DURING INSPECTION, BOTH AFT EMERGENCY LIGHT LENSES WERE FOUND TO BE CRACKED. THE LENSES WERE REPLACED IN ACCORDANCE WITH THE MAINTENANCE MANUAL. NO FURTHER DEFECTS WERE NOTED.
DURING INSPECTION, THE EMERGENCY LIGHT BATTERY PACK 15LN FAILED A FUNCTIONAL CHECK. THE BATTERY PACK WAS REPLACED IN ACCORDANCE WITH THE SAAB AMM.
THE CREW NOTED THAT THEY HAD INCORRECT FLAP INDICATIONS WHILE CONFIGURING THE AIRCRAFT FOR LANDING. THE FLAPS INDICATED 13 WHEN SET TO 15 DEGREES AND 18 WHEN FLAPS 20 WAS SELECTED. THE CREW DECLARED AN EMERGENCY AND RETURNED TO FIELD. THE AIRCRAFT LANDED WITHOUT INCIDENT. MAINTENANCE PERFORMED OPERATIONAL CHECKS OF THE FLAP SYSTEM WITH NO DEFECTS NOTED.
DURING INSPECTION, CORROSION WAS FOUND ON THE ANGLE AT LT WING WS 337.0-343.0 . THE ANGLE WAS REPLACED IN ACCORDANCE WITH THE SRM.

DURING INSPECTION, CORROSION WAS FOUND ON THE PLATE AT STA 541.5 CENTER AREA. THE PLATE WAS REPLACED IN ACCORDANCE WITH THE SRM
DURING CLIMBOUT OF FL130 FROM SCEL THERE WAS VIBRATION FOLLOWED BY A LOUD BANG, ACCOMPANIED BY YAW AND HIGH EGT ON NUMBER 3 ENGINE. THE EGT LIGHTS FOR NR 3 CAME ON AND THE GAUGE INDICATION, PEGGED OUT. ENGINE SHUTDOWN RECALL ITEMS AND THE SEVERE DAMAGE CHECKLIST WERE ACCOMPLISHED TO GW 635,000 POUNDS. NORMAL THREE ENGINE APPROACH AND LANDING WERE CONDUCTED TO RUNWAY 17R AND NO SPECIAL ASSISTANCE WAS REQUIRED AFTER LANDING OR TAXI TO RAMP. REMOVED AND REPLACED NR 3 ENGINE IAW, SAI, RNR 4771001R2.

EMPENNAGE WHILE CARRIED OUT MODIFICATION AT STATION 2598 RT BULKHEAD IAW SB 747-53-2473, REV 2, FOUND 6 EA EXISTING HOLES IAW DWG 624U0061 FASTENERS TO BE INSTALLED (.1875) HILOKS. REPAIR CARRIED OUT IAW EN NR 355-09.
DURING CRUISE INBOUND TO KMIA, CREW REPORTED MAIN DECK WINDOW 850R SOUNDED LIKE AIR LEAK. FURTHER INSPECTION ON GROUND REVEALED 9 INCH CRACK IN MAIN ENTRY DOOR 2R SIDE FRAME. DOOR ASSEMBLY REMOVED AND REPLACED IN ACCORDANCE WITH AMM 52-11-01/PG 401.
ON TAXI-OUT AT SKBO, WIND SHEAR LIGHT CAME ON WHEN THROTTLE ADVANCED. MULTIPLE FAILURES, WIND SHEAR LIGHT FLICKERED AND WENT OFF, THEN SMOKE CAME UP THROUGH THROTTLE QUADRANT; LOST GEAR DOWN GREEN LIGHT, AND ANTI-SKID LIGHT ILLUMINATED ON FWD PANEL; LOST ANTI-SKID LANDING TILT INPUTS. LOST POWER LIGHTS TO LEFT SIDE PROBE HEATER; NO ANTI-SKID HYDRAULIC LIGHT WITH PARKING BRAKES ON; NR 3 ENGINE HYDRAULIC AND ENGINE LOW PRESSURE LIGHT OUT WITH ENGINE OFF. I-52 C/B P-12 PANEL FOUND POPPED. AIRCRAFT RETURNED TO BLOCKS. INSPECTION FOUND WIRE HARNESS CHAFING AGAINST NR 1 THROTTLE CABLE, WITH EVIDENCE OF ARCING. REPAIRED WIRES IN ACCORDANCE WITH STANDARD WIRING PRACTICES MANUAL (D6-54446), CHAP 20-10-13. RESET CIRCUIT BREAKER IN ACCORDANCE WITH GMM 4.35-1. NO FURTHER FAULTS DETECTED. COMPLIED WITH AMM 5-51-50/PG 201 INSPECTION FOR SMOKE AND FUMES IN CABIN.
