Maintenance disclosures
DURING PHASE INSPECTION THE TURBOCHARGER BEARING FLANGE WHERE IT MEETS THE TURBOCHARGER TURBINE WAS CRACKED IN TWO PLACES CAUSES EXHAUST GAS TO JET OVER THE TOP OF THE TURBOCHARGER. OTHER FAILURES HAVE BEEN FOUND IN OUR FLEET AS WELL. NO CAUSE KNOWN OR RECOMMENDATIONS AT THIS TIME.
WING FUEL CAP INTERNAL BLACK PLASTIC RIB BREAKS OFF INSIDE THE LOWER FUEL CAP RETAINER ASSEMBLY.
IT WAS FOUND THAT THIS AIRCRAFTâS ENGINE OIL SUMP BAFFLE IS CRACKED DURING A BORESCOPE INSPECTION THROUGH THE OIL DIPSTICK/SERVICE LOCATION. ADDITIONAL INFORMATION WILL BE MADE AVAILABLE ONCE THE OIL SUMP BAFFLE IS ACCESSED AND PICTURES TAKEN. ATT: 1092.9 ETT: 1092.9 ESTIMATED CYCLES ON ENGINE: 4371.6. THE REPAIR BS PERFORMED THIS INSPECTION BECAUSE AN IDENTICAL ACFT IN THE FLEET EXPERIENCED LOW OIL PRESSURE DUE TO THE OIL SUMP BAFFLE CRACKING INTO PIECES. 1 OF THE SMALLER PIECES GOT LODGED IN THE OIL PICK UP TUBE CAUSING LOW OIL PRESSURE WITH A WARNING AND RESULTING ECU FAIL CODE BEING ACTIVATED. THE ENGINE MFG HAS BEEN MADE AWARE OF THE FAILURE. NO RECOMMENDATIONS AT THIS TIME. OIL SUMP BAFFLE REPLACED WITH ENGINE MFG LATEST PART REVISION.
DURING A 100 HOUR INSPECTION OF THE FUEL CAP AND FUEL CELL AREA A PIECE OF PLASTIC WAS FOUND LAYING IN THE FUEL FILLING AREA. AFTER INSPECTING THE FUEL CAP THE PIECE OF PLASTIC THAT WAS FOUND WAS CONFIRMED TO COME FROM THE PLASTIC PART OF THE FUEL CAP.
TRAINING FLT 9101 - CLT-HKY-CLT - ON APPROACH INTO CLT WHEN THE LANDING GEAR WAS SELECTED DOWN, THE NOSE GEAR INTRANSIT LIGHT (RED) DID NOT EXTINGUISH AND THE NOSE GEAR DOWN LIGHT (GREEN) DID NOT ILLUMINATE. FOLLOWED THE CHECKLIST PROCEDURES, DECLARED AN EMERGENCY, AND DID A FLYBY OF THE TOWER FOR NLG POSITION. AN UNEVENTFUL LANDING WAS INTO CLT. THE CAPTAIN THEN NOTIFIED MAINTENANCE CONTROL OF THE PROBLEM. THE LANDING GEAR WAS PINNED AND THE AIRCRAFT WAS TAKEN TO HEAVY MAINTENANCE. THE PROBLEM WAS A FAULTY NLG DOWNLOCK SWITCH. REMOVED AND REPLACED SWITCH. OPS CHECK GOOD. (M)
FLT 5194 - CLT-AGS - ON APPROACH INTO AGS, WHEN THE LANDING GEAR WAS SELECTED DOWN THE NOSE GEAR INTRANSIT LIGHT DID NOT EXTINGUISH AND THE GREEN LIGHT DID NOT ILLUMINATED. FOLLOWED CHECKLIST PROCEDURES AND DECLARED AN EMERGENCY, DID A FLY-BY OF THE TOWER FOR NLG POSITION. EMERGENCY EQUIP STANDING BY THE AIRCRAFT MADE AN UNEVENTFUL LANDING IN AGS. THE GEAR WAS PINNED AND THE RETRACTION SYSTEM DEFERRED PER MEL 32-3 UNDER CONTROL NR 0701-10C. THE AIRCRAFT WAS THEN RETURNED TO SERVICE. THE MSL WAS CLEARED BY OPS CHECK OF LANDING GEAR SYSTEM NUMEROUS TIMES ON 7-2-96 CCAIR WO NR 34679. NO DEFECTS NOTED. (M
FLT NR 5058 - PGV-CLT - ON APPROACH TO CLT WHEN THE CREW SELECTED GEAR DOWN THE NOSE GEAR FAILED TO INDICATE DOWN AND LOCKED. THE CAPTAIN DECLARED AN EMERGENCY AND PERFORMED A FLYBY THE GEAR WAS IN THE DOWN POSITION. THE AIRCRAFT MADE A SAFE LANDING IN CLT. MX POSITIONED JACK UNDER THE THE NOSE GEAR WAS FOUND TO BE LOCKED. THE GEAR WAS PINNED AND THE RETRACTION SYSTEM WAS DEFERRED TO THE MSL PER MEL 32-3 UNDER CONTROL NR 0308-07C. THE A/C WAS TAKEN TO THE HANGER TO HAVE THE MSL CLEARED. THE AIRCRAFT WAS JACKED AND THE SELECTOR SWITCH WAS FOUND TO BE INOPERATIVE. THE SELECTOR WAS REPLACED AND THE SYSTEM TESTED. ALL INDICATIONS WERE NORMAL. THE MSL WAS THEN CLEARED AND THE A/C WAS RETURNED TO SERVICE. (M)
CLT/OAJ - FLT 5006 - ON 2-13-96, HAD ROTATED AND JUST SELECTED GEAR UP WHEN NR 1 ENGINE PRODUCED 'LOUD BOOM' AND SHUT ITSELF DOWN. CAPTAIN DECLARED EMERGENCY, PERFORMED EMERG CHECKLIST ITEMS, AND RETURNED TO FIELD FOR UNEVENTFUL LANDING. AFTER PASSENGERS DEPLANED, MX INSPECTED NR 1 ENGINE. OIL FOUND AROUND INTAKE AREA. OIL FILTER WAS PULLED AND FND CONTAMINATED WITH FINE METAL PARTICLES. COMPRESSOR SCREEN PULLED AND COMPRESSOR COULD NOT BE TURNED BY HAND. BASED ON THESE FINDINGS, DECISION MADE TO REPLACE ENGINE. NO ENGINE IN STOCK, ONE ORDERED AOG. ACFT CURRENTLY IN HANGAR WITH NR 1 ENGINE REMOVED AWAITING ARRIVAL OF A SERVICEABLE ENGINE. (X)
