CAPTAIN INFORMED MX THAT THE UPON APPROACH TO LONG BEACH AIRPORT AND DURING GEAR EXTENSION, THE RIGHT HAND MAIN LANDING GEAR GREEN LIGHT DID NOT ILLUMINATE. THE CREW CHECKED AND CONFIRMED THAT THE GEAR WAS DOWN AND LOCKED, THE CREW HAD DECLARED EMERGENCY AND LANDED THE AIRCRAFT SAFELY.
WHILE CRUISING AT 38,000 FT. WE HAD A MACH TRIM FAIL INDICATION. AUTO PILOT KICKED OFF AND AIRCRAFT STARTED TO PITCH UP. WE NO TRIM CONTROL AND TOOK BOTH PILOTS TO PUSH THE YOKE FORWARD TO KEEP AIRPLANE LEVEL.
FLAPS WOULD NOT RETRACT AFTER TAKEOFF. AIRCRAFT DIVERTED FOR MAINTENANCE. FOUND DRIVE UNIT INOPERATIVE.
APPROX 11:30 PST, FLIGHT LEVEL 390, CAPTAIN REPORTED FLUCTUATING OIL PRESSURE, SECURED RT ENGINE AND DIVERTED. DISPATCHED MX CREW TO ACFT, INSP REVEALED OIL PRESSURE FITTING ON THE FWD 4 OCLOCK POSITION VIEWED FWD, HAD PULLED THE THREADS OUT OF THE HSG. THE PUMP HAD BEEN REPLACED 271 HOURS PREVIOUSLY, AND THE ACFT HAD EXPERIENCED AN IN FLIGHT PROBLEM WITH THE ACFT IN JANUARY 2010, 89 HRS PREVIOUSLY. MFG IS DOING A ROOT CAUSE ANALYSIS HOWEVER THE FIELD SUPPORT. REP DID NOT FEEL THESE WERE RELATED DUE TO THE AMOUNT OF TIME BETWEEN INCIDENTS. R & R PUMP ADJUSTED OIL PRESSURE AND RETURNED. ACFT TO SERVICE AFTER A CLEAN REPORT ON SOAP SAMPLE WAS RECEIVED.
PILOT REPORTED AT 40K FT, ABOUT 4 HOURS INTO THE FLIGHT BACK, WHEN THEY HAD A VIBRATION. PILOT NOTICED A SPLIT IN THE GENERATORS AND AN INCREASE IN VIBES ON THE RT ENGINE. AFTER A FEW MINUTES THE GENERATORS WENT BACK TO ABOUT A 5 VOLT SPLIT. AFTER ABOUT ANOTHER 15 MINUTES THEY LOST THE RT GENERATOR, A DESCENT TO FL200 WAS REQUESTED SO THEY COULD START THE APU AND GET A SECOND ELECTRICAL SYSTEM. WITH THE APU RUNNING THEY REQUESTED A CLIMB TO FL240 TO SAVE A BIT OF FUEL. IN THE CLIMB THEY LOST THE LT AND RT HYD SYS. THE AILERONS ARE BOOSTED AND THE AUX HYD SYS WAS OPERATING TO GEIVE TERM BOOSTED AILERONS AND WOULD GIVE THEM BREAKS WITHOUT ANIT-SKID ON LANDING. AT THE TIME OF THE HYD FAILURE THE WEATHER WAS CLEAR PILOT ELECTED TO TO LAND WHERE THEY HAD GOOD WEATHER, LONG RUNWAYS AND A REPAIR FACILITY. AN EMERGENCY WAS DECLARED THEY EXTENDED THE GEAR USING THE EMERGENCY SYS AND THEY MADE AN UNEVENTFUL LANDING. PILOT WAS ABLE TO CLEAR THE RUNWAY BUT COULD NOT TAXI AS THERE IS NO NOSE WHEEL STEERING WITH BOTH PRIMARY HYD SYSTEMS FAILED. RT ENG HAD A HYD LINE FAIL AND SPRAY FLUID INOT ENG COWLING, GENERATOR INGESTED FLUID AND FAILED HYD SYS EMPTIED CAUSING FAILURES LISTED ABOVE.
