AFTER TAKEOFF AND SELECTING GEAR UP THE AMBER GEAR DISAGREE AND CYAN PROX SYS FAULT DISPLAYED ALONG WITH LT GEAR SHOWING EXTENDED WITH THE GEAR HANDLE IN THE UP POSITION. COMPLIED WITH CHECKLIST AND DIVERTED TO KIAD WITH NO FURTHER ISSUES. INTERROGATED THE MDC AND FOUND THAT SENSOR PX39, PX09 AND PX05 WAS WITHIN THE TOLERANCE RANGE AND THE HEALTH WAS OK BUT SENSOR PX39 WAS RIGGED CLOSE TO THE EDGE OF THE TOLERANCE RANGE. PERFORMED A GEAR SWING & FOUND AFTER GEAR WAS RETRACTED WE GOT A PROX SYS FAULT MSG. AFTER INTERROGATING THE MDC FOUND THAT THE FAULT CODE WAS LGDLK2 SENSOR B3-007504 INDICATING THAT SENSOR PX39 IS REPORTING HEALTHY BUT OUT OF THE TOLERANCE RANGE. WORK PERFORMED IAW AMM 32-61-00.
DURING TAKEOFF, IMMEDIATELY AFTER ACFT BECAME AIRBORNE, HEARD 3-4 LOUD BANGS FROM WHAT SOUNDED LIKE THE LT ENG. BELIEVE IT WAS POSSIBLY A COMPRESSOR STALL. RIGHT ENG UNEXPECTED SURGE. BELIEVE THE RT ENG SURGE WAS DUE TO THE LT ENG COMPRESSOR STALL. C/W ENGINE DOWNLOADS TO MCC, IT WAS DETERMINED THE LT ENGINE HAD NO ENGINE SURGES/ISSUES. RT ENGINE SHOWED ONE SINGLE SURGE EVENT. THIS PROMPTED RT ENGINE CHANGE. REMOVED RT ENGINE. RT ENGINE INSTALLED IAW EMM 72-05-22 REV.12 AND CH-350 AMM REV. 39 OPERATIONAL TEST REQUIRED PERFORMED OPERATIONAL CHECK IAW HONEYWELL 72-05-22 REV.12 AND CH350 AMM REV 39.VERIFIED .RIGGING/ADJUSTMENT REQUIRED OPERATIONAL TEST SATISFACTORY.
THERE IS A STRONG SMELL OF FUEL IN THE COCKPIT DURING TAKEOFF ROLL. UNABLE TO ISOLATE WHERE ITS COMING FROM BUT ITS VERY APPARENT DURING TAKEOFF AND CLIMB OUT. INSPECTED AND WIPED DOWN ENGINES AND COWLINGS OPERATED ENGINES AT HIGH POWER SETTINS FOR 7 MINUTES VERIFIED NO FLUID LEAKS. NO FUEL SMELL PRESENT IN CABIN OR COCKPIT. NOTIFIED MCC. PER MCC REQUEST, INSPECTED APU CONTAINMENT AREA FOR EVIDENCE OF FLUID LEAKS. INSPECTED FITTINGS AND PLUMBING FOR DAMMAGE OR LOOSE FITTINGS. IAW 5PM. 20-22-00 AND 20-30-00. ALL INSPECTION CHECKED GOOD. OPERATED APU, OBSERVED FOR LEAKS, NO FLUID LEAKS PRESENT. MM 49·11·00.
CLOSING CABIN DOOR. DOOR ELEVATED ABOUT A FOOT OFF GROUND WHEN I FORGOT SOMETHING. RELEASED BLACK DOOR CLOSE BUTTON TO ALLOW DOOR TO FALL OPEN. WHEN DOOR REACHED FULL OPEN, HEARD A SNAP. PULLEY AT TOP OF STAIRS FOUND BROKEN & CABLE FRAYED & NOT IN PULLEY. NO FORCE APPLIED TO DOOR, LET DOOR FALL OPEN. IN THE PROCESS OF REMOVING MAIN DOOR CABLE, CABLE SNAPPED. TECHS WERE ABLE TO VERIFIED BINDING SOUND OF DOOR ACTUATOR. R & R DAMAGED CABLE PULLEY AND SAFETIED WITH NEW COTTER KEY. DISCONNECTED DOOR CABLE & R & R DAMAGED DOOR ACTUATOR. RECONNECTED DOOR CABLE & SAFETIED WITH NEW COTTER KEY. OPS CHECKED GOOD, NO DISCREPANCIES NOTED. WORK WAS DONE IAW MM 52-11-00, 52-11-26, & 52-11-25.
INTERIOR CABIN EMERGENCY LIGHTS ABOVE SEATS 2L AND 3R DON NOT ILLUMINATE WITH EMER LIGHT SWITCH PLACED TO ON FOR PREFLIGHT. GAINED ACCESS TO POWER SUPPLIES IN LOWER SIDEWALL AND BEHIND GALLEY AND SWAPPED POWER SUPPLIES PN 2013-1A SN 120510 AND PN 2013-1A SN 120296 AND FOUND NO CHANGE IN DISCREPANCY. REPLACED PREVIOUSLY SWAPPED POWER SUPPLIES INTO THEIR ORGINAL LOCATIONS WITH FURTHER INSPECTION FOUND ADDITIONAL EMERGENCY LIGHT POWER SUPPLY PN BR9767-001 SN C1473 INSPECTED UNIT AND FOUND CANNON PLUG DISCONNECTED. RESUCURED CONNECTOR PLUG TO SAID POWER SUPPLY AND PERMED OPERATIONAL CHECKS WITH NO DEFECTS NOTED ALL WORK COMPLETED REF CHALLEGER 350 AMM 33-51-17 AND 33-50-00-710-801.
