ATA 2750

INCRUISE FLIGHT WITH AUTOPILOT ENGAGED, FLAPS WENT TO THE FULLY EXTENDED POSITION UNCOMMANDED. STANDBY FLAP CIRCUIT BREAKER POPPED. PILOT RESET CIRCUIT BREAKER AND RETRACTED FLAPS TO THE 10 DEGREE POSITION IN THE STANDBY MODE. PILOT LANDED WITH FLAPS AT 10 DEGREES. UNABLE TO DETERMINE CAUSE. REPLACEMENT OF STBY MOTOR WAS TO REPAIR DAMAGE CAUSED BY MALFUNCTION AND NOT THE CAUSE. STANDBY FLAP MOTOR DRIVE SHEARED. SINCE NO CAUSE WAS DETERMINED NO C/A TAKEN. C/A WILL MONITOR FLEED TO ANY REOCCURANCE. (K)

ATA 2820

DEPARTED AUS , FLT 8914 TO SJT/HRL. PILOT REPORTED ON LANDING THAT FUEL PRESS LOW LIGHT CAME ON AND WOULD NOT EXTINGUISH. MECH OPENED PANELS FOR ACCESS TO RESERVOIR. OPENED DRAIN VALVE FOR THE RESERVOIR, A VACUUM SUCKED AIR INTO FUEL SYSTEM. HE THEN LOOKED AT THE TOP OF WING AND SAID THEY WERE OIL CANNED ON TOP. WHEN FUEL CAPS WERE OPENED,THE SKINS POPPED BACK OUT. MAINT INSTRUCTED HIM TO TRY AND BLOW OUT THE VENT LINES WITH AIR PRESS, HE WAS UNABLE TO DO SO. HE GOT A NITROGEN BOTTLE AND WITH HIGH PRESS WAS ABLE TO BLOW OUT THE VENTS. BOTH VENTS WERE PLUGGED UP ABOUT 12 TO 14 INCHES INTO THE VENTS BY DIRT DAUBERS, THE ACFT WAS THEN REPOSITIONED BACK TO AUS ON A 91 FLIGHT.

AIRCRAFT WAS FLOWN FROM AUS TO HRL. UPON APPROACH TO HRL, LOW FUEL PRESSURE WARNING LIGHT ILLUMINATED AND WOULD NOT GO OFF. INSPECTION REVEALED THE FUEL TANKS WERE UNDER VACUUM. WHAT APPEARED TO BE CAKED MUD WAS FOUND INSIDE BOTH LEFT AND RIGHT FUEL VENT LINES. CLEARED VENTS, GROUND RAN, OPS CHECKED OK. BOTH WINGS UPPER SKINS WERE DISTORTED AND TWO RIBS JUST INBD OF WS 214.3 APPEARED TO BE CRUSHED. AIRCRAFT WAS FERRIED TO ICT FOR REPAIR. A COVER FOR THE WING TIP SIMILAR TO THE PITOT COVERS USED ON THESE AIRCRAFT MAY PREVENT THIS PROBLEM.
