Maintenance disclosures
WHILE ON FINAL APPROACH WITH GEAR EXTENDED, FLIGHT CREW RECEIVED A RED GEAR "UNLOCK" LIGHT AND GEAR WARNING HORN, ALONG WITH 3 GREEN DOWN AND LOCKED INDICATIONS. THEY THEN CONDUCTED A GO AROUND AND CYCLED THE GEAR, WITH THE SAME RESULTS. A HOLDING PATTERN WAS THEN REQUESTED AND THE CREW RAN THE CHECKLIST. AFTER DISCUSSION WITH OPERATIONS AND PER THE "LANDING GEAR FAILS TO EXTEND" CHECKLIST, CREW ELECTED TO UTILIZE THE EMERGENCY GEAR EXTENSION TO FURTHER ENSURE THAT THE GEAR WAS FULLY EXTENDED. FLIGHT CREW DECLARED AN EMERGENCY WITH ATC, AND LANDED UNEVENTFULLY. DURING TROUBLESHOOTING, MAINTENANCE FOUND BOTH THE PCB MODULE NC002 (P/N: 9914728-2 S/N: 252) AND THE LT GEAR DOWNLOCK SWITCH (P/N: 6-0407-1) TO BE AT FAULT. BOTH DEFECTIVE PARTS WERE REPLACED WITH NEW. LANDING GEAR RESET AND OPERATIONAL CHECKS WERE NORMAL. AIRCRAFT WAS RETURNED TO SERVICE.
FLIGHT CREW RECEIVED A HYD QUANTITY LOW A AMBER CAS MESSAGE WHILE AT FL450. CREW CONDUCTED THE APPROPRIATE CHECKLIST AND DISCUSSED THEIR OPTIONS WITH MX CONTROL. SINCE MIA HAD THE LONGEST RUNWAY AND FAVORABLE WINDS, THE DECISION WAS MADE TO DIVERT THERE, WHERE THEY LANDED WITHOUT INCIDENT. MAINTENANCE FOUND A FAILED LT ENGINE DRIVEN HYDRAULIC PUMP AS THE CAUSE OF THE HYDRAULIC LEAK. HYDRAULIC SYSTEM A WAS FLUSHED AND FILTERS REPLACED. THE LT EDP WAS REPLACED WITH AN OVERHAULED UNIT; LEAK AND OPERATIONAL CHECKS WERE NORMAL. THE AIRCRAFT IS SCHEDULED FOR A CONFIDENCE FLIGHT PRIOR TO IT BEING RETURNED TO SERVICE.
WHILE ON DESCENT INTO SBA, FLIGHT CREW HEARD AN AUDIBLE POP AND SHORTLY AFTERWARDS RECEIVED A MASTER CAUTION ALONG WITH A HYD PUMP B FAIL CAS MESSAGE. AFTER CONFERRING WITH MX CONTROL THE DECISION WAS MADE TO DIVERT TO VNY WHICH HAD THE LONGER RUNWAY. CREW LANDED WITHOUT INCIDENT IN VNY. MAINTENANCE FOUND THE B HYDRAULIC SYSTEM ENGINE DRIVEN PUMP (P/N: 66166) HAD FAILED. THE B HYDRAULIC SYSTEM EDP WAS REPLACED WITH AN OVERHAULED UNIT, ALONG WITH A COMPLETE SYSTEM FLUSH AND FILTER REPLACEMENT. SYSTEM BLEED, LEAK AND OPERATIONAL CHECKS WERE NORMAL. THE AIRCRAFT WAS RETURNED TO SERVICE.
AFTER APPROXIMATELY 4.5 HOURS INTO CRUISE FLIGHT, THE FLIGHT CREW RECEIVED A âRE-TRIM R WING LOWâ AMBER CAS MESSAGE. CREW TRANSFERRED FUEL TO THE LEFT WING AND MADE MINOR ADJUSTMENTS TO THE AILERON AND RUDDER TRIM; THE CAS MESSAGE EXTINGUISHED. DURING DESCENT, AND JUST AS THEY WERE ABOUT TO INTERCEPT THE LOCALIZER, THE AIRCRAFT SUDDENLY VEERED OFF COURSE TO THE LEFT. CREW DISCONNECTED THE AUTO PILOT, AND NOTED WHILE TRYING TO FLY TO THE RIGHT THAT THE YOKE WAS VERY STIFF, NO EMERGENCY WAS DECLARED. CREW LANDED SAFELY, AND ONCE ON THE GROUND THE YOKE SEEMED TO LOCK UP COMPLETELY TO THE RIGHT (CLOCKWISE).THE LH AILERON PCU WAS BALANCED AND AILERON SYSTEM FUNCTIONAL CHECKS PERFORMED, WITH NO DEFECTS NOTED.
DURING A POST FLIGHT INSPECTION, THE FLIGHT CREW NOTED FUEL LEAKING FROM THE LT ENGINE GANG DRAIN AREA. A CONTRACTED MAINTENANCE TECHNICIAN FOUND A FIRE LOOP HAD WORN A HOLE INTO AN ADJACENT FUEL LINE AND ALSO CAUSED SIGNIFICANT WEAR INTO A SECOND FUEL LINE; NO DAMAGE WAS NOTED ON THE FIRE LOOP. BOTH FUEL LINES (P/N: 23056449 & 23056762) WERE REPLACED WITH NEW; LEAK AND OPERATIONAL CHECKS WERE NORMAL. A FLEET WIDE INSPECTION CAMPAIGN IS CURRENTLY BEING CONDUCTED ON OTHER AIRCRAFT FOR ANY DEFECTS.
WHILE EN ROUTE, THE FLIGHT CREW REPORTED THE B HYD SYS DROPPED TO 17 PERCENT. THE CREW FOLLOWED THE CHECK LIST & SHUTDOWN & UNLOADED THE B HYD SYS. THE A SYS CONTINUED TO FUNCTION NORMALLY. IT WAS DISCOVERED THAT THE LT ELEVATOR PCU PRESS SWIVEL FITTING WAS LEAKING FROM THE O RING SEAL. SEAL WAS R & R, LEAK CHECKED NORMAL.
WHILE ENROUTE AT FL190, THE PILOT SMELLED SOMETHING BURNING. THE CO-PILOT CONFIRMED. A TRACE OF SMOKE WAS NOTED IN THE COCKPIT. THE FLIGHT CREW THEN DONNED EMERGENCY EQUIPMENT. LANDED WITH NO FURTHER ISSUES. DETERMINED THE COCKPIT AIR CYCLE MACHINE HAD FAILED.