DURING PRE-FLIGHT FROM KMIA, FOUND FORWARD AND AFT UPPER DECK EMERGENCY EXIT SIGNS INOPERATIVE. REMOVED AND REPLACED BATTERY PACKS IN ACCORDANCE WITH AMM 33-51-04.
ON TAKEOFF FROM KMIA, UNABLE TO OBTAIN NECESSARY N1 OF 113.6 PERCENT FOR NR 2 ENG. AT N1 OF 109.0 PERCENT, RED WARNING CAME ON AND N1 MAINTAINED INDICATION AT 109.0 PERCENT. ON CLIMBOUT FROM KMIA, BOTH CHANNEL A & B NACELLE TEMPERATURE INDICATIONS WERE 5.0. REDUCED THRUST LEVER TO IDLE AND BOTH NACELLE TEMPS REDUCED TO 2.25 (A) AND 2.75 (B). ACFT RETURNED TO KMIA. NO FUEL WAS DUMPED. INSPECTION FOUND BLEED AIR PRECOOLER LEAKING WITH CRACKED FLANGE. ENG CHANGE REQUIRED. ALL AD`S ARE CURRENT FOR THIS ENG. (S)
IN FLIGHT ON CLIMBOUT, NR 4 ENGINE FIRE WARNING LIGHT ILLUMINATED, ACCOMPLISHED AIRCRAFT OPERATING MANUAL (AOM) (SAI OP-523) PROCEDURES. FIRE INDICATIONS WENT OUT WHEN ENGINE SHUTDOWN. NO FIRE BOTTLES BLOWN. NR 4 ENGINE CENTER LOWER ACCESS DOOR FOUND OPEN AFTER LANDING. POST FLIGHT: CLOSED AND SECURED NR 4 ENGINE LOWER ACCESS DOOR IAW AMM 78-30-01. ACCOMPLISHED HIGH POWER ENGINE RUNS ON NR 4 ENGINE AND FOUND TO BE WITHIN ALL PARAMETERS OF OTHER ENGINES WITH EXCEPTION OF NR 4 THRUST LEVER 3/4 BEHIND NR 1, 2, AND 3. IAW AOM, VOL 1, LIMITATIONS OKAY FOR CONTINUED SERVICE. AS PRECAUTION, LOCKED OUT NR 4 ENGINE HIGH STAGE VALVE IAW AMM 36-11-17. TRANSFERRED TO DI NR 16258 IAW DDPG, MEL 36-1 (2). PLACARD INSTALLED. EGT'S ALL READING ERRATIC DURING CLIMB-OUT FROM KMIA. OFF FLAGS FOR NR 2 COMES ON ERRATICALLY, NO LIGHTS COME ON. OPERATIONAL NORMAL AT ENGINE START AND CRUISE FLIGHT. RETURNED TO KMIA. NO FUEL DUMPED. REMOVED AND REPLACED VERTICAL SCALE EGT INDICATOR IN ACCORDANCE WITH AMM 77-21-03. OPERATIONAL CHECK GOOD.
NOSE LANDING GEAR DOOR OPEN LIGHT ILLUMINATED IN FLIGHT. COMPLETED DOOR OPEN LIGHT PROCEDURES IAW QRH. FOUND LOOSE ACCESS PANEL AND PARTS BAG OBSTRUCTING RIGHT HAND DOOR HINGE. SECURED PANEL AND OPERATIONALLY CHECKED NOSE GEAR DOOR SYSTEM WITH NO FURTHER DISCREPANCIES NOTED.
INADVERTENTLY DEPLOYED RIGHT SIDE UPPER DECK SLIDE WHILE AIRCRAFT WAS PARKED AT KMIA. REMOVED SLIDE IN ACCORDANCE WITH AMM 25-68-07. TRANSFERRED TO DI LOG #12408 IN ACCORDANCE WITH MEL 25-11. PLACARD INSTALLED. SLIDE DEPLOYED WITHOUT ANY MALFUNCTION.