UPPER EXIT SIGN AT THE TOP OF THE MED STAIRS DOES NOT ILLUMINATE WHEN SELECTED ON. UPON INVESTIGATING MAIN ENTRY DOOR UPPER EXIT SIGN LIGHT, WE DISCOVERED A LOOSE CONNECTION ON THE CONNCETORS ON THE BACK OF THE EXIT SIGN LIGHT. RECONNECTED THE CONNECTORS TO SECURE A PROPER MATE ON THE TERMINALS. PERFOMED AN OPERATIONAL CHECK OF THE EXIT SIGN LIGHT, OPERATIONAL CHECK GOOD. NO OTHER DISCREPANCIES NOTED.

LEFT ENGINE ENGINE COMPRESSOR STALL EVALUATE - ON TAKEOFF FROM TEB RUNWAY 24 WE EXPERIENCED TWO SUBSTANTIAL AIRFRAME SHUDDERS AFTER POWER REDUCTION AND DURING GEAR RETRACTION AT ABOUT 500' AGL 170KTS FLAPS 10. THERE WERE NO COCKPIT INDICATIONS OR WARNINGS. UPON RETURNING TO LAND AT TEB ALL SYSTEMS FUNCTIOND NORMALLY. GROUND PERSONNEL AND OBSERVING PILOTS AT TEB ADVISED US THEY HEARD THREE LOUD BANGS AND SAW SPARKS COMING FROM AIRPLANE ENGINE AREA DURING TAKEOFF. *** CORRECTIVE ACTION *** TROUBLESHOT #1 ENGINE BY OBTAINING DOWNLOADS OFF EEI. REVIEW OF THE ECFR EVENT DATA ON EEI REVEALED MCID 1805 UNEXPECTED ENGINE I SURGE FAULT. FOLLOWED ALL STEPS FOR TROUBLESHOOTING FROM TABLE 2027, FOUND NO EVIDENCE OF METALLIC DEBRIS ON FUEL FILTER OR OIL DEBRIS MONITIORS, REPLACED FILTER AND MONITORS. VERIFIED FREEDOM OF ROTATION OF THE NL AND N2 SPOOL. SPOOL ROTATION WAS NORMAL. PERFORMED BOROSCOPE OF FIRST STAGE LPC BLADES ALONG WITH PLUG POSITIONS 4,5,6 ON THE RIGHT SIDE OF THE ENGINE. ALL LPC BLADES AND BLADES AT BOROSCOPE PORTS WERE IN GOOD CONDITION WITH NO SIGNS OF DAMAGE. ABRAIDABLE MATERIAL IN GOOD CONDITION. A VISUAL INSP OF ALL THE CVG HARDWARE WAS PERFORMED ALONG WITH THE CONDITION OF THE CVG ARMS WITH SPHERICAL BEARINGS. NOTICED TWO CVG LINKAGE ARMS MISSING THE SPHERICAL BEARINGS COMPLETELY. REMOVED AND REPLACED ENGINE, ACCOMPLISHED POST INSTALLATION GROUND RUN FOR LEAK AND PERFORMANCE WITH NO DISCREPANCIES IAW AS907-2-1A LMM 72-05-22 AND AMM 71-00-00.
ADDED INFO: APPROACHING RWY 22R ON TAXIWAY N, CREW DETECTED AN UNUSUAL ODOR IN THE AIRCRAFT... POSSIBLY FM THE FLIGHT DECK VENTS OUTBOARD OF THE SEAT ARM RESTS. BOTH ENGINES & THE APU WERE RUNNING. THE ECS WAS ON THE APU. CREW & PAX OBSERVED LIGHT GRAY HAZE IN THE ENTIRE GALLEY AREA. EMERGENCY EVACUATION CONDUCTED PER THE QRC. ENGINES WERE SHUTDOWN VIA THE FIRE CONTROL PANEL. PAX EVACUATED. LINGERING ODOR DETECTED OUTSIDE THE AIRCRAFT BY CREW, PAX AND FIRE RESCUE PERSONNEL. CREW INITIALLY SUSPECTED AN ELECTRICAL ISSUE BUT THE UNUSUAL ODOR HAD A SOMEWHAT OILY SMELL. CORRECTIVE ACTION: R&R BLU-RAY DISC PLAYER BDP3009-001-001
BEHIND LEFT PILOT SEAT, A LOUD SQUEALING NOISE STARTED FOR ABOUT 10+ MINS THEN FOLLOWED BY A STRONG BURNING ODOR IN THE MAIN CABIN. THEN IT QUIT. TROUBLESHOT AND FOUND AVIONICS COOLING FAN TO BE THE SOURCE OF THE SQUEALING AND BURNING ODOR. REMOVED AND REPLACED THE COOLING FAN. ALL WORK PERFORMED IAW AMM 21-23-05. OPS CHECKED GOOD.
