FLIGHT 7676, HOM-ANC. DISCREPANCY: WHILE BEING MARSHALLED FOR DEPARTURE, THE RIGHT BRAKE FAILED. AIRCRAFT WAS SHUTDOWN. AFTER POST-FLIGHT INSPECTION, IT WAS DISCOVERED THAT THE RIGHT BRAKE PAD HAD FALLEN OFF ON THE RAMP. CORRECTIVE ACTION: MAINTENANCE REPLACED THE BACK PLATE ASSY ON THE RMG. OPERATIONS AND TAXI CHECK GOOD.
FLIGHT 7661 ON APPROACH INTO ANC. DISCREPANCY: AUTOPILOT PITCHED STRAIGHT UP ON ILS 7R INTO PANC. TRIM FOLLOWED. PILOT PRESSED AUTOPILOT DISCONNECT, PULLED ELEVATOR TRIM CIRCUIT BREAKER AND A/P FD CIRCUIT BREAKER. CONTINUED APPROACH AND LANDED SAFELY IN ANC. CORRECTIVE ACTION: MAINTENANCE INSPECTED ELEVATOR TRIM SERVO AND PERFORMED AUTOPILOT OPS CHECK. NO DEFECTS NOTED. OPS CHECK SATISFACTORY.
THE PILOT SMELLED FUEL IN THE COCKPIT DURING CLIMB-OUT. RETURNED TO ANC WITHOUT INCIDENT. AN EMERGENCY WAS NOT DECLARED. FOUND FUEL LINE B-NUT LOOSE AND NOT SAFETY WIRED AT BRACKET ON ENGINE SIDE OF FUEL FLOW TRANSMITTER. TIGHTENED AND SAFETY WIRED B-NUT ON FUEL LINE. ON GROUND RUN, NO LEAKS WERE NOTED. AIRCRAFT WAS RETURNED TO SERVICE. (M)
ANCHORAGE - APPROX .25 MILE OUT FROM ANC PILOT NOTED THE DOOR OPEN WARNING LIGHT CAME ON. THE PILOT TURNED BACK TO ANC. WHILE RETURNING, THE UPPER CARGO DOOR OPENED IN FLIGHT. THE AIRCRAFT LANDED SAFELY AT ANC. MAINTENANCE ADJUSTED THE CARGO DOOR HANDLE DETENT. MAINTENANCE INSPECTED DOOR AND NO DAMAGE NOTED. AIRCRAFT WAS RETURNED TO SERVICE. (M)
ON TAXI OUT OF ANC, DEICING CONDENSATION BEGAN TO BUILD ON THE INSIDE OF THE WINDSHIELD. PILOT ATTEMPTED TO USE BLEED AIR TO DEFROST THE WINDSHIELD. VISABILITY WAS DENSE FOG AND THE RAMP WAS COVERED WITH ICE AND SNOW. THE PILOT WAS IN THE PROCESS OF STOPPING THE ACFT TO WAIT FOR THE WINDSHIELD TO DEFROST WHEN HE HEARD A CLUNK. PILOT IMMEDIATELY SHUT DOWN THE AIRCRAFT. VISUAL INSPECTION DISCOVERED ACFT HAD STRUCK AN OBSCURED TAXIWAY LIGHT. REPLACEMENT PROPELLER EN ROUTE. TURBINE SECTIONS INSPECTED IAW P & W MM. NO DEFECTS NOTED. PROPELLER REMOVED AND REPLACED IAW MANUFACTURE INSTRUCTIONS. ENGINE GROUND RUN AND FUNCTION CHECK WAS GOOD. AIRCRAFT WAS RETURNED TO SERVICE 11-21-96. (X)
DURING COMPLIANCE OF CESSNA SB CAB 96-15 'FUEL HOSE INSPECTION-REPLACEMENT', FOUND ONE OF THE MAIN FUEL SUPPLY HOSES WAS DETERIORATED TO THE POINT THAT FUEL FLOW WAS RESTRICTED. SUBMITTER FEELS THIS CONDITION COULD LEAD TO AN ENGINE SHUT DOWN IN-FLIGHT.
