIN GEAR UNSAFE LIGHT REMAINED ILLUMINATED. CORRECTIVE ACTION: R&R RIGHT MAIN GEAR UPLOCK PROXIMITY SWITCH IAW AMM 32-61-21, PERFORMED OPS CHECK AND GEAR SWING IAW AMM 32-61-21
DURING CLIMB OUT AT 14,000 FT CABIN PRESSURE GOES TO 10,000 FT IN STAND BY AND AUTO MODES
UPON LANDING RIGHT HAND MAIN LANDING GEAR TORSION LINK BROKE, CORRECTIVE ACTION & OTHER PERTINENT INFORMATION: R&R THE MAIN LANDING GEAR UPPER AND LOWER TORSION LINK, TORSION LINK PIN, LEFT HAND SHIMMY DAMPER, #3 & #4 MWA, #3 BRAKE LINE, #4 BRAKE LINE AND NUMBER 3 BRAKE. PERFORMED MAIN GEAR DAMPER TEST / INSPECTION, NO DEFECTS NOTED.
ON CLIMB OUT AT 8000 FEET THE CAPT AND FO RED MAIN CARGO SMOKE ANNUNCIATOR LIGHTS ILLUMINATED ON THE BOTTOM FWD INSTRUMENT PANEL AS WELL AS THE NR 5 SMOKE DETECTOR LIGHT ON THE AFT OVERHEAD PANEL. CORRECTIVE ACTION: FOUND LOOSE CONNECTOR ON NR 5 MAIN DECK SMOKE DETECTOR. RE SECURED CONNECTOR, OPS CHECK GOOD IAW AEI MM 26-19-01, NO DEFECTS NOTED.
UPON LANDING AT KCVG ON RUNWAY 09 AND AT THE TIME OF TOUCHDOWN THE AIRCRAFT SHOOK LEFT TO RIGHT AND PULLED TO THE RIGHT. POST FLIGHT INSPECTION FOUND LEFT MAIN LANDING GEAR SHIMMY DAMPENER MISSING AND TORSION LINK BROKEN AND DAMAGE TO THE HYDRAULIC LINES AND WHEEL ASSY. AIRCRAFT TAXIED UNDER ITS OWN POWER TO ITS PARKING LOCATION. AIRPORT CLOSED RUNWAY UNTIL A FOD CHECK WAS COMPLETED. REPAIR AND CORRECTIVE ACTION IN WORK AT THIS TIME.
