DURING DEPARTURE CLIMB OUT OF NONESSENTIAL ELECTRICAL ITEMS. HE INSREASED VENTILATION AS THE CO INDICATOR GOT DARKER INDICATING CO INCREASE. HE RETURNED AND LANDED WITHOUT FURTHER PROBLEMS ALTHOUGH HE DID EXPERIENCE A HEADACHE AND NAUSEA. MAINTENANCE REPLACED THE MUFFLER AND HEATER SHROUD ON THE LT SIDE. OPS CHECK FLIGHT WAS NORMAL.

PILOT REPORTED UNCONTROLLABLE PITCH CONTROL AND LIMITED AILERON CONTROL. IN FLIGHT, AFTER TAKEOFF, WHICH RESULTED IN A CRASH LANDING. ON EXAMINATION OF THE AIRCRAFT IT WAS DISCOVERED THAT THE RIBBON CABLE ASSY FOR ELECTRICAL CONTROLS ON THE YOKE, HAD FOULED THE CONTROL WHEEL BEARING ASSEMBLY. (UNIVERSAL) BY JAMMING THE CABLE BETWEEN THE ROLLERS AND THE CONTROL TUBE. THIS KEPT THE PILOT FROM HAVING ELEVATOR RESPONSE UNLESS MAXIMUM FORCE WAS APPLIED.

PILOT REPORTED ALTERNATOR INOPERABLE AT TAKEOFF POWER. AT IDLE, ALTERNATOR APPEARED TO OPERATE NORMALLY. UPON EXAMINATION, FOUND THE COARSE THREAD INTERNAL WRENCHING BOLTS HOLDING THE ALTERNATOR CASE HALVES WERE LOOSE, WHICH AT HIGH ENGINE RPM, CAUSED THE CASE TO FLOAT ON THE ARMATURE DRIVING THE ALTERNATOR OFF-LINE. SUBMITTER SUGGESTED MANUFACTURER OR OVERHAUL FACILITY SHOULD CONSIDER A HIGHER TORGUE VALUE ON THESE BOLTS TO CURTAIL FURTHER RROBLEMS. THIS WAS THE SECOND OCCURRENCE ON THIS AIRCRAFT WITHIN A 100-HOUR TIME FRAME. (X)
