THE AIRCRAFT WAS AT CRUISE AND DURING THAT TIME THE CREW HEARD A LOUD BANG AND THEN NOTICED THAT THE RIGHT WINDSHIELD WAS CRACKED . CREW NOTIFIED ATC AND RAN THE QRH . AIRCRAFT LANDED SAFELY AT KPWA
THE FLIGHT CREW REPORTED A WING AND STAB OVERHEAT FLASHING FAULT ANNUNCIATOR AFTER TAKEOFF. THE MAINTENANCE BASE TROUBLESHOT AND DETERMINED THE P40 ANTI-ICE RELAY PANEL WAS DEFECTIVE. THE PANEL WAS REMOVED, AND A REPAIRED UNIT INSTALLED. OPS CHECKS WERE SATISFACTORY, AND THE AIRCRAFT WAS RETURNED TO SERVICE
THE FLIGHT CREW REPORTED THE PRIMARY PITCH TRIM FAILED ON SHORT FINAL WHEN FULLY CONFIGURED WITH THE AUTO PILOT. THE LANDING WAS UNEVENTFUL. THE MAINTENANCE VENDOR TROUBLESHOT AND FOUND THE TRIM ACTUATOR AND TRIM CONTROL BOX DEFECTIVE. THE PITCH TRIM ACTUATOR WAS REMOVED AND A REPAIRED ACTUATOR INSTALLED AND THE TRIM CONTROL BOX WAS REMOVED WITH A REPAIRED UNIT. OPS CHECKS WERE SATISFACTORY AND THE AIRCRAFT WAS RETURNED TO SERVICE. TRIM CONTROL BOX REMOVED P/N 6627401000-003 S/N D450 STAB ACTUATOR REMOVED P/N 6627401000-009 S/N B0596
THE FLIGHT CREW REPORTED A PRIMARY PITCH TRIM FAIL (AMBER) CAS MESSAGE ON SHORT FINAL. THE AIRCRAFT WAS FULLY CONFIGURED, FLAPS 40 DEGREES. WHITE MACH TRIM ADVISORY CAS. THE LANDING WAS UNEVENTFUL. THE MAINTENANCE PROVIDER TROUBLESHOT AND DETERMINED A DEFECTIVE HORIZONTAL STAB CONTROL BOX. THE UNIT WAS REMOVED AND REPLACED BY A REPAIRED UNIT. OPERATIONAL CHECKS WERE SATISFACTORY AND THE AIRCRAFT WAS RETURNED TO SERVICE.
THE FLIGHT CREW PRIMARY PITCH TRIM FAIL ON SHORT FINAL ACCOMPANIED BY AN AMBER CAS MESSAGE. THE LANDING WAS UNEVENTFUL. THE BASE MAINTENANCE TEAM PERFORMED AN OPERATIONAL CHECK OF THE PITCH TRIM SYSTEM PER THE AIRCRAFT AMM AND FOUND THE TRIM SYSTEM OPERATIONAL CHECKS WERE SATISFACTORY. THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING FINAL APPROACH, ONE PASSENGER CAME FORWARD AND ASKED FOR THE LOCATION OF THE FIRE EXTINGUISHER. STATED SOMETHING TO THE EFFECT OF SMELLING SMOKE AND AN ELECTRICAL ODOR. CHECKED CIRCUIT BREAKERS AND FOUND THE RT CABIN SPOT LIGHT CIRCUIT BREAKERS HAD TRIPPED. CHECKED BACK WITH THE PASSENGERS, AND THEY STATED THAT THEY NO LONGER SMELLED ANYTHING. CONTINUED APPROACH WITHOUT ANY FURTHER EVENTS. WIRING IN THE CONTROL PANEL FOR THE NR 2 SEAT, VIP SEAT, RUBBED AGAINST EXPOSED MOVING PARTS OF THE RETRACTABLE CUP HOLDER FOR THAT SEAT CAUSING THE CUP HOLDER TO HEAT UP PRODUCING AN ODOR. EVENTUALLY THE CIRCUIT BREAKER FOR THE LIGHTING CONTROL UNIT TRIPPED AND POWER WAS REMOVED FROM THE WIRE PREVENTING ANY FURTHER DAMAGE. INTERNAL INVESTIGATION ALONG WITH MFG, FOUND THAT THE MODEL CUP HOLDER HAS EXPOSED MOVING PARTS THAT RUBBED AGAINST THE WIRING.
