Maintenance disclosures
DURING TAKEOFF, CREW NOTICED AN ELECTRICAL BURNING ODOR WITHOUT SECONDARY INDICATIONS. ELECTED TO RETURN TO THE HOSPITAL.
WHILE IN CRUISE FLIGHT, DETECTED A STRONG ELECTRICAL BURNING ODOR WITHOUT SECONDARY INDICATIONS. ELECTED TO RETURN TO BASE.
BEFORE LANDING CHECK, WE HAD A LG EXTEND UNSAFE INDICATED BY THREE GREEN LIGHTS & ONE RED UNLOCKED LIGHT ALL ILLUMINATED. WE PERFORMED THE EMER PROCEDURE & CYCLED THE HANDLE SEVERAL TIMES. WITH THE UNLOCKED LIGHT REMAINING ILLUMINATED, WE RETURNED TO BASE & HOVERED TO ALLOW THE MECHANIC TO ENSURE THE GEAR WAS DOWN & ALLOW HIM TO PIN THEM IN PLACE. AFTER THE GEAR WAS PINNED, WE LANDED WITH NO FURTHER INCIDENT.
FUEL ODOR IN CABIN DURING CRUISE FLIGHT WITH HEATER ON. MAINTENANCE REPLACED THE ADJUSTED VALVE ASSY ON THE NR 1 ENGINE.
RAISED GEAR AFTER TAKEOFF, RED GEAR LIGHT AND NOSE CENTERED LIGHT ILLUMINATED, EXTENDED GEAR, RED LIGHT OUT, CENTERING LIGHT STILL ON. RETURNED TO BASE. MAINTENANCE ADJUSTED CENTERING SWITCH.
NR 2 ENGINE GENERATOR FAILED IN FLIGHT. MAINTENANCE REPLACED THE STARTER GENERATOR.
WHILE IN CRUISE NR 1 GENERATOR FAILED. PERFORMED EMERGENCY PROCEDURES AND RETURNED TO MCY. MAINTENANCE FOUND NR 1 STARTER GEN DRIVESHAFT SHEARED.
